Modified translabyrinthine approach and hearing preservation: imaging evaluation.

AIM: To establish if the computed tomography (CT) and magnetic resonance imaging (MRI) appearances of the vestibule, after the removal of vestibular schwannoma by a modified translabyrinthine approach, correlate with a successful outcome, defined as hearing preservation. MATERIALS AND METHODS: Our study group consisted of 16 patients with vestibular schwannoma. All patients' pre-operative hearing was graded as class one or two according to the Gardner-Robertson scale. On MRI scans, the schwannoma, including the intracanalicular segment, were less than 2 cm in size in all the patients. The int

Vertigo: a physiological approach.

An understanding of vestibular function and a careful history will quickly separate patients with disease of the vestibular system from others. Further history and physical examination, often coupled with audiometric and vestibular function tests, will aid in localizing the lesion to the central or peripheral portions of the system and in determining the affected side. Medical management, which is often nonspecific and involves considerable psychological support, usually suffices.

3D-FLAIR magnetic resonance imaging in the evaluation of mumps deafness.

A 6-year-old boy suffered acute profound right side deafness after his classmates had mumps. Although his salivary glands were not swollen, he had high levels of anti-mumps IgM and IgG antibodies. The three-dimensional fluid-attenuated inversion recovery (3D-FLAIR) procedure applied to magnetic resonance imaging (MRI) showed high signals in the right cochlea and vestibule. This indicated hemorrhage or a high concentration of protein in the right inner ear. This is the first case demonstrating a high 3D-FLAIR MRI signal of the inner ear in a patient with mumps deafness. Our findings suggest tha

Labyrinthine anomalies with normal cochlear function.

Three cases of labyrinthine anomaly confirmed by polytomography and CT scan are reported. They showed similar dysplasia of the bony labyrinth: dilation and fusion of the lateral semicircular canal (SCC) and of the vestibule with a normally shaped cochlea and other SCCs. One side was involved in 2 cases and both sides in 1 case. The 1st case showed normal hearing levels with markedly reduced response to caloric stimulation in the affected ear. The 2nd case showed conductive hearing loss due to cholesteatoma with normal bone conduction hearing levels and normal caloric response. The 3rd case sho

Review: morphological changes associated with endolymphatic hydrops.

Endolymphatic hydrops of the inner ear is identified as a swelling of the endolymphatic spaces. This morphopathology in man can only be confirmed at post-mortem examination although it is believed to underlie the auditory dysfunction and vestibular disturbances associated with Menière's disease. This is an illusive inner ear disorder characterized typically by the fluctuant hearing loss, tinnitus and episodes of vertigo. Menière's disease remains a major problem in otorhinolaryngology since the cause of the disease is not known and various treatments are recommended, often with unsatisfactory

Benign paroxysmal vertigo of childhood.

Benign paroxysmal vertigo of childhood is a vestibular disorder characterized by multiple sudden brief episodes of true vertigo with nystagmus usually beginning after the age of 4. The diagnosis is based on the characteristic history, because the otologic and neurologic examinations, electronystagmogram, audiogram, and electroencephalogram are usually normal. The etiology of the disorder remains unknown, although a vascular disturbance of the posterior circulation with ischemia of the labyrinth or vestibular nuclei is most commonly postulated. A review of the literature and five new cases are

Perspectives on the pharmacotherapy of vertigo.

Recent evidence from experiments in animals suggests a rationale for more effective pharmacologic and physical therapy in patients with vertigo. In particular, one must consider the possible effects of medications on the adaptive processes that naturally mediate recovery from vestibular lesions. The challenge now is for clinicians to devise medication and exercise regimens for patients with vestibular disturbances and critically (and quantitatively) evaluate their effects.

Vestibular and non-vestibular nystagmus in examination of dizzy patients.

Observation of nystagmus is of great importance in the otoneurologic examination. By means of electronystagmography it is possible to examine the nystagmus under different visual conditions such as in darkness or with eyes closed. Physiological nystagmus is characterized by miniature eye movements with an amplitude of 4-5 minutes of one degree. With fixation, the amplitude decreases and the frequency increases. Vestibular nystagmus is a biphasic eye movement with slow and rapid phases of opposite directions. The intensity increases when the eye bulb is directed in the same direction as the fas

Posturographic findings in two common peripheral vestibular disorders.

Peripheral vestibular pathology disturbs the vestibulospinal reflex in a number of cases. The recording of postural sway furnishes an evaluation of this vestibulospinal influence. Two common peripheral vestibular syndromes were studied: Ménière's disease and peripheral unilateral vestibular hypofunction. It is evident that, by posturography, vestibulospinal disturbance is shown in these peripheral syndromes, whereas its occurrence cannot be predicted by other tests, e.g. caloric and rotation tests. Normalization of the vestibulospinal reflex depends on central adapting mechanisms, which were e

Cogan's syndrome: a treatable cause of neurosensory deafness.

Cogan's syndrome, nonsyphilitic interstitial keratitis associated with vestibuloauditory deficits, can vary in its presentation. This paper describes an unusual case in which deafness developed while the patient was receiving topical therapy with prednisolone and homatropine for mild bilateral iritis and right-sided nodular episcleritis. The 10-year-old boy's hearing improved dramatically with high-dose systemic steroid therapy. The importance of being aware that deafness may occur in children undergoing treatment for "routine" iritis is emphasized.

Romberg and his test.

In the eponymous test described by Romberg in 1846, the erect patient is asked to close his eyes. If he falls, the test is positive and indicates the presence of a dorsal column lesion. Slight modifications have been described in the test but in all cases, great care must be taken to do it carefully. The test compares the stability of eye-opening with that of eye-closure. A positive test does not indicate vestibular or cerebellar disease.

Do Filling Defects of the Vestibule Represent a Neuroradiological Marker for Congenital Cytomegalovirus? A Retrospective Case Series.

INTRODUCTION: The aim of this study was to investigate whether radiological marker(s) of the inner ear can be detected in congenital cytomegalovirus (cCMV) patients with severe-profound sensorineural hearing loss. METHODS: A retrospective imaging review of confirmed cCMV paediatric patients that had undergone consecutive cochlear implantation was performed at a tertiary hospital. Available pre- and postoperative imaging was examined, and abnormalities of the labyrinth were catalogued by a consultant neuroradiologist in the study group and control group. RESULTS: Twenty-eight paediatric patie

The Correlation of a 2D Volume-Referencing Endolymphatic-Hydrops Grading System With Extra-Tympanic Electrocochleography in Patients With Definite Ménière's Disease.

Background: Although magnetic resonance imaging (MRI) of the membranous labyrinth and electrocochleography (ECochG) have been used to diagnose endolymphatic hydrops (ELH) in patients with Ménière's disease (MD), the relationship between imaging and ECochG is not well-documented. Objectives: This study evaluates the ELH using 3D-FLAIR MRI and extra-tympanic ECochG (ET-ECochG) and correlates the results from 3D-FLAIR MRI to those from ET-ECochG. Materials and Methods: 3D-FLAIR MRI images of 50 patients were assessed using a 2D volume-referencing grading system (VR scores, relative scores accordi

Restoring vestibular function during natural self-motion: Progress and challenges.

The vestibular system is integral to behavior; the loss of peripheral vestibular function leads to disabling consequences, such as blurred vision, dizziness, and unstable posture, severely limiting activities of daily living. Fortunately, the vestibular system's well-defined peripheral structure and well-understood encoding strategies offer unique opportunities for developing sensory prostheses to restore vestibular function. While these devices show promising results in both animal models and implanted patients, substantial room for improvement remains. Research from an engineering perspectiv
