Complete decongestive physical therapy in a patient with secondary lymphedema due to orthopedic trauma and surgery of the lower extremity.

BACKGROUND AND PURPOSE: This case report describes a patient who developed lower-extremity lymphedema secondary to orthopedic trauma and surgery and reports the response to complete decongestive physical therapy (CDP), with 8 treatment sessions over 3 months. CASE DESCRIPTION: The patient was a 56-year-old man who sustained a right ankle displaced fibular fracture, underwent open reduction internal fixation surgery 12 days later, and developed lymphedema 4 months postinjury. The patient's impairments of the right lower extremity included increased girth, decreased ankle range of motion, and i

Minimally invasive plate osteosynthesis of distal fractures of the tibia.

Minimally invasive plate osteosynthesis of distal tibial fractures is technically feasible and may be advantageous in that it minimizes soft tissue compromise and devascularization of the fracture fragments. The technique involves open reduction and internal fixation of the associated fibular fracture when present, followed by temporary external fixation of the tibia until swelling has resolved. Subsequent limited, but open reduction and internal fixation of the articular fragments when displaced followed by minimally invasive plate osteosynthesis of the tibia utilizing precontoured tubular pl

Stress fractures associated with osteosarcoma of the lower limb.

Three patients with osteosarcoma of the femur developed abnormal radiopharmaceutical uptake in the bones of the contralateral leg. This uptake was not due to metastases. The histology in one patient, the form of the lesion and the disappearance of the abnormal uptake without treatment in the other two, indicated that the uptake was probably due to stress fractures. Changes in weight bearing and walking in the normal leg as a result of the osteosarcoma in the other leg could have been the cause of the stress fractures. It should be recognized that new abnormal uptake on bone scintigraphy in pat

Treatment of nonunion with a constant direct current: a totally implantable system.

There is now sufficient basic research and clinical experience to establish that electrical stimulation produces osteogenesis. Furthermore, electrical stimulation significantly helps union where impaired bone healing exists. The implanted bone growth stimulator is one effective method of electrical stimulation. It can be used in a wide variety of problems: delayed union and nonunion of bones with or without chronic infection and in failed posterior spinal fusion. Successful treatment of congenital pseudarthrosis of the tibia has been encouraging. The implanted bone growth stimulator technique

[Free transplantation of a fibula with epiphysis].

Problems of free bone transplantation, as well as those of bone transplantation by microvascular anastomoses are exemplified by a case of free fibula transplantation--the epiphysis being conserved--by microvascular anastomoses. If revascularisation of the bone transplant is immediately achieved, the chance for survival of bone tissue is improved and there is a chance that the epiphysis will retain the capacity to grow. By multiple surgical interventions the stability of the abnormally mobile forearm was reestablished and considerable lengthening of the shortened skeleton was achieved. By tendo

[Open diaphyseal fractures of the long tubular bones and their treatment].

Based on the study of the immediate results of therapy in 172 patients and late results in 123 patients with open diaphyseal fractures of long tubular bones, the number of poor immediate and late results were found to be directly related with the degree of damaging soft tissues at the fracture site. An analysis of the results also indicated their reliable dependence on the choice of the therapeutic method employed. A predominant immobilization of the fracture in a plaster dressing after primary surgical treatment of the wound and open or closed reposition of the fracture is largely recommended

Feasibility of opportunistic osteoporosis screening using ankle CT Hounsfield units in older adults with ankle fractures.

BACKGROUND: Fragility fractures reveal osteoporosis, but ankle fractures are not consistently defined as fragility fractures. CT-derived Hounsfield unit measurements may provide an opportunistic method to identify patients at risk for osteoporosis. PURPOSE: This study investigated the relationship between Hounsfield units (HU) from ankle CT scans and bone mineral density (BMD) measured by dual-energy X-ray absorptiometry (DEXA), with the aim of determining whether sufficient signal exists between ankle HU and DEXA-derived BMD to justify a larger prospective validation study, rather than to es

Current Trends in the Surgical Treatment of Fibular Fractures: A National Database Study of Intramedullary vs. Plate Fixation Practice Patterns, Complications, and Cost.

Existing primary evidence comparing fibular intramedullary fixation (IMF) with traditional plate fixation (PF) for the treatment of distal fibular fractures remains limited by modest sample sizes. Using a large national database, this study aims to compare use rates, fracture patterns, patient characteristics, time to surgery, complication rates, and cost between fibular IMF and PF within the United States. Adults treated with fibular IMF or PF between October 2015 and October 2021 were identified within the PearlDiver Database. The ratio of IMF-treated to PF-treated patients was tracked tempo

Reconstruction of distal tibia fractures using a posterolateral approach and a blade plate.

OBJECTIVE: The aim of this article is to report a technique for the management of distal tibia fractures with significant anteromedial soft-tissue injury. The patients were initially treated with a spanning external fixator, open reduction and internal fixation (ORIF) of the fibula at the discretion of the surgeon, and soft-tissue management or flap coverage. ORIF of the tibia was performed on a staged basis, using a 90-degree cannulated blade plate and autogenous iliac crest bone graft through a posterolateral approach. DESIGN: Retrospective analysis of a consecutive series of patients. SET

Simultaneous flexion and extension checkrein deformity following tibial and fibular fractures: a rare case treated with concomitant surgical intervention.

Checkrein deformity of the hallux is commonly characterized by rigid flexion of the interphalangeal joint in ankle dorsiflexion but with flexibility in ankle plantarflexion which can be named as flexion checkrein deformity and as rare condition, extension checkrein deformity, characterized by rigid extension of the first metatarsophalangeal joint in ankle plantarflexion and flexibility in ankle dorsiflexion, has been reported. However, there has not reported coexistence of flexion and extension checkrein deformity. The patient, a 27-year-old male, was referred to our department 3 years after t

Stress fractures around the knee.

Stress fractures of the lower extremities are common, especially in the younger athletic population. The current literature consists mainly a variety of case reports but is devoid of any sizeable series of knee stress fracture investigations. Diagnosing a stress fracture around the knee can be a challenge. The proximity of the stress fracture to the knee joint may lead the clinician to investigate intra-articular or other periarticular pathology. The differential diagnosis can be large, including bursitis, tendonitis, mechanical causes, insufficiency fracture, and tumor. A high index of suspic

The management of combined skeletal and arterial injuries of the lower extremity.

Combined skeletal, soft-tissue, and arterial injuries in lower extremity trauma are relatively common. Controversy has surrounded the priority of management of these limb- and life-threatening injuries. There are proponents for initial skeletal stabilization, and an equal number in favor of initial revascularization. The management of a multiply-traumatized patient with an ischemic limb and other life-threatening injuries, and a literature review, are presented to examine some of the issues and describe the use of vascular shunts as a method for rapid temporary revascularization of a damaged l

Malleolar fractures treated with rigid internal fixation and immediate mobilization.

Twenty=eight consecutive patients with displaced malleolar fractures were treated with anatomically exact open reduction, rigid internal fixation and immediate postoperative exercises, following the principles of the Swiss ASIF group. There were no operative or postoperative complications. One patient had a minor abscess over a loosened screw, while 1 patient developed pes equinus that required treatment. AT follow-up 2 to 12 months after the operation, 25 patients were walking normally. The movements of the ankle joint were excellent or good in 25 patients. The method is recommended.
