Twelve patients presented with symptomatic internal intussusception of the rectum between 1979 and 1987. All were women with a mean age of 55.5 years. Ten patients had symptoms of obstructed defaecation and only three were completely continent. Polyvinyl alcohol sponge abdominal rectopexy was performed in each patient. Over a mean follow-up period of 26.9 months there was no recurrence of internal intussusception. The functional results, however, were mixed but only one patient remained incontinent for solid stool. Rectal discomfort and defaecatory difficulties persisted; six patients continue

The principles of antegrade colonic washout and the Mitrofanoff non-refluxing catheterisable channel were combined to produce a continent catheterisable colonic stoma. The intention was that antegrade washouts delivered by this route would produce complete colonic emptying and thereby prevent soiling. The procedure has been successfully carried out in five patients with intractable faecal incontinence.

Pelvic floor physiology is poorly understood. The funnel shape of the pelvic floor and anal canal is uniquely developed to provide discriminatory continence of gas, liquid, and solid. Proximally, the pelvic floor consists of the pubococcygeus and iliococcygeus muscles. Distally, the anal canal is surrounded by the internal and external sphincter muscles. The anorectal ring is situated between the proximal pelvic floor and the distal anal canal. It is the site of the puborectalis muscle, which is anatomically, neurologically, and functionally merged with the deep portion of the external sphinct

Many different technics have been described for the treatment of rectal prolapse. Transabdominal fixation of the rectum is the procedure currently most favoured, as in The Netherlands, where the majority of patients with rectal prolapse are treated in this way. A questionaire sent to all teaching hospitals showed that in only seven out of 27 hospitals postanal repair is carried out in cases of postoperative fecal incontinence.

It is generally accepted that laying-open constitutes the only effective cure for anal fistulae. The sacrifice of at least some part of the anal sphincter apparatus is therefore the inevitable consequence of every operation for fistula. In the case of an intersphincteric or low-trans-sphincteric fistula, only the internal sphincter or the lower half of the external sphincter need to be incised. This may usually be performed without endangering postoperative faecal continence. High trans-sphincteric or suprasphincteric fistulae, however, involve the entire external sphincter and/or the puborect

Ten extrasphincteric fistulas have been treated by the two-stage seton procedure. The seton was left in situ for three months; follow-up varied from six months to 2 1/2 years. There were no recurrences. Six patients experienced slight soiling and one patient became incontinent. Provided that only muscle tissue is enclosed by the seton, the procedure gives good results; a diverting colostomy is not necessary.

Megacolon must be considered as a diagnosis in any case of intractable constipation also in the adult. Besides the Hirschsprung's disease and the idiopathic megacolon, which have no apparent organic cause, many other pathologic conditions may produce a marked dilatation of the large bowel. The differential diagnosis depends, after a valuation of the clinical and radiologic features, also from enzymatic, histologic and functional tests. The Hirschsprung's disease can be observed after the infancy in: 1) Patients in whom diagnosis is made only in adult age; 2) Patients with a diagnosis made in i

BACKGROUND: Pelvic floor disorders are a group of disorders affecting the pelvic floor that include clinically definable conditions such as pelvic organ prolapse, urinary incontinence and fecal incontinence. These conditions silently affect millions of women worldwide and related problems are not well disclosed by women due to associated social stigma or lack of access to services in developing countries. Thus, the magnitude and related burden of these conditions vary, and little is known about them. This study was conducted to assess the magnitude and associated factors of symptomatic pelvic …

PURPOSE: The role of the antegrade continence enema (ACE) in children has been studied extensively; however, its role specifically in children with an anorectal malformation (ARM) is less clearly documented. This systematic review aimed to synthesise and evaluate existing data regarding long-term outcomes of ACE in children with ARM.
METHODOLOGY: Two databases (Embase and MEDLINE) were reviewed. Studies were eligible if their population included children under the age of 18 with an ARM who underwent an ACE procedure. All studies from when the ACE was first described in April 1990, to April 20

INTRODUCTION: The ligation of the intersphincteric fistula tract (LIFT) procedure has garnered popularity with its success rates and purported sphincter-continence preservation. However, there remains a paucity in the literature on the objective evaluation of sphincter integrity and fecal continence after the LIFT procedure.
OBJECTIVES: The present study undertakes a scoping review to systematically explore and map the published literature, to evaluate the sufficiency of data on the impact on continence in patients after the LIFT procedure.
DESIGN: A systematic search of MEDLINE, PubMed, EMB

BACKGROUND: Children with constipation and encopresis are often treated with medication and behavioral approaches. When constipation persists, surgical interventions such as antegrade continence enema (ACE) procedures are considered. Many children benefit from these procedures; however, some children continue to have incontinence, experience complications, or discontinue the use of the ACE stoma. There is some evidence in the literature to indicate that psychosocial factors can have an impact on ACE outcomes; however, standardized biopsychosocial guidelines related to ACE candidacy and surgery

PURPOSE: To assess the feasibility and clinical outcomes of telehealth-delivered pelvic floor muscle training (PFMT) for urinary incontinence (UI) and/or faecal incontinence (FI) after gynaecological cancer surgery.
METHODS: In this pre-post cohort clinical trial, patients with incontinence after gynaecological cancer surgery underwent a 12-week physiotherapist-supervised telehealth-delivered PFMT program. The intervention involved seven videoconference sessions with real-time feedback from an intra-vaginal biofeedback device and a daily home PFMT program. Feasibility outcomes included recrui

BACKGROUND: Non-randomised studies assessing intrarectal botulinum toxin type A (BoNTA) injections for faecal incontinence are promising. We aimed to evaluate the efficacy of BoNTA for the treatment of faecal incontinence in a randomised study.
METHODS: In this randomised, double-blind, placebo-controlled study, we included adult patients who had at least one urgency or faecal incontinence episode per week for at least 3 months and who had experienced a failure of conservative or surgical treatment from eight French specialist hospital units with the skills to manage patients with faecal inco
