Fourteen patients with extramammary Paget's disease of the vulva treated at Yale-New Haven Medical Center from 1982 through 1993 were reviewed to evaluate the accuracy of methods used to delineate surgical margins and to determine if radical operations or surgical margin status was associated with likelihood of recurrence. A 72-year-old female with a complaint of hemorrhagic leucorrhea was referred to our department. She had a black tumor on her urethral meatus suspected as a malignant melanoma. Urethrectomy, vulvectomy, partial vaginectomy, bilateral inguinal lymphadenectomy, and cystostomy were performed. Pathological diagnosis was malignant melanoma of the urethra. Local recurrence and lymph node metastasis were found 9 months later, and she has been given DAV-feron combined therapy.

A 58-year-old woman presented with typical cutaneous features of lichen planus (LP). She had suffered from vulval pruritus for 3 years, and examination showed lesions suggestive of LP on the vulva. Three months later a plaque developed on the clitoral hood, and biopsy showed an invasive squamous carcinoma. After vulvectomy, features of LP were found in the excised specimen. Vulval LP should be carefully monitored for possible malignant transformation.

The perineum is an area of rich blood supply with multiple arterial anastomoses. Flaps raised on perforators around the perineum resemble the petals of the lotus and can be used to reconstruct a variety of vulvo-vaginal defects. Thirteen such flaps have been used successfully without any loss of flaps in eight patients. Eight flaps in four patients were used for vulvar reconstruction following radical vulvectomy.

We report the first case of detection of sentinel node in a 54 year-old woman presenting an adenocarcinoma of Bartholin's gland. Primary carcinoma of Bartholin's gland is rare and represents 2-7% of vulvar malignant lesions; this could explain the lack of consensus about treatment. The best attitude could be vulvectomy and inguinal lymphadenectomy. Pelvic lymphadenectomy is not required when no pelvic sentinel node is observed or when no metastatic inguinal node can be detected.

A woman with stage I squamous carcinoma of the vulva associated with diffuse lichen sclerosus was treated with radical vulvectomy plus inguinofemoral and pelvic lymphadenectomy. This procedure was followed by vulvar reconstruction using bilateral gracilis myocutaneous grafts. Two years later the previously normal grafted skin had developed lichen sclerosus. This occurrence is unique and completely unexpected in view of the graft technique, which preserves the original blood supply and deep dermal tissues.

BACKGROUND: Vulvar masses in adolescents have a broad differential diagnosis, yet few reports exist detailing masses of mammary origin.
CASE: A nulliparous, healthy 16-year-old adolescent presented with a longstanding, ulcerated, 17-cm vulvar mass of unknown origin and pronounced inguinal lymphadenopathy. The patient underwent a left radical partial vulvectomy, with pathology revealing terminal duct lobular units consistent with polymastia.
CONCLUSION: Differential diagnosis of a vulvar mass in an adolescent should include polymastia.

BACKGROUND: Basosquamous cell carcinoma (BSC) of the vulva is a rare entity with interesting prognostic and therapeutic implications. Currently, there is no definitive treatment due to its low incidence.
CASE: A case of recurrent BSC of the vulva treated with unilateral radical vulvectomy and unilateral lymph node dissection is reported.
CONCLUSION: BSC is a rare disorder of the vulva. The metastatic potential of this tumor is not fully understood, but likely is intermediate between squamous cell carcinoma and basal cell carcinoma. Local recurrence is common and close follow-up is warranted.
