Ocular and periocular injuries associated with an isolated orbital fracture depending on a blunt cranial trauma: anatomical and surgical aspects.

The anatomical location of fractures following blunt cranio-orbital trauma is important for neurosurgeons and maxillofacial surgeons. In this study, 588 cranio-orbital fractures following blunt trauma were evaluated retrospectively with regard to the anatomical site and surgical treatment. Orbital cranial nerve injuries and the outcomes of the medical and/or surgical treatment are described. Distribution of the zygomatic complex and orbital fractures were as follows: zygomatic complex fractures (n:304), isolated orbital fractures (n:58), complex comminuted fractures (n:226). In 58 cases, 69 or

Intraoperative Radiographic Rotational Profile of the Tibia: Technique and Clinical Series.

Recreating tibial torsion in patients with complex tibial fractures is an important intraoperative consideration, particularly for fractures with comminution or segmental bone loss. Failure to accurately restore tibial alignment may lead to malrotation, poor functional outcomes, and reoperation to correct rotational deformity. No consensus currently exists regarding the optimal technique for intraoperative measurement of tibial torsion in patients undergoing tibial fracture fixation. This article describes a technique using a mobile C-arm fluoroscope to intraoperatively measure torsion of the

The radio-radial external fixator in the treatment of fractures of the distal radius.

The technique of radio-radial monobloc-fixation with the small AO external fixator device has been applied to 17 consecutive Colles' fractures. The fracture types were mainly A3 and C2, according to the AO classification. We found this technique to be easy and quick in application and stable in fixation. Direct, precise and atraumatic reduction can be achieved by using the distal pins as joy-sticks. Furthermore, disimpaction of the fracture to regain length is possible without bone grafting. Normal carpal mobility and load transfer is preserved during fracture healing and the injured hand can

Fractures of the fibula at the distal tibiofibular syndesmosis.

Two basic fracture types are found at the level of the syndesmosis, supination-external rotation (SE) and pronation-abduction (PA), reflecting the mechanism of injury. The SE-type fracture of the fibula extends from the anterior edge in a posterosuperior direction, and the PA-type fracture extends from the medial surface either transversely or obliquely in a laterosuperior direction, and are often comminuted. The SE-types exist in 2 locations above and below the anterior fibular tubercle. All 3 types exist also in atypical forms. Treatment of the initial stages is by cast immobilization, and a

Management of upper midfacial injuries.

Midfacial fractures may extend into orbital roofs, frontal sinus walls, and ethmoidal and sphenoidal bones if the traumatic impact is of sufficient force. The cranium and facial skull are affected and there is the risk of intracranial complications. Management thus requires a teamwork approach. Isolated compound fractures of the anterior frontal sinus may be reconstructed by direct access through the wound. We prefer a coronal hairline incision in most cases. In isolated (compound) fractures of the orbital roof, combined frontomaxillary fractures without dural involvement, and frontobasal frac

Complications in the treatment of adolescent clavicle fractures.

This study evaluated the complications associated with the treatment of clavicle fractures in adolescents. All cases of clavicle fractures were identified during an 8-year period between January 2005 and January 2013. During the study period, 153 clavicle fractures occurred in patients between the ages of 14 and 17 years who were treated at the authors' center. Medical records and radiographs were reviewed to determine injury mechanism, fracture pattern, treatment, and complications. Injury severity was assessed as high, medium, and low. Patients with medial or distal metaphyseal fractures and

[A woman with a painful swelling of the upper arm].

A 81-year-old woman presented at the emergency department with a painful right arm due to a comminuted fracture of the humeral head. Operation was postponed because of a wound and vulnerable skin due to a large hematoma. By the time of the operation, the humeral head had migrated subcutaneously which resulted in a solid swelling of the upper arm.

Thoracolumbar Burst Fracture without Neurological Deficit: Review of Controversies and Current Evidence of Treatment.

Thoracolumbar burst fracture without neurological deficit is a common spinal injury. The ideal classification for the diagnosis and the optimal management strategies, including conservative management, surgical approach, implant constructs, need for spinal fusion, and implant removal, are controversial and currently being investigated. This article reviews the current literature to provide updated evidence on these topics. Posterior ligamentous complex integrity plays an important role in the classification and decision-making process of treatment. A brace is not necessarily required in conser

Treatment of Distal Metaphyseal Tibial Fractures with Anterolateral Plates or with Anterolateral-Medial Plates: A Retrospective Series.

Distal metaphyseal tibial fractures (3-5 cm from the joint with zones of comminution or <3 cm from the joint) are challenging to fix and are associated with many complications. The study objective was to evaluate the functional outcomes and complications after treating distal metaphyseal tibial fractures using anatomical anterolateral tibia locking plates or anterolateral-medial plates. This retrospective study included 57 patients with distal metaphyseal tibial fractures. Thirty patients were treated by open reduction internal fixation with anterolateral plates; 27 patients were treated with

Rotational stability in screw-fixed scaphoid fractures compared to plate-fixed scaphoid fractures.

