[Complicated diverticular disease debuting as necrotising fasciitis of pelvic limb. A case report].

BACKGROUND: Diverticular disease, and the diverticulitis, the main complication of it, are widely studied diseases with multiple chronic cases reported in the literature, but there are no atypical presentations with extra-abdominal symptoms coupled with seemingly unrelated entities, such as necrotising fasciitis. CLINICAL CASE: Female 52 years old, was admitted to the emergency department with back pain of 22 days duration. History of importance: Chronic use of benzodiazepines intramuscularly. Physical examination revealed the presence of a gluteal abscess in right pelvic limb with discolorat…

[Conservative laparoscopic treatment of diverticular peritonitis].

GOAL: To evaluate the results of a strategy of conservative laparoscopic treatment of peritonitis due to perforated diverticulitis for all patients, without exception for intraoperative findings or general patient condition, and to study the feasibility of eventual second-stage laparoscopic colectomy. MATERIALS AND METHODS: Between January 2003 and May 2007, 25 consecutive patients were urgently hospitalized with acute peritonitis due to perforated diverticulitis. All patients underwent laparoscopic peritoneal lavage and debridement; when there was a large perforation (ten cases), suture clos…

Early lap-band erosion associated with colonic inflammation: a case report and literature review.

INTRODUCTION: Laparoscopic adjustable gastric banding is an effective and safe surgical modality for the treatment of morbid obesity. Erosion of the band into the stomach has been reported. No reports are available on erosion of the Lap-Band following diverticulitis of the colon. CASE REPORT: A 31-year-old female with a body mass index (BMI) of 52 underwent an uneventful laparoscopic Lap-Band placement. Postoperative contrast study revealed good positioning of the band and no evidence of leakage. The patient's recovery was uneventful except for an elevated temperature of 101.5 degrees F that …

Possible Contribution of a Diverticulum to the Development and Rupture of Colonic Lymphangioma.

A 55-year-old Japanese man with a history of diverticulitis underwent colonoscopy for careful evaluation of progressive anemia. A 5-mm depressed lesion oozing spontaneously was observed at the hepatic flexure. On suspicion of depressed-type of cancer, right-sided hemicolectomy was performed. Histopathological examination indicated a collapsed lymphangioma exactly over a diverticulum, which had previously been complicated diverticulitis. The colonic mucosa and lymphangioma prolapsed beyond the subserosal layer via the muscularis propria defect, resulting in a depressed lesion and mucosal lacera…

A comparison of the accuracy of ultrasound and computed tomography in common diagnoses causing acute abdominal pain.

OBJECTIVES: Head-to-head comparison of ultrasound and CT accuracy in common diagnoses causing acute abdominal pain. MATERIALS AND METHODS: … quent final diagnoses in the 1,021 patients (mean age 47; 55% female) were appendicitis (284; 28%), diverticulitis (118; 12%) and cholecystitis (52; 5%). The sensitivity of CT in detecting appendicitis and diverticulitis was significantly higher than that of ultrasound: 94% versus 76% (p < 0.01) and 81% versus 61% (p = 0.048), respectively. For cholecystitis, th …

Imaging diverticular disease.

Barium enema remains the gold standard for demonstrating the extent and severity of colonic diverticular disease. As such, barium studies have a role in clarifying the differential diagnosis of patients with abdominal pain and altered gut function. In acute diverticulitis or suspected diverticular perforation water soluble contrast studies are preferred to barium. An alternative in this acute scenario is cross-sectional imaging by ultrasound, or more usefully computed tomography (CT). CT is especially helpful in complicated diverticular disease. Diverticular disease is a common finding at colo…

[Uncommon presentation of perforated colonic diverticuli].

Diverticulosis of the colon is common in the sigmoid, but much less so in the cecum, ascending or transverse colon. We report 2 patients who presented with clinical findings of perforated diverticulitis of the transverse and left colon. A 42- and a 46-year-old man underwent extended right and left hemicolectomies, respectively. The first patient ran an excellent postoperative course. In the second, in whom a double-barrelled colostomy was formed, the proximal colostomy had to be redone because of stomal retraction, but both stomata eventually were closed. On follow-up both patients are symptom…

A classification system for diaphragmatic defects in catamenial pneumothorax.

Colonic diverticula are protrusions of the mucosa through the outer muscular layers, which are usually abnormally thickened, to form narrow necked pouches. Diverticular disease of the colon covers a wide clinical spectrum: from an incidental finding to symptomatic uncomplicated disease to diverticulitis. A quarter of patients with diverticulitis will develop potentially life-threatening complications including perforation, fistulae, obstruction or stricture. In Western countries diverticular disease predominantly affects the left colon, its prevalence increases with age and its causation has b…

[The pathology of Meckel's diverticulum].

