It is assumed, that patients with coronary heart disease (CHD) more often suffer from sleep related disorders of breathing than healthy subjects. A relation to an impaired left ventricular performance is discussed. In 40 CHD-patients and 30 cardio-respiratory healthy controls we therefore measured arterial oxygenation during sleep by means of pulse-oximetry. Our results show a marked increase in the frequency of nocturnal oxygen-desaturations along with the degree of impaired left ventricular function independent of a special sleep apnea risk. In case of cardiac insufficiency at rest cyclical

Pulmonary mechanics were studied with the constant pressure body plethysmography method in 78 infants during the first year of life. Registrations of breathing frequency, tidal volume, minute volume, dynamic compliance, pulmonary functional resistance, and end-expiratory resistance were made at rest and during carbon dioxide-induced hyperventilation. Data from 70 infants demonstrated strong correlations (p less than 0.001) between all pulmonary function parameters and length. Carbon dioxide-induced hyperventilation was achieved mainly by an increased tidal volume. Pulmonary functional resistan

The role of the parasympathetic nervous system, operating via the vagus nerve, in determining heart rate (f(H)) and cardiorespiratory interactions was investigated in the neotropical fish Piaractus mesopotamicus. Motor nuclei of branches of cranial nerves VII, IX and X, supplying respiratory muscles and the heart, have an overlapping distribution in the brainstem, while the Vth motor nucleus is more rostrally located. Respiration-related efferent activity in the cardiac vagus appeared to entrain the heart to ventilation. Peripheral stimulation of the cardiac vagus with short bursts of electric

We investigated the effects of aminophylline and isoproterenol on diaphragmatic fatigue produced by phrenic stimulation in dogs. With a cast around the abdomen, the diaphragm contracted quasi-isometrically while the thorax was open and the animal was ventilated. We assessed contractility by measuring transdiaphragmatic pressure during supramaximal stimulation of the phrenic nerves at different frequencies and also during spontaneous inspiratory efforts. At doses of 20, 40, and 80 mg/kg, aminophylline significantly improved contractility (p less than 0.01) in a dose-dependent manner during low,

The purpose of the present study was to clarify the effects of consciously controlled slow breathing on cardiac parasympathetic nervous activity Postexercise. Fifteen young healthy adult men participated in this study. They exercised on the bicycle ergometer at 50% of peak oxygen uptake for 10 min and then rested on the chair for 10 min. Two conditions were performed, namely slow breathing at 6 breaths per minute (SLOW) and spontaneous breathing (CON) at recovery phase in randomized order. We measured the oxygen uptake (V̇O2), respiratory rate (RR), minute ventilation (V̇E), tidal volume (VT),

BACKGROUND: Despite the rapid widespread use of a high-flow nasal cannula (HFNC) during the COVID-19 pandemic, its indications and appropriate use as perceived by physicians remain poorly known.
METHODS: In September 2021, we sent a questionnaire to each respiratory physician from 15 institutions in Shizuoka prefecture, Japan. In this survey, we compared the perceptions of HFNC indications and interventions during implementation to those of non-invasive ventilation (NIV) and invasive mechanical ventilation (IMV). Furthermore, this study examined concerns about SARS-CoV-2 infection spread and

BACKGROUND: Patients with Chronic Obstructive Pulmonary Disease (COPD) are susceptible to respiratory failure which would ultimately lead to their hospitalization. Need to Non-Invasive Ventilation (NIV) is considered as the choice respiratory support in acute respiratory failure and is associated with a beneficial role in patients with COPD exacerbation. Hence, determining patients that would benefit NIV could be of great assistance.
OBJECTIVES: We aimed at evaluating the use of Rapid Shallow Breathing Index (a ratio determined by the frequency (f) divided by the tidal volume (VT)) in NIV req

OBJECTIVE: Accurate and validated predictors of outcome for infants with congenital diaphragmatic hernia are needed. Score for Neonatal Acute Physiology-II has been validated to predict mortality in newborns. We investigated whether Score for Neonatal Acute Physiology-II scores in congenital diaphragmatic hernia could predict mortality, need for extracorporeal membrane oxygenation (in patients born in a center with extracorporeal membrane oxygenation availability), and development of bronchopulmonary dysplasia (oxygen dependency beyond 28 d after birth) in survivors.

Over the past decade, the survival rate of infants with congenital diaphragmatic hernia (CDH) treated in the intensive care unit of the Royal Children's Hospital, Melbourne, has remained unchanged at 56% +/- 6%. Newer forms of treatment, such as extracorporeal membrane oxygenation (ECMO), high-frequency oscillation, and surfactant and nitric oxide therapy, are now available. The exact role of these therapies in the management of infants with CDH has not been determined. This study examines five clinical parameters derived from an infant's best preoperative ventilatory and blood gas data in the

INTRODUCTION: Critical Care Air Transport (CCAT) operational experience has steadily declined during recent low-tempo operations, with deployment readiness suffering from the "peacetime effect." The objective of this study was to compare critical care interventions performed by CCAT teams during high-tempo versus low-tempo operations.
MATERIALS AND METHODS: A retrospective analysis of the patient population and clinician-performed interventions during high-tempo (2007-2012) versus low-tempo (2015-2019) operations was conducted using abstraction from CCAT records in the Department of Defense T

To examine heart rate variability (HRV) and inflammatory markers as predictors for neurological injury in neonates undergoing therapeutic hypothermia for hypoxic-ischemic encephalopathy (HIE). We hypothesized that HRV would differentiate between infants with no/mild injury and infants with moderate/severe injury observed on MRI. Because HRV can be associated with the inflammatory cascade, cytokine concentrations were compared with the severity of brain injury indicated by MRI. Further, we studied the effect of temperature, sex, and mechanical ventilation on HRV. HRV was prospectively collected

Inborn errors of metabolism have not previously been recognized as a risk factor for acute respiratory distress syndrome (ARDS). We report this complication in four patients with defects of the mitochondrial trifunctional protein (MTP). This enzyme catalyses three steps in the beta-oxidation of long-chain fatty acids. Three of the patients were homozygous for the 'common' 1528G>C mutation in the alpha-subunit of the MTP, giving rise to long-chain 3-hydroxyacyl-CoA dehydrogenase deficiency. The fourth patient did not carry this mutation but had severely decreased activities of long-chain 3-hydr
