PURPOSE: To review detailed ophthalmic examinations of myotonic dystrophy patients especially with regard to refractive error and ocular alignment.
METHODS: Retrospective chart review of detailed ophthalmic examinations of 32 patients (27 children and 5 adults) with myotonic dystrophy.
RESULTS: The majority (86%) of eyes were hypermetropic. The average degree of hypermetropia was +6.0 D for children and +2.10 D for adults: 56% of children had esotropia; 22% had amblyopia.
CONCLUSIONS: Children with myotonic dystrophy should receive detailed eye examinations with emphasis placed on recogniti…

In the ophthalmological field, it is not always easy to adopt a primary prevention. Much more useful would be a secondary prevention program towards the affections at birth or those later developed that, if diagnosed and treated early, grant a normal visual development. Differentiated prevention can be carried out, according to various ages: at the tender age congenital glaucoma and cataract, strabism, ametropia, anisometropia (possible cause of amblyopia) have to be considered; in the adult-senile age must be prevented damages due to hypertension, diabetes, thrombosis, as well as cataract, gl…

Results of pathogenetical surgical treatment of congenital glaucoma were analyzed in late periods after surgery, from 1 to 17 years, in 105 patients (171 eyes). Ophthalmic tone normalization was attained in 87.5% of cases. High vision acuity was retained in 57.7% of cases. The principal causes of reduced central vision in process stabilization were astigmatism in 61.7%, amblyopia in 71.5%, strabismus in 59%, anisometropia in 22.8%. To improve vision acuity computer-aided pleoptic treatment was carried out in the postoperative period in 42 children (76 eyes). The efficacy of the method was 84.2…

The effects of long-term monocular lid-suture deprivation on visual-evoked cortical potentials (VEPs) and flash- and pattern-evoked electroretinograms (FERGs and PERGs, respectively) were assessed in the cat. VEPs were virtually eliminated when recorded with the deprived eye, indicating that the lid suture produced a severe amblyopia in that eye. In contrast, FERGs and PERGs were more similar for both deprived and nondeprived eyes and comparable to those recorded in normal animals. The current findings demonstrate that long-term deprivation (3-4 yr) does not produce systematic changes in the e…

Cataract surgery was performed in six eyes of four patients with typical colobomata of the iris, choroid, and retina. Contrary to previous reports of poor surgical results in these eyes, we saw no surgical or postoperative complications. Final visual acuity was excellent in four eyes. The other two had preexisting amblyopia. Most previous reports describe outmoded extracapsular techniques or contemporary intracapsular cataract extraction, all without an intraocular lens. Five of the six eyes in this study had modern cataract surgery--phacoemulsification with a posterior chamber lens placed in …

A 30-year-old obese patient with a rapidly progressing chiasmal syndrome presented with acute bitemporal hemianopia, severe bilateral amblyopia, mental confusion, and lethargy. X-ray films of the skull showed a normal sella turcica and computed tomography scan as well as angiography revealed a vascular mass within the chiasmatic cistern. At operation, via left subfrontal approach, an intrachiasmal hematoma was evacuated and biopsies of the hematoma cavity revealed a cavernous hemangioma. Visual symptoms improved markedly postoperatively. This rare association of chiasmal apoplexy and intrachia…

Two infants, aged three days and six weeks, respectively, presented with dense extensive persistent pupillary membranes as sporadic isolated findings. A clear central pupillary aperture was unable to be achieved with mydriatic agents. The patients were treated by surgical removal of the membranes at age five days and six months respectively. Surgery was without complication and visual development has been normal. We suggest that early intervention in the first weeks or months of life is necessary to prevent the development of amblyopia. If mydriasis is unsuccessful, surgery is recommended foll…

BACKGROUND: This study aimed to evaluate the factors influencing final visual acuity in pediatric traumatic cataracts.
METHODS: Data of patients who presented with traumatic cataracts were reviewed retrospectively. We evaluated age at trauma; gender, trauma type, cause, and zone; duration between the time of trauma and cataract surgery; surgical method used; time, location, and type of intraocular lens (IOL) implantation; initial and final best corrected visual acuity (BCVA); amblyopia rate; and complications.
RESULTS: In all, 61 eyes of 59 patients aged < 16 years with cataracts after traum…

PURPOSE: The visual status of adolescents in Saudi Arabia (SA) has not been well reported. To date, the prevalence and types of refractive errors (REs), amblyopia, strabismus, and correctable visual impairments have not been quantified. The aim of the study was to investigate the visual status in adolescents in Riyadh, SA.
METHODS: This study was based on a population cross-sectional and random cluster design. After design and the sample calculations, 1,007 participants, 12-20 years of age, were screened during the study. Nine participants were excluded due to ocular disorders. The participan…

BACKGROUND: Periocular hemangiomas can induce irreversible amblyopia by multiple mechanisms: visual deprivation, refractive error (astigmatism and/or anisometropia), or strabismus. There is a subset of complicated periocular hemangiomas most effectively managed by resection.
METHODS: The authors reviewed all patients from 1999 to 2008 with a periocular hemangioma that was either completely resected or debulked; whenever necessary, the levator apparatus was reinserted. Infants were included in the study if they had complete preoperative and postoperative ophthalmic assessments and there was mo…

Data regarding onset (noticed age), type, head turn, primary position deviation, narrowing of the palpebral fissure, leash phenomenon, refractive error, amblyopia, and history of surgery were collected. Chi-square test and Student's t-test were used.

BACKGROUND: Epikeratophakia is a potentially reversible corneal surgical procedure that can correct refractive errors in children who are aphakic and poor candidates for intraocular lens implantation. The correction of aphakia in the pediatric population poses specific problems because of associated amblyopia.
METHODS: The clinical records of 61 consecutive patients (82 eyes) treated for pediatric aphakia by epikeratophakia were reviewed retrospectively with a follow-up of 3 to 5 years. For the purpose of analysis, the patients were divided into seven groups.

Down Syndrome is one of the most common chromosomal conditions in the world, affecting an estimated 1:400-1:500 births. It is a multisystem genetic disorder but has a wide range of ophthalmic findings. These include strabismus, amblyopia, accommodation defects, refractive error, eyelid abnormalities, nasolacrimal duct obstruction, nystagmus, keratoconus, cataracts, retinal abnormalities, optic nerve abnormalities, and glaucoma. These ophthalmic conditions are more prevalent in children with Down Syndrome than the general pediatric population, and without exception, early identification with th…

An 8-year-old girl had an orbital-adnexal lymphangioma and ipsilateral orbital and middle cranial fossa arteriovenous malformations. High-resolution magnetic resonance image scanning, orbital ultrasonography, and digital subtraction angiography were used for diagnosis and preoperative assessment. Complications related to this vascular neoplasm included amblyopia, acute hemorrhage with proptosis, exposure keratitis, cosmetic deformity, and recurrent preseptal cellulitis. The girl was treated with both embolization and orbital surgery for recurrent hemorrhage and proptosis. We postulated that th…