Sixteen cases of Meckel's diverticulum are reported, 14 of them detected during emergency operative intervention for acute surgical abdomen and 2 during planned operation. The complicated forms of Meckel's diverticulum were 10: acute diverticulitis 4 (25 per cent), intestinal obstruction 4 (25 per cent), neoplastic process 1 (6.25 per cent), intestinal hemorrhage 1 (6.25 per cent). Symptom-free were 43.75 per cent of the diverticuli. The clinical symptoms of acute diverticulitis resembled much those of acute appendicitis. The method of choice for its surgical management is cuneiform resection …

Early appendicitis--a safe diagnosis?

INTRODUCTION: Serosal inflammation of the appendix in association with a significant peritoneal exudate can be mistaken for early acute appendicitis. We highlight the importance of looking for other sources of intra-abdominal sepsis in this clinical setting. CLINICAL PICTURE: We present 3 cases of so-called "mild" appendicitis with serosal inflammation that was ultimately shown to be caused by Meckel's diverticulitis. TREATMENT: All 3 patients initially underwent an appendicectomy. In 2 of these cases, a further laparotomy and excision of a Meckel's diverticulum was carried out. All 3 made a…

[Intra-abdominal infections].

50% of the patients admitted for acute abdominal pain have an infectious intra-abdominal disease, most of them an appendicitis, a cholecystitis or a diverticulitis. These infectious diseases are due to a noninfectious lesion of the wall of an intestinal organ. The defense mechanism and the bacterial synergism limit the number, nature and local extension of the multiple micro-organism, producing a strong selection. For the treatment, antibiotics play an important adjuvant role. The main question in all cases is to determine if the cure of the wall lesion is necessary. Furthermore, it is importa…

[Preoperative preparation in colorectal surgery: antibiotics (author's transl)].

Out of a total of 2727 operations of the large bowel because of tumors and inflammatory disease, performed over a 14-year period, 897 were one-stage resections of the colon and rectum without relaxing colostomy. Standard preoperative preparation of the bowel consists of a balanced diet, laxatives, and enema supplemented by 9 g Neomycin and 1.8 g Achromycin, within a 2-day period. Disturbances in wound healing occurred in 12.5%, seroma included. Anastomotic insufficiency occurred in 4%, and fatal fecal peritonitis due to tumors in 1.3% and due to diverticulitis in 1.2%. Total mortality was abou…

Colovesical fistula repair: is early Foley catheter removal safe?

BACKGROUND: Colovesical fistula (CVF) are the most common occurring fistulae secondary to diverticulitis. Review of the literature reveals great variability in postoperative Foley catheter management, as well as the role of a cystogram. The purpose of this study was to review our experience in early vs. late removal of the Foley catheter after CVF repair secondary to diverticulitis. Our hypothesis was that early Foley catheter removal is not associated with increased complications, and postoperative cystogram is of low value. METHODS: This is a retrospective study (January 2002-March 2008) of…

Irritable bowel syndrome and colonic diverticular disease: overlapping symptoms and overlapping therapeutic approaches.

PURPOSE OF REVIEW: Irritable bowel syndrome (IBS) is a common symptomatic disorder in the Western world and colonic diverticula are also prevalent; however, relationships between IBS-type symptoms and diverticula have been a source of much debate. Our goal was to reassess these relationships in the light of new data. RECENT FINDINGS: On removing from consideration clinical scenarios which are directly related to diverticula (i.e., diverticulitis, diverticular hemorrhage, and complications of diverticulitis, such as stricture and fistula), relationships between IBS and diverticula can be seen …

Laparoscopic peritoneal lavage: our experience and review of the literature.

INTRODUCTION: Over the years various therapeutic techniques for diverticulitis have been developed. Laparoscopic peritoneal lavage (LPL) appears to be a safe and useful treatment, and it could be an effective alternative to colonic resection in emergency surgery. AIM: This prospective observational study aims to assess the safety and benefits of laparoscopic peritoneal lavage in perforated sigmoid diverticulitis. MATERIAL AND METHODS: We surgically treated 70 patients urgently for complicated sigmoid diverticulitis. Thirty-two (45.7%) patients underwent resection of the sigmoid colon and cre…

CT abnormalities in right lower quadrant inflammatory disease: review of findings in 26 adults.

Abnormal abdominal computed tomographic (CT) examinations, performed on 26 adults who presented with clinical evidence of right lower quadrant inflammatory disease, were retrospectively studied. Final diagnoses, established surgically in each case, included complicated appendicitis (15 patients), diverticulitis of the ileum or right colon (4), previously undiagnosed Crohn's disease (3), perforated cecal neoplasm (2), right tuboovarian abscess (1), and paracolic abscess of unknown cause (1). The CT features in each diagnostic group are presented in detail. Solid or ring calcification within a p…

Mega-megacolon: a case report.

PURPOSE: Large bowel obstruction and megacolon formation secondary to complicated diverticulitis is rare. METHODS: We present a case of an 84-year-old woman surviving large bowel obstruction and mega-megacolon formation secondary to complicated diverticulitis, with an impressive presentation of abdominal distention. RESULTS: The patient's symptoms, laboratory test results, and imaging were consistent with large bowel obstruction. The patient underwent urgent exploratory laparotomy. Upon entry in the abdomen, it was unexpected that the extreme colonic wall thickening had prevented perforation…
