Test-retest reliability in performance of persons with hemiparesis tracking by means of compatibly displayed myoelectric feedback derived from upper limb muscles.

The aim of this study was to assess whether persons with hemiparesis will yield statistically reliable test-retest tracking performance on a procedure using limb-generated, compatibly displayed, myoelectric video feedback. A convenience sample of 50 inpatients and outpatients with upper extremity involvement of at least six months were recruited. 30 had hemiparesis and had both upper extremities tested. 20 had hemiplegia and had the nonparetic upper extremity tested. Tracking skill was measured as mean tracking error. Repeated measures analysis of variance yielded statistically significant eff

Cognitive-behavioral treatment of social phobia: new advances.

Cognitive-behavioral treatment (CBT) for social phobia is an effective treatment for many patients, but some patients do not benefit from the treatments and many remain symptomatic. Therefore, researchers have been examining techniques that may improve treatment outcome. In this paper, recent psychopathology and treatment outcome research, much of which supports the expectation that a second-generation CBT treatment may further improve outcome, are discussed. Finally, the authors present a number of CBT techniques that are tailored for the individual treatment of patients with social phobia. T

Assessment of work postures and movements using a video-based observation method and direct technical measurements.

The aim was to study postures and movements during repetitive work using video-based observations and direct technical measurements (inclinometers and goniometers). A total of 21 healthy women from a poultry processing plant volunteered. Neck flexion > 20 degrees was registered during 92% of the recorded time with the observation method, while the corresponding value measured with the inclinometer was 65%. Different reference positions and different measured variables apparently contributed to the differences between the methods. Mean wrist position was measured to be 0 degrees in flexion-exte

Effect of localized muscle fatigue on vertical ground reaction forces and ankle joint motion during running.

The purpose of this study was to examine the changes in the vertical ground reaction force (VGRF) and ankle joint motion during the first 50% of the stance phase of running following fatiguing exercise of either the dorsiflexors or the invertors of the foot. VGRFs, sagittal and rearfoot kinematic data were collected from 11 female recreational runners running at 2.9 m/second on a treadmill prior to and following localized muscle fatigue of either the invertors or dorsiflexors of the right foot. Loading rate of the impact peak force significantly increased following fatiguing exercise of the do

New insights into the clinical management of partial epilepsies.

The diagnosis, treatment, and prognosis of seizure disorders depend on the correct identification of epileptic syndromes. Partial epilepsies are heterogeneous and can be divided into idiopathic, cryptogenic, and symptomatic epilepsies. The most common of the idiopathic localization-related epilepsies is benign epilepsy with rolandic or centrotemporal spikes (BECTS). Seizures remain rare and the use of antiepileptic drug (AED) treatment in all patients does not appear justified. Children who present with some of the electroclinical characteristics of BECTS may also display severe unusual neurol

Kinematic and dynamic characteristics of solid pellet movement during the pharyngeal phase of swallowing.

Information on solid particle movement through the pharynx, in addition to its physiologic importance, has relevance to the swallowing of medications in pill form. The purpose of this study was to determine the kinematics of a solid particle during the oral-pharyngeal phase of swallowing. We used a concurrent manometric-videofluoroscopic technique and identified two distinct zones of increasing bolus velocity, one at the tongue base and the other at the pharyngo-upper esophageal sphincter (UES) region. Velocity decreased significantly (p < .05), to 9.0 +/- 1.0 cm/s, while the bolus traversed t

Defining and observing social signals in deaf and hearing infants.

Infant gaze, gestures, and affective expression have become generally accepted as indicators of the infant's efforts to initiate or resume interaction with a partner, particularly during moments when the mother may be temporarily unresponsive as shown experimentally in the maternal "Still-Face" situation. Previous studies comparing deaf and hearing infants using this paradigm have revealed diminished signalling by deaf infants with hearing mothers, when signals were defined by the typical indices mentioned above. This study compares results from both a microanalytic coding system (used with 59

[Neuropsychological analysis of transitory partial performance disorders. A video and MRI supported study].

Apraxia is an impairment of voluntary movements not caused by paresis, sensory deficits or aphasia. It appears mainly after left-hemispheric parietal or callosal lesions. Our patient had a temporary apraxia that could be correlated with the development of the perifocal area around a left parietooccipital cerebral bleeding. These neuropsychological disorders belonged to the development and the disappearance of the perifocal area as seen via several MRI.

How reliable is measurement of the anorectal angle by videoproctography?

The anorectal angle can be determined either by constructing a straight line along the lower border of the rectum (Method A) or by using the central longitudinal axis of the lower rectum (Method B). We have used a computer program to derive the centroid of the rectum for Method B. The coefficients of variation for angles measured at rest, during maximum pelvic floor contraction, and during attempted defecation were 0.616, 0.351, and 0.358, respectively, compared with 0.993, 0.972, and 0.968 for Method B. The presence of a rectocele had no influence on the measurement of the anorectal angle in

Simultaneous recordings of force and sliding movement between a myosin-coated glass microneedle and actin cables in vitro.

