Spontaneous subcapital femoral neck fracture complicating osteonecrosis of femoral head.

Spontaneous subcapital fracture (SSF) of femoral neck in pre-existent osteonecrosis of femoral head (ONFH) is a rare presentation. Only a few cases have been reported so far and majority of them have been reported to have unilateral hip involvement. We retrospectively reviewed clinical-radiological data of 10 patients (12 hips) with SSF complicating ONFH. All of them underwent uncemented total hip arthroplasty. All the available publications in the English language based medical literature were critically reviewed and results were summarized. The median age of presentation was 32 years (range

[USE OF SMARTPHONE IN ACETABULAR COMPONENT ANGLE MEASUREMENT DURING TOTAL HIP ARTHROPLASTY].

OBJECTIVE: To evaluate the accuracy of using smartphone to measure the angle of acetabular component in total hip arthroplasty (THA). METHODS: Between June 2012 and September 2015, the acetabular abduction and anteversion angles were measured in 50 patients undergoing THA. There were 24 males and 26 females, aged 37 to 83 years (mean, 71 years). The left hip was involved in 22 cases and the right hip in 28 cases. Of 50 patients, 34 suffered from fracture of the femoral neck, and 16 suffered from avascular necrosis of the femoral head. Acetabular dysplasia was excluded in all cases. A smartpho

Head-shaft angle is a risk factor for hip displacement in children with cerebral palsy.

BACKGROUND AND PURPOSE: Hip dislocation in children with cerebral palsy (CP) is a common and severe problem. The Swedish follow-up program for CP (CPUP) includes standardized monitoring of the hips. Migration percentage (MP) is a widely accepted measure of hip displacement. Coxa valga and valgus of the femoral head in relation to the femoral neck can be measured as the head-shaft angle (HSA). We assessed HSA as a risk factor for hip displacement in CP. PATIENTS AND METHODS: We analyzed radiographs of children within CPUP from selected regions of Sweden. Inclusion criteria were children with G

Diagnosis of femoral neck fracture associated with femoral shaft fracture: blinded comparison of computed tomography and plain radiography.

OBJECTIVE: Femoral neck fractures that are associated with femoral shaft fractures have historically been associated with high rates of missed diagnosis. Despite the potentially serious consequences of a missed femoral neck fracture, little work has been conducted to rigorously evaluate the ability of commonly used imaging studies to detect such fractures. Our hypothesis was that axial-view computed tomography is superior to plain radiography at detecting femoral neck fractures. DESIGN: Blinded assessment of a randomized image set that included axial-view computed tomographic scans of the pel

Surgical treatment of femoroacetabular impingement in a patient with Stickler syndrome: a case report.

Stickler syndrome is a multisystem connective tissue disorder caused by mutations in collagen genes that can present with craniofacial, ocular, audial, or skeletal abnormalities. Here, we report on a male patient with a COL2A1 missense mutation (c.647G>A; p.Gly216Asp). He complained of an out-toeing gait and restricted hip mobility. Radiographs showed broad and elongated femoral necks with coxa valga. An alpha angle of 119° and 96° for his left and right femur, respectively, and almost no femoral head-neck offset, suggested a femoroacetabular impingement. Considering the patient's unwillingnes

The pathology of Legg-Perthes disease and its comparison with aseptic necrosis.

A review of pathological material obtained in cases of Legg-Perthes disease and in aseptic necrosis in the adult shows some similarities in distribution of the involved segment of the femoral head. X-ray signs of separation of the central avascular segment from the superior lateral and medial viable segments of the femoral head are recognizable. Crushing in of the weight-bearing segment then occurs and is recognized by flattening of the femoral head outline. At this point the child is in danger of suffering lateral displacement of the viable lateral segment leading to an incongruous extruded f

Treatment of femur fracture with associated head injury.

OBJECTIVES: The aim of this study was to determine (a) whether delay in femur fracture stabilization beyond twenty-four hours in patients with head injury increased the risk of pulmonary complications and (b) whether immediate (up to twenty-four hours) femur fracture stabilization increased the risk of central nervous system (CNS) complications. DESIGN: Retrospective analysis. MATERIALS AND METHODS: Thirty-two patients with femur fracture and head injury were identified. Fourteen underwent immediate stabilization of their fractures, and eighteen underwent delayed (four-teen patients) or no (

Tumour-to-tumour metastasis: head and neck squamous cell carcinoma to enchondroma.

Metastatic disease is a common occurrence in malignancy with certain cancers recognised to have a high incidence of metastases, particularly the liver, lungs, and bones. Tumour-to-tumour metastases are an exceptionally rare occurrence, where a distant malignancy metastasises to a secondary distinct tumour. Breast and lung carcinomas are the more commonly reported origin sites, whilst renal cell carcinomas and meningiomas are the most reported recipient malignant and benign tumours respectively. In this article, we describe an unusual case of a metastatic head and neck squamous cell carcinoma (

"External Iliac Artery Injury After Total Hip Arthroplasty in an Infected Intrapelvic Hemiarthroplasty": A Case Report.

CASE: We report a patient with intrapelvic prosthetic protrusion associated with infection after hemiarthroplasty for a fracture neck of the femur. We discuss the challenges involved in the management of this complex case with recommendations to manage a vascular injury. CONCLUSION: In patients with an intrapelvic prosthesis, thorough preoperative vascular evaluation and vigilant postoperative check are required to prevent a vascular injury. In a suspected vessel injury after primary or revision total hip arthroplasty, it is imperative to get proximal control of the vessel before re-explorati

[Effects of SERMs on bone health. The effects of raloxifene on bone quality evaluated by hip structure analysis].