BACKGROUND: The literature describes the treatment of scaphoid fractures comparing the volar and dorsal approaches, the advantages and disadvantages of percutaneous screw fixation, as well as the treatment of scaphoid nonunions using different types of cancellous or corticocancellous bone grafts. Yet, to date no studies are available comparing the outcome of rotational stability in screw-fixed scaphoid fractures to angular stable systems. The purpose of this study is to provide reliable data about rotational stability in stabilised scaphoid fractures and to gain information about the rigidity

Flexor carpi radialis tendon rupture as a complication of a closed distal radius fracture: a case report.

Although extensor pollicis longus tendon ruptures have been noted as a complication of distal radius fractures, flexor tendon ruptures in association with acute fractures of the distal radius are rare. We report a rupture of the flexor carpi radialis tendon as a complication of an acute distal radius fracture that was discovered during operative management of the fracture.

Trans-Table Intraoperative Fluoroscopic Technique for Obtaining a True Lateral View of the Proximal Femur in the Lateral Decubitus Position.

The management of proximal femoral fractures, especially comminuted subtrochanteric ones, poses a surgical challenge. It is relatively easier to perform the open reduction of these fractures in the lateral position on a standard radiolucent table, but obtaining an accurate lateral view of the femoral head and neck remains difficult. This study presents a method that overcomes the limitations of fluoroscopy in the lateral decubitus position and improves the accuracy of obtaining a true lateral view. The technique involves positioning the patient in the lateral decubitus position with the unaffe

[Outcomes of minimally invasive plate osteosynthesis (MIPO) with volar locking plates in distal radius fractures: A review].

Minimally invasive plate osteosynthesis (MIPO) has been used in recent years to treat fractures of the distal radius with volar locking plates. Its advantages are the preservation of the pronator quadratus and good esthetics. The MIPO technique was described originally with two incisions: one distal transverse or longitudinal incision and one proximal longitudinal incision. The trend is now to use a single longitudinal incision less than 20mm long. Functional and radiological outcomes are comparable to those of conventional techniques. The MIPO technique is indicated for extra-articular and in

An alternative method of fixation of calcaneal tuberosity fractures using the Tightrope(®) technique.

Calcaneal tuberosity fractures account for 1% to 3% of all calcaneal fractures. These fractures are frequently seen in the osteoporotic or diabetic elderly population. The patient's comorbidities, coupled with the usually osteoporotic bone, make fixing this fracture pattern a challenge. Numerous surgical techniques have been advocated for this fracture, including the standard lag screw fixation, tension band wiring, suture anchors, and direct suture repair. Whichever method is used, the construct must resist the massive pull of the gastrocnemius-soleus complex. We have described a method of fi

Operative fixation of a subtrochanteric fracture in a patient with previous spontaneous hip fusion.

Patients with a fused hip can present with various clinical problems, such as contralateral hip arthritis, ipsilateral knee arthritis and degenerative disc disease. There are significant deforming forces at the proximal femur in patients with a fused hip who have ipsilateral subtrochanteric fractures. We report a 66-year-old patient with a spontaneously fused right hip, who sustained an ipsilateral comminuted proximal femoral fracture secondary to a road traffic accident. Optimal operative fixation of this patient posed as a surgical challenge. We discuss the management options in this paper.

Revision Surgery for Occlusal Correction in Facial Trauma.

Revision surgery for occlusal correction in facial trauma is a highly specialized and technically demanding aspect of reconstructive surgery. While contemporary practices in primary fracture management have reduced the incidence of posttraumatic malocclusion, circumstances such as delayed treatment, insufficient fracture reduction, overlooked injuries, and complex comminution continue to create challenging cases in which revision becomes inevitable. Effective secondary correction of occlusion involves a deep understanding of both trauma principles and orthognathic concepts, rigorous diagnostic

Surgical Considerations in the Treatment of Osteoporotic Proximal Humerus Fractures.

Proximal humerus fractures in the setting of osteoporosis can be technically challenging. Intraoperative challenges include comminution, thin cortical bone, and crushed cancellous bone that lead to difficulties in obtaining and maintaining a reduction and hardware fixation. Loss of fixation and varus collapse continue to be problems despite the utilization of modern locking plate fixation. A clearer understanding of predictors of fixation failure and the encouraging early results of reverse total shoulder arthroplasty (RTSA) have resulted in increased utilization of RTSA for the primary treatm

Spontaneous cecum perforation following rectus abdominis free flap transfer for isolated lower limb trauma.

A case of a 32-year-old motorcyclist, who sustained an open comminuted fracture of the left tibia and subsequently developed spontaneous cecal perforation following successful fixation of the fracture and reconstruction of the soft tissue defect with a rectus abdominis free flap, is reported. Although benign cecal perforation has been described in patients with thermal burns and blunt trauma of the abdomen or pelvis, our association has not been reported previously in the medical literature. It is important to recognize cecal perforation early as it is associated with a high mortality from per

Anaesthesia for major orthopaedic surgery in a child with an acute tracheobronchial injury.

A 12-year-old boy presented after a motorbike accident with mediastinal and cervical emphysema but no pneumothorax, minor head injury and several fractures including a comminuted open leg fracture. The child had no signs of respiratory compromise and was stable. The presumed tracheobronchial injury was managed conservatively. To avoid general anaesthesia and the risks associated with intubation and ventilation, urgent surgery for correction of his orthopaedic injuries was successfully conducted under spinal, epidural and intravenous regional anaesthesia. The surgical and anaesthetic management
