Secondary tracheoesophageal puncture with a flexible transillumination device: a new technique and its outcomes.

OBJECTIVES: This study aims to describe a new secondary tracheoesophageal puncture technique with voice prosthesis insertion under local anesthesia with a low-cost device and evaluate its outcome. METHODS: With a transoral flexible transillumination device of aluminum of 2.3 mm with fiberoptic light, the esophagus's anterior wall is visualized through the tracheostomy. A tracheoesophageal fistula is made with a blade and passed through the fistula. Then a bent trocar is introduced into the lumen using the device as a guidewire. Once in the lumen, a thin guide wire is passed through up to the

Revision of failed Rubis II prostheses with a dual-mobility Touch implant.

OBJECTIVES: Trapeziometacarpal osteoarthritis is the most prevalent upper limb arthropathy. Total joint arthroplasty has established itself as a trapezium-preserving alternative to trapeziectomy, but single-mobility implants carry a significant risk of dislocation. When revision is required, conversion to a dual-mobility implant is an appealing option, yet no published series has specifically evaluated the outcomes of this procedure. This study aimed to assess implant survival, functional outcomes and complications after revision trapeziometacarpal arthroplasty by conversion from the single-mo

Retinal Prostheses: Engineering and Clinical Perspectives for Vision Restoration.

A retinal prosthesis, also known as a bionic eye, is a device that can be implanted to partially restore vision in patients with retinal diseases that have resulted in the loss of photoreceptors (e.g., age-related macular degeneration and retinitis pigmentosa). Recently, there have been major breakthroughs in retinal prosthesis technology, with the creation of numerous types of implants, including epiretinal, subretinal, and suprachoroidal sensors. These devices can stimulate the remaining cells in the retina with electric signals to create a visual sensation. A literature review of the pre-cl

The M6-C Cervical Disk Prosthesis: First Clinical Experience in 33 Patients.

STUDY DESIGN: Retrospective study. OBJECTIVE: To determine the short-term clinical succesrate of the M6-C cervical disk prosthesis in primary and secondary surgery. SUMMARY OF BACKGROUND DATA: Cervical disk arthroplasty (CDA) provides an alternative to anterior cervical decompression and fusion for the treatment of spondylotic radiculopathy or myelopathy. The prevention of adjacent segment disease (ASD), a possible complication of anterior cervical decompression and fusion, is its most cited--although unproven--benefit. Unlike older arthroplasty devices that rely on a ball-and-socket-type de

Classifying prosthetic use via accelerometry in persons with transtibial amputations.

Knowledge of how persons with amputation use their prostheses and how this use changes over time may facilitate effective rehabilitation practices and enhance understanding of prosthesis functionality. Perpetual monitoring and classification of prosthesis use may also increase the health and quality of life for prosthetic users. Existing monitoring and classification systems are often limited in that they require the subject to manipulate the sensor (e.g., attach, remove, or reset a sensor), record data over relatively short time periods, and/or classify a limited number of activities and body

Fungal endocarditis at the aortotomy site after aortic valve replacement.

This brief report presents a patient with fungal endocarditis involving the suture location in the ascending aorta after aortotomy to replace a stenotic aortic valve. It emphasizes the importance of expanding the diagnostic investigation for endocarditis beyond the prosthesis, which was normal in this case. It also reiterates the value of transesophageal echocardiography during the evaluation of prosthetic valves and reminds the echocardiographer to visualize the aortotomy site carefully.

Improvement in reading performance through training with simulated thalamic visual prostheses.

Simulations of artificial vision are used to provide the researcher an opportunity to explore different aspects of visual prosthesis device design by observing subject performance on various tasks viewed through the simulation. Such studies typically use normal, sighted subjects to measure performance at a given point in time. Relatively few studies examine performance changes longitudinally to quantitatively assess the benefits from a training plan that would be akin to post-implantation rehabilitation. Here, we had six normal, sighted subjects use a standard reading task with daily practice …

[Keratoprosthesis with biocolonizable support].

Keratoprosthesis consists in replacing the central cornea with an optical system. The optic requires a support which was usually located in front of or behind the cornea. In our opinion microporous materials can hold the prosthesis in place and prevent melting and leakage. The prosthesis is implanted in a single procedure. The optical core is made of clinical quality PMMA. The core is 5 mm in diameter and 2.5 mm long. To promote tissue stability, 9 mm of a new microporous fluorocarbon (200 microns thick and 80 microns pore size) is used. The material becomes transparent and wettable after impl