The effects of treatment with raloxifene on bone quality (proximal femur geometry) are evaluated by hip structure analysis. Raloxifene shows the significant improvement of the index of resistance to bending forces (section modulus) in Japanese patients with postmenopausal osteoporosis. Raloxifene decreased the endocortical bone resorption without affecting the periosteal bone formation. These mechanism induced the increasing the cortical thickness, cross section area, and bone strength. A similar trend was seen in the bisphosphonates. On the other hand, teriparatide increase bone mineral densi

Parathyroid carcinoma.

Parathyroid carcinoma is a very rare cause of primary hyperparathyroidism and these tumors are usually hyper-functioning as compared to other malignant endocrine tumors. Surgery is the only effective primary treatment. We report a patient, who presented with pathological fracture of femur, hypercalcemia, bilateral renal stones, markedly raised Parathormone levels and palpable mass in the neck. Parathyroid adenoma was initially diagnosed and localized at left lower gland by Sestamibi scan and ultrasonography. She underwent surgery and enlarged parathyroid gland was removed. The hip joint in Down's syndrome. A study of its structure and associated disease.

Clinical and roentgenographic examination of the hip was performed in 114 patients with Down's syndrome to study range of motion, roentgenographic anatomy, and incidence of hip pathology. The study found increased external rotation of the hip. Roentgenographic studies demonstrate that, in comparison with a normal acetabulum, the acetabulum of a patient with Down's syndrome is deep, more horizontally placed, and has increased anteversion. The proximal femur of a patient with Down's syndrome has a normal neck-shaft angle and a moderate increase in anteversion. Of the patients, 7.9% had some hip

Incarcerated Cannulated Cancellous Screw Removal during Total Hip Arthroplasty with a Novel Trick: A Case Report.

Salvage total hip arthroplasty (THA) may be required to manage femoral neck fracture implant failures, avascular necrosis and secondary hip arthritis. Cannulated cancellous screws can become stripped or incarcerated during the initial implantation and pose hardware removal issues. We present a novel technique for safe screw removal in a 62-year-old female patient with a painful right hip. She had undergone cancellous screw fixation for a fracture of the neck of femur ten years ago. There was avascular necrosis with screw cut out leading to secondary hip arthritis necessitating THA. Intra-opera

Robotic total hip arthroplasty for fused hips in ankylosing spondylitis patients: Our experience with robotic arm technology.

The conversion from a fused hip to conventional total hip arthroplasty (THA) in patients with ankylosing spondylitis can be challenging. The problems are related to patient positioning, surgical exposure, femoral neck osteotomy, identifying the true acetabulum, and proper implant positioning. This case series describes our experience using the Mako Robotic-Arm in four bilateral THA procedures (each conducted in a single session) and one unilateral procedure in a fifth patient. Robotic total hip arthroplasty (RTHA) simplified THA by providing real-time information on the relative positions of t

Successful Airway and Anesthesia Management Using a High-Flow Nasal Cannula in a Fibrodysplasia Ossificans Progressiva Patient During General Anesthesia: A Case Report.

Fibrodysplasia ossificans progressiva (FOP) is a rare hereditary disorder causing neck stiffness, ankylosis of temporomandibular joints, and severe restrictive respiratory dysfunction due to progressive heterotopic ossification of the connective tissue. Herein, we report a case of successful airway and anesthesia management using a high-flow nasal cannula (HFNC) in a 51-year-old man with FOP undergoing partial bone resection of the right greater trochanter of the femur. Although general anesthesia with awake fiberoptic nasotracheal intubation has been described as the gold standard, HFNC may y

A mountain among molehills: removing an impinging large femoral neck osteochondroma in a man with hereditary multiple exostoses.

A 31-year-old man with a history of hereditary multiple exostoses (HME) presented with persistent right groin pain and reduced hip range of movement. Examination demonstrated a positive FADIR (flexion, adduction and internal rotation) test suggesting femoroacetabular impingement (FAI). Investigations showed multiple sessile osteochondromata of the right femur with a dominant anterolateral femoral neck osteochondroma causing flexion block. The patient underwent an uncomplicated proximal femoral exostectomy. Six-week postoperative pain, range of movement and daily activity had greatly improved.

Prevention of complications after treatment of proximal femoral fractures.

Two factors are primarily responsible for complications after treatment of proximal femoral fractures. First, the strong deforming forces across the hip joint and proximal femur can make fracture reduction difficult. Second, the placement of the implant affects fracture healing and outcome more dramatically than in other areas of the body. In subtrochanteric fractures, the use of appropriate reduction and stabilization techniques can prevent varus malreduction and subsequent failure of the fixation device. In intertrochanteric fractures, lag screw cutout can be prevented by correct implant pos

Crystal-Storing Histiocytosis: Report of a Rare Case Presenting With Pathological Fracture of Femur. Is There More to the Entity?

Crystal-storing histiocytosis (CSH) is a rare histiocytic lesion, most often described in association with lymphoid malignancies, especially plasma cell myeloma or lymphomas associated with monoclonal gammopathy. A few cases have also been described in patients without an underlying lymphoid/plasmacytic neoplasm. The histiocytes are characterized by intralysosomal accumulation of crystals composed of whole or part of the immunoglobulin molecule. The pathobiology is largely unclear. It is a rare phenomenon and the available literature is restricted to case reports and a few case series. We pres
