Thermal properties of handwear at varying altitudes.

BACKGROUND: Total handwear insulation (I(T)) is dependent on the rate of heat transfer in air through the skin-handwear interface, handwear layers, and the surface boundary air layer. As altitude increases, the corresponding decrease in air pressure reduces convective heat loss. As convective heat losses decline, I(T), which is inversely related to the rate of heat loss, should increase. Increasing air velocity also reduces the insulation (Ia) provided by the boundary layer. METHODS: The military issue test handwear, Light-duty glove (LD), Trigger-finger mitten (TF), and Arctic mitten (AM), w

Urinary continence in women during centrifuge exposure to high +Gz.

BACKGROUND: One earlier study and anecdotal evidence suggest a possible association between exposure to high +Gz forces and urinary incontinence in women. High +Gz could possibly contributes to the prolapse of the bladder neck, moving it into a position which decreases the leak point pressure resulting in urinary incontinence. HYPOTHESIS: We tested the hypothesis that increased urinary incontinence is associated with high +Gz. METHODS: 25 females were exposed to a high +Gz profile. Following the exposure they were asked to answer a questionnaire grading their urinary continence under high +G

Cancer incidence and mortality among flight personnel: a meta-analysis.

BACKGROUND: Increased cancer risk among flight personnel have previously been noted, including breast cancer among flight attendants and acute myeloid leukemia among pilots. HYPOTHESIS: Exposure to cosmic radiation and other physical or chemical agents may pose health risks for flight personnel. METHODS: We performed an exhaustive search for published and unpublished cohort studies of flight personnel from 1986-98. We combined relative risks (RR) for selected causes from four mortality and/or incidence studies of pilots and two incidence studies of flight attendants, using standard meta-anal

Perception of surrounding space controls posture, gaze, and sensation during Coriolis stimulation.

BACKGROUND: Motion sickness evokes not only discomfort but also marked equilibrium ataxia. However, its mechanism of ataxia remains unclear. HYPOTHESIS: We supposed that stance, gaze, and sensation may be uniformly controlled by the perception of the surrounding space. METHODS: Using Coriolis stimulation, we examined eight normal subjects and two patients with bilateral labyrinthine loss. They were asked to stand on a platform; then while being rotated clockwise 60 degrees x s(-1), they tilted their head 30 degrees. Displacement of the center of pressure and ocular movements were recorded us

Gastric myoelectric activity during low frequency oscillation.

BACKGROUND: The normal 3 cycles per minute (3 c/m) resting activity of the stomach has been reported to shift into the range of 4-9 c/m (tachyarrhythmia) in subjects with symptoms of motion sickness. Previous studies have provoked motion sickness with an optokinetic stimulus and have not normally reported changes relative to a control condition. The present experiment investigated changes in the electrogastrogram (EGG) during a control condition and during exposure to 0.125 Hz off-axis yaw oscillation of the body. METHODS: There were 16 male subjects who participated in the two conditions 1 w

Head roll compensation in a visually coupled HMD: considerations for helicopter operations.

BACKGROUND: The helmet-mounted display (HMD) research program at the Flight Research Laboratory of the National Research Council of Canada examined the effects of HMD camera platform dynamics on pilot workload. Many currently fielded visually coupled HMD systems do not reproduce head movements in the roll axis which can lead to the presentation of visual information that is not consistent with vestibular and proprioceptive information. HYPOTHESIS: Our hypothesis was that this sensory conflict can induce motion sickness and increase pilot workload. METHODS: To examine this premise, three pilo

Medical imaging in microgravity.

Microgravity environment induces significant pathophysiological changes in humans. As long-term space habitation and interplanetary missions become commonplace in the next century, researchers and imaging specialists must embrace the unique challenges of microgravity research in developing innovative uses of imaging technology. Microgravity results in cephalad fluid shift, loss of electrolytes, loss of muscle and bone mass, anemia, reduced immune response, variability in gastric emptying and hepatic metabolism, increased bowel transit, and development of space motion sickness. Medical imaging

Promethazine affects optokinetic but not vestibular responses in monkeys.

BACKGROUND: Promethazine is used to treat motion sickness including Space Adaptation Syndrome, but there is incomplete information about how it affects vestibular and optokinetic responses. METHODS: Vestibular and optokinetic nystagmus, recorded with eye coils, were characterized in monkeys after administration of promethazine at dosages approximately equivalent to those used by humans in space. RESULTS: The initial increase of horizontal eye velocity during optokinetic nystagmus (OKN) was reduced after receiving the drug. Consequently, it took a longer time for eye velocity to rise to 60% o

The effects of serotonin (5-HT3) receptor antagonists on gastric tachyarrhythmia and the symptoms of motion sickness.

