Patient and public involvement and engagement in critical care clinical research: A review.

Across the world, there is increasing recognition of the importance of engaging with and involving patients, family members, and members of the public in the design, delivery, and dissemination of clinical research. This reflects the value of grounding research in lived experience, thereby ensuring the relevance and integrity of the research. In this review paper, we seek to provide an up-to-date review of patient and public involvement and engagement in critical care clinical research. Our review is informed by a targeted MEDLINE search and author experience, including that of a critical care

Waiting for translation: A review of translational research in psychological treatment.

Translational psychological treatment aims to translate basic research findings and principles into novel psychological interventions, but recent research indicates unrealized potential. To better understand and overcome barriers in this translational process, we systematically reviewed 647 publications from a recently published bibliographic dataset. We took a mixed-methods approach, drawing on a previously proposed conceptual framework for translational research in clinical psychology. Specifically, we identified the exact translational stage of each publication and specified the object of t

EV-Checklist: AI-Powered Rapid Documentation for Enhancing Transparency and Accessibility of Extracellular Vesicle Research Data.

Transition of extracellular vesicle (EV) research from basic discovery to clinical application raised hopes regarding diagnostic, therapeutic and prognostic progress. Rigorous reporting of experimental details is required to align the EV field with pharmaceutical quality standards. MISEV2023 recommendations encourage concise reporting but widespread adoption remains limited. Current reporting tools are time-consuming, and adherence declines despite rapidly growing number of EV studies. We therefore created EV-Checklist, a complementary digital tool that streamlines reporting and increases tran

Cell Membrane Biophysics as a Therapeutic Interface for Nanomedicine: From Disease-Associated Remodeling to Translational Qualification.

Nanomedicine has yielded clinically useful platforms, including liposomes, albumin-bound nanoparticles, and lipid nanoparticles; yet, many systems translate poorly because of nonspecific biodistribution, limited target-site accumulation, inefficient cellular uptake and intracellular delivery, immune clearance, and off-target toxicity. These bottlenecks are often shaped at cell membrane interfaces, where therapeutic materials are recognized, retained, internalized, or cleared and may elicit unsafe responses. Here, we frame cell membrane biophysics as a therapeutic interface for nanomedicine. We

Recent advancements and clinical translation of cinnamaldehyde derivatives in therapeutics.

OBJECTIVE: This review systematically evaluates why CA derivatives have failed to transition from bench to bedside despite superior preclinical performance. SIGNIFICANCE: Cinnamaldehyde (CA) clinical translation is stymied by a biopharmaceutical paradox: promising in vitro efficacy is negated by poor aqueous solubility (≈1 mg/mL), rapid first-pass metabolism, and a short plasma half-life (≈1.7 h). Also, the reactive α,β-unsaturated aldehyde moiety often triggers Pan-Assay Interference (PAINS) behavior. KEY FINDINGS: We identify three primary chemical strategies that circumvent native CA's li

Bibliometric analysis of Alzheimer's and dementia research in Latin America.

INTRODUCTION: Dementia is increasing rapidly in Latin America and the Caribbean (LAC), but research output remains limited. Tracking publication trends, themes, and collaborations is key to guiding regional research and policy. METHODS: Bibliometric analysis was conducted on dementia-related publications from 21 LAC countries (1990 to 2024) using Scopus. Thirteen keywords identified relevant articles, classified into themes through artificial intelligence (AI)-assisted and manual review. Bibliometrix and VOSviewer assessed publication trends, country and institutional output, and collaboratio

Mapping the Landscape of Epigenetic Research in Diabetes Mellitus: A Decade-Long Bibliometric Analysis (2014-2024).

INTRODUCTION: Epigenetic regulation constitutes critical molecular mechanisms underlying the pathogenesis of diabetes and disease progression. While substantial mechanistic investigations exist, the field lacks systematic mapping of research trends, collaborative networks, and emerging frontiers. OBJECTIVES: To conduct the first comprehensive bibliometric evaluation of epigenetic studies in diabetes mellitus and its complications (2014-2024), identifying key research domains, international collaboration patterns, and innovative investigative directions to inform strategic research planning an

Developing a core outcome set for clinical research on Chinese medicine dampness syndrome.

OBJECTIVE: This study develops a core outcome set (COS) for clinical research concerning Chinese medicine (CM) dampness syndrome (COS-CMDS) that might improve heterogeneity of outcomes, reporting, and inadequate attention to the CM characteristic outcomes in clinical research on CM dampness syndrome. METHODS: An initial outcome pool was constructed based on a systematic review of clinical studies related to CM dampness, registered trials, and semi-structured interviews with patients and healthcare professionals. Various stakeholders were invited to participate in a 2-round Delphi survey to sc

Contextualizing key principles to promote a justice-oriented informatics research agenda: proceedings and reflections from an American Medical Informatics Association workshop.

OBJECTIVES: Advancing health through informatics requires attending to justice. Recent policy changes in the United States have introduced significant barriers to promoting justice within informatics due to targeted funding cuts and hostility to science, especially science that prioritizes justice. MATERIALS AND METHODS: We present five key principles for advancing a justice-oriented informatics agenda, synthesized from our workshop held at the American Medical Informatics Association 2022 Annual Symposium. RESULTS: These principles are: (1) Recognize knowledge and methodologies across commu

From pathogenic carriers to therapeutic hope: the dual role and translational prospects of exosomes in diabetic kidney disease.

Diabetic kidney disease is one of the most serious microvascular complications of diabetes. Its pathogenesis is complex, and methods for early diagnosis and intervention are limited, posing major challenges in clinical practice. Exosomes, as key carriers of intercellular communication, are involved in various physiological and pathological processes, such as immune regulation, tumor metastasis, and tissue repair. The regulation of their release is closely related to disease progression and has become a hotspot in research on disease diagnosis and treatment. In diabetic kidney disease, exosomes

Challenges in translating nanotechnology from research to clinical practice.

Nanotechnology has emerged as a promising frontier in the identification and treatment of skin cancer by offering innovative platforms for targeted drug administration, real-time imaging, and enhanced therapeutic efficacy. Though preclinical results are promising and scientific enthusiasm is rising, the shift of nanotechnological breakthroughs from research laboratories to clinical environments remains hampered. The main translational problems preventing the clinical acceptance of nanomedicine in skin cancer treatment are investigated in this chapter. It explores obstacles like manufacturing s

MicroRNAs in Obesity-Associated Metabolic Inflammation: From Molecular Mechanisms to Clinical Translation.

Metabolic inflammation, triggered by obesity is a core pathological link in complications such as type 2 diabetes, cardiovascular diseases, and non-alcoholic fatty liver disease. This review explains the central hub role of microRNAs in obesity-related chronic low-grade inflammation, achieved through their precise regulation of macrophage polarisation, inflammatory signalling pathways, and adipokine secretion. Importantly, this relationship is bidirectional-while miRNAs regulate inflammatory cascades, the inflammatory environment itself can alter miRNA expression, forming complex feedback loop
