[The mechanism of the chronic supraventricular tachycardia: an intracavitary electrophysiological study (author's transl)].

Chronic Supraventricular Tachycardia (CST) or Chronic Atrial Tachycardia is a relatively rare arrhythmia. Because of very poor number of cases studied with electrophysiologic techniques its mechanism is still debated. Three cases of CST, one of persistent type and two of repetitive type, are reported. His bundle electrogram, high and low right atrial electrograms and atrial stimulation were performed in every case, ventricular stimulation in only one case. The Amiodarone, Pindolol, Verapamil and vagal manoeuvres effects were evaluated in all cases. The site of origin of tachycardia, atrial in

[Paroxysmal junctional tachycardia. Determination of the inferior point of junction of the reentry circuit. Dissociation of the intra-nodal reciprocal rhythms].

Stimulation of the bundle of His and of the uppermost portion of the interventricular septum gives us an opportunity to make a precise study of capture phenomena in patients with paroxysmal nodal tachycardia. According to whether the capture is correctly timed, delayed, or unusually premature, the inferior junction point of the reentry circuit can be located precisely by reference to the H wave and the onset of the R wave. Out of a series of 65 patients, only 30 of whom had a true WPW syndrome, it was shown that 43 cases had a bundle of Kent which ensured retrograde conduction during the tachy

Bradyarrhythmias in Cardiac Amyloidosis and Role of Pacemaker.

Cardiac amyloidosis (CA) is an underdiagnosed disease that is caused by deposition of misfolded transthyretin (ATTR) or immunoglobulin light chain (AL) fibrils in the myocardium. Bradyarrhythmias are commonly seen in CA, due to disruption of conducting system by amyloid fibrils. Atrioventricular conduction defect is more common than sinus node dysfunction. Bradyarrhythmias are most prevalent in wtATTR, followed by hATTR and AL. Pacemaker implantation, when indicated, can help provide symptomatic relief but does not confer mortality benefit. Progression of conduction system disease is common an

Successful radiofrequency ablation of swallowing-induced atrial tachycardia arising from left superior ganglionated plexus.

A man in his early 40s developed palpitations brought on by swallowing and was found to have short runs of atrial tachycardia induced by swa … iminated the paroxysmal atrial tachycardia at this site. Interestingly, in the process of ablation, atrioventricular node reentrant tachycardia was triggered. After the slow-pathway ablation procedure, no further tachycardia was induced.

Long-term Outcomes in Treated Lyme Carditis.

Lyme disease is the most reported tick-borne illness in North America. Lyme carditis (LC) is an early-disseminated manifestation of Lyme disease, most commonly presenting as symptomatic high-degree atrioventricular block (AVB) which resolves with appropriate antibiotic therapy. However, long-term outcomes of treated LC have not previously been described. We present a series of 7 patients (median 28 years, 6 male) with serologically confirmed LC treated with a standard protocol developed at our center including antibiotics and pre-discharge stress test to assess AV node stability. At a mean fol

Supraventricular tachycardia diagnosed by smartphone ECG.

Diagnosis of paroxysmal supraventricular tachycardia (PSVT) may be difficult due to its episodic nature, which can be brief and self-limited, limiting the ability for clinicians to diagnose the specific rhythm disorder in a timely manner. We present a case of PSVT, which was unable to be diagnosed through typical evaluation with an event monitor despite several years of symptoms. The patient was ultimately diagnosed using the AliveCor Mobile ECG, a smartphone-based ECG device and application, which he purchased himself and captured a typical atrioventricular node re-entrant tachycardia. The pa…

Reversal of fenestration flow during ventricular systole in Fontan patients in junctional or ventricular paced rhythm.

Sinus node dysfunction is relatively common in patients with Fontan palliation for single ventricle congenital heart disease, and such patients often are in junctional rhythm or may have pacemaker systems for bradycardia. Because the physiologic determinants of left atrial pressure play a major role in determining pulmonary blood flow and therefore cardiac output in Fontan patients, the loss of atrioventricular (AV) synchrony in junctional rhythm or demand ventricular pacing in these patients might be expected to influence cardiac output. We report two cases of Fontan patients with the absence

Adenosine as a therapeutic agent.

Adenosine, an endogenous nucleoside has been recently approved for use in the treatment of paroxysmal supraventricular tachycardia. Adenosine is nearly 100% effective in terminating tachycardia in which the atrioventricular node forms part of the reentrant circuit. Although most ventricular tachycardias are insensitive to adenosine, this substance is effective in ventricular tachycardia induced by catecholamines or exercise. An intravenous bolus dose of 6 mg is the initial dose. If no effect is noted a further bolus of 12 mg can be given. The most common side effects are dyspnea, chest pressur…

Supraventricular tachycardia: diagnosis and treatment.

Specific mechanisms of supraventricular tachycardia include sinoatrial, intra-atrial, atrioventricular (AV) nodal as well as concealed and manifest bypass tract reentry. In dual pathway reentry, at least one of the pathways involves the AV node, usually the slow pathway and the other pathway, perinodal fibers within the atria. Localization of the perinodal fibers is critical for ablative procedures to eliminate AV nodal tachycardias. Other mechanisms of supraventricular tachycardia include chaotic atrial tachycardia and automatic atrial tachycardia with and without AV block. However, drug ther

[Syncope and transient neurologic deficits of undetermined origin. Clinical and electrophysiological correlations].