BACKGROUND: The purpose of this study was to determine the effects of the serotonin (5-HT3) receptor-antagonist antiemetics ondansetron and granisetron on the development of gastric tachyarrhythmia, nausea, and other symptoms of motion sickness. METHODS: In a double-blind, counterbalanced, repeated measures design, 12 motion sickness susceptible college students participated in 3 sessions with an intersession interval of 1 wk. Participants received either 8 mg of ondansetron, 2 mg of granisetron, or placebo 1 h before exposure to a rotating optokinetic drum. Electrogastrograms (EGGs) were rec

Outcome after treatment of neurological decompression illness is predicted by a published clinical scoring system.

BACKGROUND: After recompression therapy, some cases of neurological decompression illness (DCI) have a significant residual deficit. Boussuges et al. report a scoring system to predict sequelae using weighted historical and clinical indices at presentation which we applied to the British Hyperbaric Association (BHA) database of UK diving accidents, held at the Institute of Naval Medicine (INM). METHODS: A database search identified 234 cases of neurological DCI from the 2 yr, 1995-6. Of these, 217 case records contained sufficient data to apply the scoring system. Outcome was classified as se

Pulmonary and auditory function among experienced construction divers: a cross-sectional study.

We studied pulmonary and auditory function in a cross-sectional study of 26 experienced construction divers compared with 26 workshop workers matched for age, height, and smoking habits. The divers used air as breathing gas and performed open-sea bounce dives to a maximum of 50 m in sea water. The mean number of dives over a mean diving period of 20 yr (SD = 11) was 4746 (SD = 4743) (Range: 450-17000). Assessment of lung function included dynamic lung volume and flow and diffusion capacity (transfer factor) for carbon monoxide (TlCO). The auditory examination was performed measuring air conduc

Effects of low frequency noise upon the reaction of pleural milky spots to mycobacterial infection.

INTRODUCTION: Milky Spots (m.s.), also known as Kampmeier's foci, are confined to the parietal leaflet and act as a barrier to mediastinum infections, having very important phagocytic functions. Previous studies have made clear that large pressure amplitude and low frequency (LPALF) noise (> or = 90 dB, < or = 500 Hz) induces morphofunctional changes on the pleural mesothelium. The purpose of this study was to investigate the dynamics of m.s., namely the response to infectious agents reaching the pleural space under noise stress. METHODS: Wistar rats (n = 20) were used; 5 as control and sham

Repetitive high G exposure is associated with increased occurrence of cardiac valvular regurgitation.

BACKGROUND: Exposure to repeated high +Gz loads and the methods to prevent loss of consciousness cause unique stresses on the cardiovascular system. The purpose of this study was to determine if the +Gz environment is associated with an increased occurrence of valvular regurgitation in pilots of high performance aircraft. METHODS: There were 247 subjects who were divided into pilot (n = 46) and non-pilot (n = 201) groups. Pilots were defined as those individuals who had flown at least 1000 h in high performance aircraft. The echocardiographic data of these subjects were examined retrospective

Tuberculosis on the flight deck.

Tuberculosis in commercial aircraft has been a concern since a 1995 incident of possible transmission from an active case of tuberculosis to passengers in the cabin of a 747. Subsequently, commercial air carriers have been vigilant in cooperating with public health authorities in tracking all known exposures to tuberculosis. In 1998, a pilot of a commercial airliner was diagnosed with active tuberculosis. Company records demonstrated that in the previous 6 mo, the pilot had flown with 48 other pilots. Every exposed pilot was contacted and evaluated by skin testing (IPPD) or chest x-ray if prev

Epilepsy in the vibroacoustic disease: a case report.

INTRODUCTION: Late-onset epilepsy was one of the first neurological problems identified in patients diagnosed with vibroacoustic disease. Other clinical situations, such as automatisms and rage-like reaction crises were also observed. Some cases of epileptic seizures were triggered by different types of stimuli. CASE REPORT: This study describes the clinical case of a 30-yr-old male metal-worker who had epileptic seizures when he used vibratory tools common to his profession, such as drills and sanders. We performed a 21-channel EEG during a seizure induced in the laboratory by direct contact
