Residential placement for veterans with addiction: American Society of Addiction Medicine criteria vs. a veterans homeless program.

The goal of this study was to compare placements of patients with addiction undertaken by a) a unidimensional, protocol-driven, independent "permanent" housing "wet" program versus b) a multidimensional, patient-individualized, contingency-based housing approach. The sample consisted of eight veterans in a single team's panel admitted to a housing program and eight matched veterans on the verge of homelessness placed by the team according to the American Society of Addiction Medicine (ASAM) criteria. The two groups (matched for sex, race-ethnicity, and age [SD, 5 years]) were similar on demogr…

Use of and Outcomes in Veterans Affairs and Other Hospitals Among Veterans Experiencing Homelessness After Expanded Access.

OBJECTIVE: Veterans in the U.S. Department of Veterans Affairs (VA) health care system experiencing homelessness have high rates of hospitalization. The 2014 Veterans Choice Act and Medicaid expansion programs under the Affordable Care Act expanded access to VA-purchased care and Medicaid for veterans. The authors aimed to examine-in VA and non-VA hospitals-changes in medical-surgical and psychiatric hospitalizations and hospital outcomes after these expansions for homeless veterans versus other veterans. METHODS: A longitudinal study (2012-2017) of veterans in 11 states was conducted. Non-VA…

Evaluating the accuracy of the Veterans Health Administration's REACH VET suicide prediction model for legal involved veterans.

The Veterans Health Administration's (VA) Recovery Engagement and Coordination for Health-Veterans Enhanced Treatment (REACH VET) suicide prevention program uses a model that estimates every patient's suicide risk. We evaluated predictive accuracy of that model for death by suicide, and separately for the combined outcome of suicide attempt or death, for all VA patients and for patients with criminal legal system involvement who have one of the highest suicide rates among VA users. We found that the model has low accuracy for the prediction of death by suicide (positive predictive value = 0.05…

Comparison of Bone Mineral Density Between Veterans and Non-Veterans and Its Impact on Fracture Risk Assessment.

Background and Objectives: Osteoporosis poses significant health risks, especially among veterans, due to lifestyle factors. This study compares bone density and fracture risks between male veterans and non-veterans. Materials and Methods: We conducted a retrospective analysis of 1427 veterans from the Taichung Veterans General Hospital osteoporosis database (2010-2022), matched 1:1 by age and gender with non-veterans. Bone mineral density (BMD) was measured via dual-energy X-ray absorptiometry (DXA). Comorbidities, fracture sites, and mortality data were tracked. Conditional logistic regressi…

Use of General Primary Care, Specialized Primary Care, and Other Veterans Affairs Services Among High-Risk Veterans.

IMPORTANCE: Integrated health care systems increasingly focus on improving outcomes among patients at high risk for hospitalization. Examining patterns of where patients obtain care could give health care systems insight into how to develop approaches for high-risk patient care; however, such information is rarely described. OBJECTIVE: To assess use of general and specialized primary care, medical specialty, and mental health services among patients at high risk of hospitalization in the Veterans Health Administration (VHA). DESIGN, SETTING, AND PARTICIPANTS: This national, population-based,…

Prevalence of chronic obstructive pulmonary disease and tobacco use in veterans at Boise Veterans Affairs Medical Center.

BACKGROUND: Although its prevalence is still debated, chronic obstructive pulmonary disease (COPD) is a leading cause of morbidity and mortality in the United States, and smoking cessation remains the only intervention that can significantly improve the prognosis of COPD. METHODS: To determine the prevalence of COPD in a typical population seeking care at a Veterans Affairs Medical Center; the impact of smoking, age, and sex on the prevalence of COPD in this population; and how often spirometry is done in patients at risk for COPD, we extracted data from the Veterans Integrated Service Networ…

Exploring PrEP Uptake in Young Women Veterans: Insights From a Detailed Chart Review Within the Veterans Health Administration.

INTRODUCTION: Uptake of HIV pre-exposure prophylaxis (PrEP) within the US Veterans Health Administration (VHA) remains modest, especially among younger women. Younger women face a disproportionately high risk of HIV infection due to biological, behavioral, and structural factors and experience the greatest barriers to PrEP access compared with older women. To examine gaps in PrEP delivery for this high-risk group, we conducted in-depth chart reviews of patients younger than 26 years identified as female at birth. METHODS: Among all VHA patients prescribed PrEP from 2012 to 2022, 24 were young…

Loneliness Among US Veterans With Problematic Substance Use: Results From the National Health and Resilience in Veterans Study.

OBJECTIVES: Loneliness is a pervasive public health problem, especially among individuals with problematic substance use (PSU). To date, however, scarce research has examined the prevalence and correlates of loneliness in vulnerable segments of the population, such as US military veterans. METHODS: Data were analyzed from the National Health and Resilience in Veterans Study, which surveyed a nationally representative sample of US veterans. The Three-Item Loneliness Scale was used to assess clinically significant loneliness (≥6). Multivariable logistic regression and relative importance analys…

Intimate partner violence in United States military veterans: Results from the National Health and Resilience in Veterans Study.

Intimate partner violence (IPV) is a significant concern among military personnel and veterans, yet few studies have examined its prevalence and correlates in nationally representative samples of veterans. This study aimed to address this gap by identifying the prevalence of IPV and factors associated with lifetime physical IPV perpetration, expanding on prior research by including a broader range of traumatic experiences (e.g., military sexual trauma) and psychiatric variables (e.g., nicotine dependence, non-suicidal self-injury). Results revealed that, of 2376 veterans surveyed, 208 (weighte…
