Preliminary study of elastic-tension digital neoprene orthoses for proximal interphalangeal joint flexion contracture.

Flexion contracture of the proximal interphalangeal joint (PIPJ) is one of the most frequent complications in finger trauma. Orthoses are the most widely used method to optimize total end-range time (TERT). No previous studies showed that an elastic tension orthosis could be applied for longer than 12 h. We aimed to demonstrate that the elastic-tension digital neoprene orthosis (ETDNO) can achieve higher TERT and therefore better range of motion than other elastic-tension orthoses (ETO) described in the literature. A prospective study of treatment of PIPJ flexion contracture included 10 PIP jo

Camptodactyly, arthropathy, coxa vara, pericarditis (CACP)syndrome: a case report.

The camptodactyly-arthropathy-coxa vara-pericarditis syndrome (CACP) is characterized by congenital or early-onset camptodactyly, childhood-onset noninflammatory arthropathy associated with synovial hyperplasia. Some patients have progressive coxa vara deformity and/or noninflammatory pericardial effusion. CACP is inherited as an autosomal recessive mode and the disease gene is assigned to a 1.9-cM interval on human chromosome 1q25-31. We describe a 10-yr-old boy who has typical features of CACP without familial association.

Severe metacarpophalangeal sprain of the thumb in ski accidents. 125 surgical repairs in group of 340 cases of metacarpophalangeal sprains from ski accidents.

In more than 12,000 ski accidents seen during 15 winter seasons (1968-1983), 340 cases of metacarpophalangeal sprains of the thumb were encountered. One hundred fifty-nine were labeled as severe, and 125 of these involving the ulnar slope of the articulation were operated as very urgent or urgent. Three types of anatomic lesions were seen at operation. Capsulo-ligamentous lesions are the most frequent (70%), followed by avulsions of large fragments from the base of teh first phalanx attached to the ulnar ligament as well as to the palmar structures (30%). Finally the true fracture of B. Stener

The acrocephalosyndactyly syndromes: a metacarpophalangeal pattern profile analysis.

Metacarpophalangeal pattern profile (MCPP) analysis was applied to patients with Pfeiffer and Chotzen syndromes, dominantly inherited types of acrocephalosyndactyly (ACS). A characteristic MCPP was obtained for the group. However, it did not discriminate between patients with Chotzen syndrome and those with Pfeiffer syndrome. Six patients in a single family showing Pfeiffer syndrome exhibited this unique MCPP profile which was not present in non-affected family members. Furthermore, three normal-appearing relatives were identified as affected by this technique, and this diagnosis was subsequen

Uncommon hand injury to the batsman in 'gully cricket': report of two cases.

Volar plate injuries are typically caused by hyperextension of the proximal interphalangeal joint. These injuries are usually seen in sports where the ball comes in direct contact with the hands. Forceful dorsiflexion of the finger caused by the speeding ball leads to volar plate avulsion. In cricket, such finger injuries predominantly occur in fielders trying to catch or stop the speeding ball with bare hands. We report two unusual cases of volar plate avulsion injury occurring in batsmen that occurred during 'gully cricket' (street-cricket). We propose the possibility of a novel contrecoup-t

On-top plasty using a free metacarpal head graft for lengthening of proximal phalanx in symbrachydactyly--a case report.

A three-year old patient with symbrachydactyly (didactyly type) presented with a little finger that was too short to allow pinching and consisting of a floppy soft-tissue envelope with hypoplastic phalanges, although the thumb was functional. As the proximal phalanx was too small to permit distraction lengthening or conventional bone grafting, on-top plasty using a 4th metacarpal graft with a cartilage head was undertaken for lengthening the proximal phalanx of the little finger for pinch reconstruction. At ten weeks after surgery, the patient achieved satisfactory pinch function due to the le

Muscle overgrowth of the upper limb in a proximo-distal gradient and concurrent hypoplasia of the index finger.

BACKGROUND: There are many forms of congenital overgrowth of the upper limb. We aim to present the characteristics of a new form overgrowth of the upper limb. METHODS: This is a case series of 3 sporadic cases from 3 different Saudi Arabian families with progressive overgrowth of one or both upper limbs. The literature is reviewed for similar cases. RESULTS: The 3 cases were considered unique because they had almost identical clinical features as follows: the progressive overgrowth only affected the muscles (and not bones or other soft tissues); the palm was wide with abnormal dermatoglyphic

Contribution of lumbrical muscle activity to the paradoxical extension phenomenon induced by injuries to the finger flexor tendons.

The "Extensor habitus" phenomenon occurs in finger flexor tendon injuries and consists of a paradoxical extension of the interphalangeal joints after an attempt to flex the finger. The mechanism of extension is considered to be a contraction of the flexor digitorum profundus that is then transmitted via the lumbrical muscle structure to the extensor expansion. Using electromyography, we recorded the lumbrical muscle activity during the paradoxical extension phenomenon to determine whether the lumbrical muscle contributed to this event. Two patterns of electromyographical activity of the lumbri

Somatotopic mapping of the human primary motor cortex with functional magnetic resonance imaging.

We applied functional magnetic resonance imaging (FMRI) to map the somatotopic organization of the primary motor cortex using voluntary movements of the hand, arm, and foot. Eight right-handed healthy subjects performed self-paced, repetitive, flexion/extension movements of the limbs while undergoing echo-planar imaging. Four subjects performed movements of the right fingers and toes, while the remaining subjects performed movements of the right fingers and elbow joint. There was statistically significant functional activity in the left primary motor cortex in all subjects. The pattern of func

Vascular anatomy of the finger dorsum and a new idea for coverage of the finger pulp defect that restores sensation.

The cutaneous vascular anatomy of the finger dorsum was studied by dissection under loupe magnification of 71 fingers from 19 preserved cadaver hands. All specimens were injected with red latex or epoxy resin through a cannula in the brachial artery to identify the small vessels. The finger dorsum was supplied by the terminal branches from the dorsal metacarpal artery around the metacarpophalangeal joint. Other areas were nourished by the dorsal cutaneous branch from the proper palmar digital artery. There were two or three dorsal branches in the proximal phalangeal region and two in the middl

Another look at the intertendinous connections: a dynamic study based on palpation, ultrasonography, and dissection.

PURPOSE: To explore the feasibility of a dynamic exploration of the intertendinous connections using palpation to capture their haptic characteristics and movement during finger flexion, and ultrasonography to reveal their structure, relationships, and precise measurements. METHODS: The authors utilized palpation and ultrasonography, blind to each other’s findings, as well as anatomical dissection. RESULTS: Both palpation and ultrasonography revealed the intertendinous connections, except for their thickness, which was noted by the latter. Palpation also showed the distal excursion of the inte

A Central Slip Injury in a Professional Basketball Player.

Hand injuries are extremely prevalent in sports such as ball sports. Delayed diagnosis or improperly managed injuries have the potential to prolong the athlete's return to the competition and permanently damage their abilities. We report the case of a 35-year-old male professional basketball player who presented to a chiropractor with pain and swelling of the right fifth finger two days after a basketball game. The injury occurred when a player passed the basketball and the ball hit his fifth finger "head-on". Examination revealed enlargement of the middle joint of the right fifth finger and a
