Use of a noninvasive electromagnetic device to place transpyloric feeding tubes in critically ill children.

BACKGROUND: The start of transpyloric feedings is often delayed because of challenges in reliably placing tubes blindly at the bedside. OBJECTIVE: To determine whether tube placement with the guidance of a noninvasive computerized electromagnetic device shortens the time needed to achieve accurate placement of transpyloric feeding tubes in critically ill children. METHODS: In a prospective, randomized trial in a tertiary-care, university-affiliated pediatric intensive care unit, 49 children requiring transpyloric feeding tube placement were randomized to have their tube placed by using conve

Assessment of growth and feeding practices in children with cleft lip and palate.

Feeding difficulties in cleft lip and palate (CLP) infants is commonly observed and is the most traumatic experience the family has to face. These infants are undernourished and have compromised growth. The purpose of this study was to 1) assess general health and growth parameters in children with CLP and in normal children; and 2) investigate the feeding methods of CLP infants and normal infants. A total of 221 children from birth to six years of both sexes, with CLP (60 children) and normal (161 children) were selected. The CLP and normal children were divided into three subgroups by age. Enteral versus parenteral nutrition after oesophagogastric surgery: a prospective randomized comparison.

BACKGROUND: There appears to be an emerging consensus that early postoperative nutritional support benefits the high-risk patient by decreasing septic morbidity, maintaining immunocompetence and improving wound healing. Enteral nutrition via a feeding jejunostomy has been associated with serious complications, with a reported mortality rate as high as 10%, while total parenteral nutrition has also been associated with a wide variety of complications. METHODS: Ninety-seven patients undergoing oesophagectomy or gastrectomy underwent pre-operative nutritional assessment and were randomized to re

Key factors influencing aflatoxin M1 concentration in dairy products: A comprehensive review.

Aflatoxin B1 (AFB1) contamination in livestock feed, followed by its ingestion by dairy animals, leads to hydroxylation of the toxin in the liver and its conversion into aflatoxin M1 (AFM1), which is subsequently excreted in milk. The occurrence of AFM1 in milk and dairy products is a major global public health concern. This review examines the key factors influencing AFM1 contamination in dairy products. The primary source is the presence of Aspergillus fungi in animal feed, which produce AFB1 during cultivation, harvesting, and storage. Environmental conditions (particularly high temperature

Ultrasound Assessment of Gastric Emptying in Premature Infants Treated With Non-Invasive Ventilatory Support.

BACKGROUND: Nasal continuous positive airway pressure (CPAP) introduces positive pressure of air into both the trachea and stomach, which may affect gastric emptying. The rate of gastric emptying can be estimated by ultrasound (US) in neonates by two validated techniques: "antral cross-sectional area" (ACSA, two-dimensional estimate of the surface area at the gastric antrum), and "spheroid gastric volume" (spheroid, three-dimensional estimate of the stomach volume). OBJECTIVE: To compare gastric emptying rates in neonates on machine-derived nasal CPAP (MD-nCPAP, Avea and RAM cannula) with tho

Feeding Type at 6-12 Months and Early-Life Iron-Related Hematologic Changes in Infants: A Longitudinal Birth Cohort Study.

BACKGROUND/OBJECTIVES: The World Health Organization recommends continued breastfeeding for up to 2 years of age. However, because breast milk contains less iron than formula milk, prolonged exclusive breastfeeding may increase the risk of iron deficiency anemia. This study aimed to compare iron status and growth outcomes at 12 and 36 months based on feeding methods during the 6-12-month period. METHODS: In this prospective birth cohort study, we followed 126 children from birth to 36 months. Participants were categorized into breastfeeding (BF, n = 21), formula feeding (FF, n = 62), and mixe…

Modification of Experimental Model of Necrotizing Enterocolitis (NEC) in Rat Pups by Single Exposure to Hypothermia and Hypoxia and Impact of Mother's Milk on Incidence of Disease.

