Hybrid Surgery for Urethral Diverticulum Penetrating the Bladder.

INTRODUCTION AND HYPOTHESIS: Urethral diverticula have various morphological patterns, and in complex cases, a transvaginal approach can be difficult. METHODS: Here, we report a case in which a urethral diverticulum originating from the ventral (posterior) side of the urethra penetrated the bladder through the diverticular wall. A 54-year-old premenopausal woman was diagnosed with a urethral diverticulum on the ventral side of the urethra and a communicating branch penetrating from the diverticular wall to the bladder, triggered by recurrent cystitis. Surgical treatment consisted of transvagi…

Perforation of Meckel's diverticulum with ectopic gastric mucosa.

Meckel's diverticulum is a congenital anomaly of the gastrointestinal tract that occurs in approximately 2% of the population. It is the result of incomplete obliteration of the vitelline duct (ductus omphaloentericus). During the fetal period, this duct connects the yolk sac with the primitive intestine and normally closes around the 5th-7th week of gestation. It is a true diverticulum because it contains all layers of the intestinal wall. Most diverticula are asymptomatic; however, they may lead to serious complications such as inflammation, perforation, obstruction, and bleeding. Our case r…

Management Strategies for Zenker's Diverticulum: A Comprehensive Review.

Zenker's diverticulum (ZD) is an esophageal condition that results in an outpouching of the mucosal layer through a weakened area in the hypopharyngeal wall. This condition can cause symptoms like dysphagia, regurgitation, and aspiration, impacting patients' quality of life. Historically, open surgery was the primary treatment. Although effective, this method is associated with longer recovery times and risks such as infections, nerve damage, and prolonged hospitalization. Rigid endoscopic stapling emerged as a less invasive alternative, offering high success rates for patients with favorable …

Case Report: Intestinal Diverticulum in a Golden Retriever.

A 1-year-old female spayed Golden Retriever presented with a 2-day history of vomiting and a chronic 4-month history of intermittent diarrho … ginning of the ileum. Traditional septotomy or Z-POEM for Zenker's diverticulum.

Zenker's diverticulum (ZD), also known as a cricopharyngeal pouch, is a pulsion pseudodiverticulum located dorsally at the pharyngoesophageal junction. The pathophysiology of ZD involves cricopharyngeal spasm, incoordination, impaired upper esophageal sphincter opening, and structural changes in the cricopharyngeal muscle, leading to symptoms such as dysphagia, regurgitation of undigested food, foreign body sensation, halitosis, unintentional weight loss, and respiratory issues. Treatment for symptomatic ZD typically involves myotomy of the cricopharyngeal muscle. Endoscopic techniques, partic…

Incidental Carcinoid Tumor Arising in a Meckel's Diverticulum.

This case report describes a patient who presented with concern for a closed-loop small bowel obstruction (SBO). During exploratory laparotomy, an area of ischemic bowel due to closed loop obstruction was resected, along with an incidentally discovered inflamed-appearing Meckel's diverticulum (MD). The resected specimen contained a well-differentiated carcinoid tumor of benign behavior with a maximum diameter of 0.6 cm, which invaded the submucosal layer (pT1b and pN0). Over the last several years, there has been a debate with little consensus regarding the proper surgical management in the ca…

Squamous Cell Carcinoma Arising in a Zenker's Diverticulum.

Squamous cell carcinoma (SCC) developing in a Zenker's diverticulum (ZD) is an uncommon condition. The preferred treatment for SCC in the pharyngeal pouch is complete diverticulum resection. Only histopathological evaluation of the pouch can rule out SCC. Here, we present a case of a 62-year-old male patient, who was evaluated for repeated episodes of aspiration and dysphagia, and diagnosed to have a large ZD, the patient underwent Zenker's diverticulectomy with cricopharyngeal myotomy with wide margins due to clinically suspicious specimen. Histopathological examination revealed well-differen…

An Epiphrenic Diverticulum with Previous Perforation Excised Laparoscopically.

An epiphrenic diverticulum (ED) is a rare pseudodiverticulum commonly located at the lower end of the oesophagus. Surgical treatment is only required in patients with worsening dysphagia or repeated chest infection due to microaspirations, and most patients are now treated with minimally invasive surgery (MIS) using the laparoscopic or thoracoscopic approach. Laparoscopic surgery is considered difficult in the presence of previous perforation of the diverticula owing to the intraperitoneal and mediastinal adhesions and is associated with an increased incidence of complications. We were able to…

Laparoscopic Diagnosis and Treatment of an Isolated Cervical Diverticulum.

BACKGROUND: Congenital müllerian anomalies are found in 8.0% of women with infertility and up to 5.5% of women in a general population. Cervical diverticulum is a type of cervical malformation that can be congenital or acquired, with only select cases documented in the literature. Cervical diverticulum can be asymptomatic or present with abnormal uterine bleeding, pelvic pain, or infertility. Previously described management options are largely limited to observation or exploratory laparotomy. CASE: A 35-year-old woman, gravida 2 para 2, presented with persistent menorrhagia, pelvic pain, and …
