Interbody Fusions in the Lumbar Spine: A Review.

BACKGROUND: Lumbar interbody fusion is among the most common types of spinal surgery performed. Over time, the term has evolved to encompass a number of different approaches to the intervertebral space, as well as differing implant materials. Questions remain over which approaches and materials are best for achieving fusion and restoring disc height. QUESTIONS/PURPOSES: We reviewed the literature on the advantages and disadvantages of various methods and devices used to achieve and augment fusion between the disc spaces in the lumbar spine. METHODS: Using search terms specific to lumbar inte

Neurologic injury in snowmobiling.

BACKGROUND: Snowmobiles are increasingly popular recreational, all-terrain utility vehicles that require skill and physical strength to operate given their inherent maneuverability, acceleration, and top speed capabilities. These same characteristics increase the risk of injury with the operation of these vehicles, particularly neurological injury. We characterize our series of 107 patients involved in snowmobiling accidents. METHODS: From January 2004 to January 2012, all snowmobiling-related injuries referred to our regional trauma center were reviewed. Information had been recorded in the

Intracranial hypotension with spinal pathology.

BACKGROUND CONTEXT: Intracranial hypotension (IH) is a syndrome in which volume depletion of the cerebrospinal fluid (CSF) results in various neurological symptoms. Most commonly, a small tear or defect in the spinal dural sac is the underlying lesion that results in a CSF leakage and IH. Causes of IH can be classified as 1) spontaneous (primary), and 2) secondary. Knowledge of IH is essential to spine surgeons, because a small proportion of patients with spontaneous IH require spine surgery, and secondary causes of IH include spine trauma, various spine-related diagnostic/therapeutic procedur

Calcitonin-gene-related-peptide-immunoreactive innervation of the rat head with emphasis on specialized sensory structures.

The distribution of calcitonin-gene-related peptide-like immunoreactivity (CGRP-IR) was studied in sections of decalcified rat head and selected whole-mount preparations in order to address the complex peptidergic innervation patterns in peripheral cephalic specialized zones and to examine neuronal ganglia in situ. Labeled neuron somata in trigeminal, glossopharyngeal, and vagal ganglia comprised a large proportion of small to medium size type B ganglion cells. Parasympathetic ganglia (ciliary, otic, sphenopalatine, submandibular) revealed a small population of labeled somata and numerous peri

Deep learning MRI halves scan time and preserves image quality across routine neuroradiologic examinations.

BACKGROUND: Magnetic resonance imaging (MRI) is a cornerstone of neuroimaging but is limited by lengthy acquisition times, which can lead to motion artifacts, patient discomfort, and delayed care. Deep learning reconstruction is an emerging technology that can offer image acquisition acceleration while maintaining image quality. PURPOSE: To compare image quality and acquisition efficiency between deep learning-accelerated vs conventional MRI (C-MRI) across a spectrum of routine neuroradiologic examinations. MATERIALS AND METHODS: In this single-center retrospective study, 26 patients underwe

Biomechanical investigation of a novel integrated device for intra-articular stabilization of the C1-C2 (atlantoaxial) joint.

BACKGROUND CONTEXT: The anatomy of the atlantoaxial joint makes stabilization at this level challenging. Current techniques that use transarticular screw fixation (Magerl) or segmental screw fixation (Harms) give dramatically improved stability but risk damage to the vertebral artery. A novel integrated device was designed and developed to obtain intra-articular stabilization via primary interference fixation within the C1-C2 lateral mass articulation. PURPOSE: To assess the atlantoaxial stability achieved with a novel integrated device when compared with the intact, destabilized, and stabili

Motor and somatosensory conduction time between the cortex and the Erb point in patients suffering from cervical spinal stenosis and tumour.

Motor and sensory conduction time between the cortex and the Erb point were examined in patients with cervical cord compression. Patients were divided into two groups: the compression was caused either by cervical extramedullary tumour (9 cases), or by cervical spondylosis or herniated disc (16 cases). In response to median nerve stimulation, pathological somatosensory evoked potentials were recorded in 66% of the patients suffering from tumour and in 60% of the patients suffering from spondylosis. All of the patients disclosed pathological motor evoked potentials. On the basis of these observ

In-vitro investigation of a new neuromuscular relaxant, AH8165.

AH8165 was compared with other neuromuscular relaxants in an in-vitro rat phrenic nerve-diaphragm preparation. Concentrations of 6-10 mug/ml AH8165 produced progressive decreases in strength of concentration. AH8165 was 0.1 times as potent as d-tubocurarine, and its effects were more rapidly reversed by washing. The times to recovery from 90 per cent blockade were the same for succinylcholine and AH8165, but the time to recovery from 50 per cent blockade was shorter for succinylcholine than for AH8165. Neostigmine reversed blockade induced by d-tubocurarine to 80 per cent of control, while it

[Somesthetic evoked potentials and serial motor evoked potentials in the study of proximal peripheral nerve conduction. Apropos of 7 cases].

The study of proximal motor and sensory nerve conduction in the thoracic outlet syndrome is still difficult and laborious in 1994. However, these conductions can be measured at different levels by means of somaesthetic evoked potentials and motor evoked potentials, when one takes the time to perform them. The study in normal subjects demonstrates that the proximal sensory and motor conduction delays are approximately 3.2 ms and are therefore comparable to that of the median nerve at the wrist. The study of 7 cases related to various diseases shows that these techniques, performed after electro

Impaired postural control in patients with cervico-brachial pain.

Dizziness and subjective balance disturbances are common complaints in cervical pain syndromes. We assessed balance function with posturography using vibration-induced and galvanically-induced body sway in consecutive patients (n = 116) with cervico-brachial pain syndrome of more than 3 months' duration. A total of 83% of the patients showed signs of cervical root compression on MRT scans. The incidence of complaints of vertigo was 50%. The patients manifested significantly poorer postural control than sex- and age-matched controls (n = 20). Disorders of the neck should be considered when asse