BACKGROUND Necrotizing enterocolitis (NEC) is a potentially life-threatening disease that affects the intestine of the neonate, causing necrosis and general inflammation. Treatment consists of feeding cessation and antibiotic therapy. In more severe cases, surgical intervention is necessary. Recently, different NEC models have been used to study the development of novel diagnostic and therapeutic methods. This work modified an experimental NEC model in rat pups by a single exposure of animals to NEC-causing factors and testing the impact of mother's milk on prevalence of the disease. MATERIAL

Clinical decision-making process of healthcare workers when feeding critically ill adults in public sector ICUs in South Africa.

BACKGROUND: Feeding critically ill patients is a complex process because swallowing is heterogeneous, and dysphagia in critically ill patients can have multiple causes. Decisions regarding dysphagia in the critically ill need to be made as a team with all possible outcomes and factors taken into account to ensure the best possible outcomes for patients. However, research has shown that in South Africa (SA), the multidisciplinary team (MDT) approach is not generally used when making decisions around feeding. OBJECTIVES: To explore the clinical decision-making (CDM) factors in feeding practices

Minimizing respiratory complications of nasoenteric tube feedings: state of the science.

This article summarizes research findings regarding ways to minimize the two most dreaded complications of tube feedings: (1) introduction of feedings through tubes positioned in the respiratory tract, and (2) pulmonary aspiration. Bedside methods that lack reliability in ruling out inadvertent respiratory placement of feeding tubes include the auscultatory method, the bubbling under water method, and observing for respiratory symptoms. Testing the pH of aspirates from feeding tubes can be of use in ruling out respiratory placement of newly inserted tubes when acidic values are properly obtain

Sugar and blood: the nutritional priorities of the dengue vector, Aedes aegypti.

BACKGROUND: Sugar-feeding behaviour is essential for mosquito survival and reproduction, and has been exploited to develop new control strategies, such as the attractive targeted sugar baits (ATSB). This study examined the sugar-feeding habits of the dengue vector, Aedes aegypti, in semi-field conditions to determine the optimal timing (age) of sugar meals and whether the availability of sugar sources could affect blood-feeding by these mosquitoes. METHODS: A series of paired-choice assays were conducted in which mosquitoes were allowed to choose between a sugar meal or a blood meal directly

Change of plasma volume, osmolality, and acid-base status in healthy calves after feeding of milk and water- and milk-based oral rehydration solutions.

Oral rehydration solutions (ORS) are a simple and cheap method to treat diarrheal dehydration and acidosis. To maintain the energy supply of diarrheic calves, it is necessary to continue milk feeding. Suckling of milk or milk-based or hypertonic water-based ORS produces a slower rate of abomasal emptying than suckling isotonic water-based ORS. The faster abomasal passage of isotonic water-based ORS implies that efficacious electrolytes reach the gut more quickly, possibly providing a faster rate of rehydration. The aim of the study was to verify when and to what extent milk and water- and milk

A pilot study on in vitro solubility of phosphorus from mineral sources, feed ingredients and compound feed for pigs, poultry, dogs and cats.

Excess phosphorus (P) as seen in cat foods can have a negative effect on health (Dobenecker, Webel, Reese, & Kienzle, ; Pastoor, Klooster, Mathot, & Beynen, ). P surpluses may affect the environment, and economics in food producing animals, whereas marginal supply may impair performance and health. P can only be absorbed if it is soluble. Solubility of feed P in water and weak acid solution-as a precondition for absorption-was investigated in feed for dogs, cats, pigs and poultry. Different P containing mineral compounds (Ca(H2 PO4 )2 , CaHPO4 •2H2 O, Ca4 Na(PO4 )3 , KH2 PO4 , K4 P2 O7 , NaH2

Effectiveness of the novel impression tray "cleftray" for infants with cleft lip and palate: a randomized controlled clinical trial.

OBJECTIVES: Cleft lip and palate (CLP) is one of the most common congenital deformities with worldwide prevalence. It causes a range of issues for infants that mainly involve difficulty in feeding due to abnormal oronasal communication. For this purpose, feeding plates are provided to infants to act as an artificial palate to aid in feeding. The most crucial procedure in fabrication of a feeding plate is creation of the impression using the traditional finger technique or impression trays. This preliminary research aims to compare the effectiveness of novel impression trays with that of the tr