A consecutive series of 140 patients who presented either with syncope or transient neurological deficit of undertermined cause underwent electrophysiological investigation. The patients were classified in 3 groups: Group I comprising 55 patients having presented "true syncope"; Group II comprising 42 patients with "false vertigo"; and Group III comprising 43 patients in whom a transient neurological deficit had been observed. Globally, the investigations were positive in 58 patients (41,4%). Paroxysmal atrioventricular block was recorded in 21 cases (15%), sinus node dysfunction in 34 cases (

Dual Uncommon Atrioventricular Connections in a Patient With Wolff-Parkinson-White Syndrome.

BACKGROUND: Accessory pathways (APs) from the atrial appendage are uncommon and primarily described on the right side. Furthermore, connections involving the left atrial appendage (LAA) are even rarer. METHODS AND RESULTS: This case report describes a 27-year-old female with recurrent supraventricular tachycardia (SVT) due to Wolff-Parkinson-White (WPW) syndrome. Electrophysiological evaluation identified dual atrioventricular (AV) connections: one from the LAA and another from the left coronary cusp (LCC). CONCLUSION: The electrical characteristics of WPW syndrome need a deeper understandin

Occurrence of primarily noninducible atrioventricular nodal reentry tachycardia after radiofrequency delivery in the slow pathway region during empirical slow pathway modulation.

BACKGROUND: The first-line therapy for atrioventricular nodal reentry tachycardia (AVNRT) is catheter-based slow pathway modulation. If AVNRT is not inducible during an electrophysiological study, an empirical slow pathway modulation (ESPM) may be considered in patients with dual atrioventricular nodal physiology and/or a typical electrocardiogram (ECG). METHODS: We screened 149 symptomatic patients who underwent ESPM in our department between 1993 and 2013. All patients fulfilled the following criteria: (1) either dual atrioventricular nodal (AVN) physiology with up to 2 AVN echo beats or ch

Cryoablation of junctional tachycardia at high risk of atrio-ventricular block.

INTRODUCTION: Transcatheter cryoablation is an alternative option for the treatment of supraventricular tachycardia, due to its very low risk of permanent atrio-ventricular block. However, the overcost of cryocatheter and the high recurrence rate of this emerging technology braked its large use. This study reports the results of an approach using cryoablation for the treatment of junctional tachycardia (JT) in selected patients at high risk of atrio-ventricular (AV) block. PATIENTS AND METHODS: Out of a series of 199 patients with JT treated by catheter ablation, 26 benefited from cryoablatio

One-year follow-up in a prospective, randomized study comparing radiofrequency and cryoablation of arrhythmias in Koch's triangle: clinical symptoms and event recording.

AIMS: To rely solely on clinical symptoms of recurrent palpitations to evaluate the success of interventional procedures can be misleading. This study was designed to assess the efficacy of event recording in evaluating long-term success in patients treated for atrioventricular nodal reentrant tachycardia (AVNRT) or right posteroseptally located accessory pathways (RPS) either by radiofrequency (RF) or by cryoablation (CA). METHODS AND RESULTS: Sixty-three patients with AVNRT and eight with RPS were randomized. Patients were encouraged to activate an event recorder in the case of recurrent pa

Noninvasive diagnosis in patients with undocumented tachycardias: value of the adenosine test to predict AV nodal reentrant tachycardia.

INTRODUCTION: Patients with symptoms suggestive of paroxysmal supraventricular tachycardia (PSVT) but no tachycardia documentation often undergo diagnostic electrophysiologic study. In dual AV node physiology with AV node reentrant tachycardia (AVNRT), the anterograde fast pathway is more sensitive than the slow pathway to the effects of adenosine. The purpose of the study was to test the hypothesis that adenosine can be used as a bedside test for the diagnosis of dual AV node physiology and hence for AVNRT. METHODS AND RESULTS: During electrophysiologic study, 37 patients without prior docum

Selective radiofrequency ablation of the "slow" atrioventricular nodal pathway for control of the ventricular response to atrial fibrillation.

In summary, this study reports 2 important findings: (1) AV nodal modification using the conservative protocol we describe reduces long-term success for ventricular rate control during atrial fibrillation but eliminates the incidence of permanent AV block; (2) directed lesions that eliminate clinical AV nodal reentry slow ventricular response to acute atrial fibrillation but are not sufficient to control ventricular response of chronic atrial fibrillation. Further refinement of these techniques may allow an optimal balance between rate control and avoidance of permanent pacing.

Lack of prognostic implications of spontaneously occurring or stimulation induced atrial tachyarrhythmias in patients with dilated cardiomyopathy.

A prospective study was undertaken in 102 patients with idiopathic dilated cardiomyopathy to assess the significance of spontaneous and inducible atrial tachyarrhythmias (ATA). Twenty-six patients were in chronic atrial fibrillation (group I) and 76 patients were in sinus rhythm (group II); 14 patients in group II had a clinical history of ATA or episodes of ATA during Holter monitoring. An electrophysiological study was systematically performed. Programmed atrial stimulation was carried out in group II, and used up to two atrial premature stimuli from the right atrium at 2 cycle lengths (sinu

Electrophysiological significance of QRS alternans in narrow QRS tachycardia.

To investigate the electrophysiological significance of QRS alternans during narrow QRS tachycardia, transesophageal atrial pacing and recording was performed in 24 patients with a history of paroxysmal supraventricular tachycardia. Standard electrocardiograms showed ventricular preexcitation in 15 patients and normal QRS pattern in nine patients. The ventriculoatrial interval during tachycardia, as defined by means of transesophageal electrogram, allowed tentative diagnosis of the tachycardia mechanism. A 12-lead ECG was recorded either during spontaneous or induced tachycardia, as well as du