To elucidate the molecular mechanism of muscle contraction resulting from the ATP-dependent actin-myosin interaction, we constructed an assay system with which both the force and the movement produced by the actin-myosin interaction in vitro can be simultaneously recorded and analyzed. The assay system consisted of the giant internodal cells of an alga, Nitellopsis obtusa, which contain well-organized arrays of actin filaments (actin cables) running along the cell long axis, and a glass microneedle (tip diameter, approximately 7 microns; elastic coefficient, approximately 40 pN/microns), which

Impaired reception of nonverbal cues in women with premenstrual tension syndrome.

The ability to perceive nonverbal facial cues was tested in 30 White, middle-class females. Pre- and post-menses responses of 15 subjects diagnosed as having Premenstrual Tension Syndrome (PMS) were compared to the responses of age-matched controls. Each subject was asked to interpret videotaped facial cues of individuals engaged in a gambling task. Interpretive ability fluctuated during the menstrual cycle and was significantly impaired during the premenstrual phase.

Video vignettes do not yield higher engagement compared to a written variant in a vignette experiment to test communication in healthcare.

OBJECTIVES: Experimental vignette designs are used to systematically test communication strategies in healthcare. To ensure validity, participants of such experiments need to optimally engage with the hypothetical scenario. Researchers have often favored video over written vignettes to maximize engagement, despite a lack of evidence for the superiority of videos. We sought further evidence to substantiate the choice for a vignette modality by testing the impact of video versus written vignette modality on engagement, perceived realism and anxiety among cancer patients and survivors. METHODS:

Esophageal speech: double-contrast evaluation of the pharyngo-esophageal segment.

Alaryngeal voice is usually accompanied by esophageal speech; however, about 40% of laryngectomy patients are unable to achieve adequate esophageal speech and must rely on mechanical devices for communication. A technique was developed for performing double-contrast studies of the hypopharynx, cervical esophagus, and pharyngo-esophageal segment using thick barium and the air normally injected for speech. Simultaneous audio and video recordings obtained during esophageal speech allowed correlation of the quality of speech with the motion of the pharyngo-esophageal segment. In 35 patients with v

Competence and Adherence Scale for Cognitive Behavioral Therapy (CAS-CBT) for anxiety disorders in youth: Psychometric properties.

The aim of the present study was to evaluate the psychometric properties of the Competence and Adherence Scale for Cognitive Behavioral Therapy (CAS-CBT). The CAS-CBT is an 11-item scale developed to measure adherence and competence in cognitive-behavioral therapy (CBT) for anxiety disorders in youth. A total of 181 videotapes from the treatment sessions in a randomized controlled effectiveness trial (Wergeland et al., 2014) comprising youth (N = 182, M age = 11.5 years, SD = 2.1, range 8-15 years, 53% girls, 90.7% Caucasian) with mixed anxiety disorders were assessed with the CAS-CBT to inves

Conversations between persons with dementia disease living in nursing homes and nurses--qualitative evaluation of an intervention with the validation method.

Living with dementia disease (DD) can include difficulties describing experiences of everyday lives, which can lead to withdrawal, social isolation or existential homelessness. Persons with DD living in nursing homes are mainly dependent on the nurses for establishing and maintaining relationships with those around them. It can be challenging for nurses to understand what a person with DD is trying to express and to make themselves understood in turn. The validation method is intended to facilitate communication with persons with DD, but to our knowledge, there have been no qualitative studies

Novel technique of auto-tunneling during peroral endoscopic myotomy (with video).

BACKGROUND: Peroral endoscopic myotomy (POEM) was developed to reduce lower esophageal sphincter pressure in patients with achalasia. POEM is technically challenging and time consuming. The creation of an esophageal submucosal tunnel is a major and integral part of the procedure. Dissection of the submucosal fibers is a lengthy task. OBJECTIVE: To present our initial experience with the use of a novel gel with dissecting properties for facilitating submucosal tunneling during POEM. SETTING: Johns Hopkins Hospital. INTERVENTION: POEM. RESULTS: The gastroscope was successfully introduced int

Hot water epilepsy: a video case report of a Caucasian toddler.

We present the case report of a 13-month-old Caucasian toddler with symptoms of loss of consciousness, central cyanosis and uncontrolled movements of the upper limbs while taking a warm bath. The diagnosis of hot water epilepsy was supported by an ictal EEG. Hot water epilepsy, also known as bathing epilepsy or water-immersion epilepsy is, in the Caucasian population, a rare form of benign epilepsy, where seizures are provoked by immersion in a hot or even just a warm bath. This is the first comprehensive video publication of a seizure provoked by water-immersion in a Caucasian child. [Publish

Task-specific craniocervical dystonia.

The pathophysiology of dystonia is incompletely understood. Unlike many other focal dystonias, cervical dystonia, a frequent dystonia leading to twisting of the head, does not appear to be related to overuse or acquisition of a demanding motor skill. Here, we report development of task-specific dystonia of the neck muscles in a 67-year-old patient following bilateral traumatic arm amputation at the age of 15. To compensate for the amputation, the patient learned to write with a pen held in his mouth. After several years of practicing this unusual and demanding skill, symptoms of task-specific
