The Impact of Clinical Documentation Integrity Programs on Diagnosis Documentation.

BACKGROUND: Clinical Documentation Integrity (CDI) programs aid in documentation of precise diagnoses. In this study, the authors assessed the rate at which certain diagnoses were present in the Pediatric Health Information System (PHIS) database and evaluated changes in the hospital-specific case mix index (CMI) pre- and post-CDI program inception at six children's hospitals. METHODS: Between 2011 and 2017, CDI programs were implemented at six children's hospitals. The authors queried the PHIS database to compare the following diagnoses in 2010 and 2021: acute respiratory failure, chronic re

Assessing the Impact of Prenatal Medication for Opioid Use Disorder on Discharge Home With Parents Among Infants With Neonatal Opioid Withdrawal Syndrome.

OBJECTIVES: The number of women with opioid-related diagnoses in the United States has significantly increased in recent decades, resulting in concomitantly higher rates of infants born with neonatal opioid withdrawal syndrome (NOWS). Addressing prenatal opioid exposure is a priority for Alaska health systems. The objectives of this study were to: (1) identify maternal and neonatal factors associated with receipt of Medication for opioid use disorder (MOUD) and (2) determine the impact of prenatal MOUD on discharge to parents among infants with NOWS in 3 Alaska hospitals. METHODS: A retrospec

Hepatic encephalopathy for the hospitalist.

The care of patients with advanced liver disease is often complicated by episodes of acute decline in alertness and cognition, termed hepatic encephalopathy (HE). Hospitalists must be familiar with HE, as it is a common reason for hospitalization in this population and is associated with significantly increased mortality. This narrative review addresses common issues related to diagnosis and classification, precipitants, inpatient management, and transitions of care for patients with HE. The initial presentation can be variable, and HE remains a clinical diagnosis. The spectrum of HE manifesta

National health information infrastructure model: a milestone for health information management education realignment.

OBJECTIVE: Challenges and drawbacks of the health information management (HIM) curriculum at the Master's degree were examined, including lack of well-established computing sciences and inadequacy to give rise to specific competencies. Information management was condensed to the hospital setting to intensify the indispensability of a well-organized educational campaign. The healthcare information dimensions of a national health information infrastructure (NHII) model present novel requirements for HIM education. MATERIALS AND METHODS: Articles related to challenges and barriers to adoption of

'We're not there yet!': a qualitative study exploring the commissioning of adult Community Health Services in England to support the avoidance of hospital admissions.

OBJECTIVES: The increased use of Community Health Services (CHS) is central to UK policy visions of moving more care out of hospital to reduce pressure across the healthcare system and, in particular, the demand on secondary care, hospital services. CHS are under-researched, and little is known about how they can best contribute towards this aim. The National Health Service (NHS) in England has recently undergone a significant reorganisation, with an increased emphasis on collaborative service delivery. In the aftermath of this reorganisation, the objective of this study was to explore how com

Association of routine inflammatory biomarkers and derived indices with heart failure progression in preserved ejection fraction.

BACKGROUND: Heart failure with preserved ejection fraction (HFpEF) is driven by systemic inflammation, yet the prognostic utility of routine inflammatory biomarkers and derived indices remains underexplored. This study evaluates the association of high-sensitivity C-reactive protein (hsCRP), albumin, neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), systemic immune-inflammatory index (SII), and pan-immune-inflammatory value (PIV) with HFpEF outcomes. METHODS: In a retrospective cohort of 312 HFpEF patients (mean age 72.6 ± 10.1 years, 54% female) from Tokat Gaziosmanpa

The Impact of the West Africa Ebola Outbreak on Obstetric Health Care in Sierra Leone.

BACKGROUND: As Sierra Leone celebrates the end of the Ebola Virus Disease (EVD) outbreak, we can begin to fully grasp its impact on already weak health systems. The EVD outbreak in West Africa forced many hospitals to close down or reduce their activity, either to prevent nosocomial transmission or because of staff shortages. The aim of this study is to assess the potential impact of EVD on nationwide access to obstetric care in Sierra Leone. METHODS AND FINDINGS: Community health officers collected weekly data between January 2014-May 2015 on in-hospital deliveries and caesarean sections (C-

Factors associated with adherence to guidelines for the use of tigecycline in a tertiary care hospital.

We assessed the adherence to the prescribing hospital protocol for tigecycline and factors associated with noncompliance. A total of 103 patients were included in the study. In 23 (22.3%) patients, tigecycline was not administered according to the protocol, mostly because of the availability of other therapeutic alternatives and prescription for indications that were not included in the guidelines. factors independently associated with nonadherence to the protocol were community-acquired infection (OR, 14.01; 95% CI, 1.54-127.12; P=0.019), and empirical tigecycline treatment (OR, 6.97; 95% CI,

Community-Acquired Legionella pneumophila Pneumonia: A Case-Control Study in Adult Inpatients from 2019 to 2024.

BACKGROUND: Legionella pneumophila is frequently acquired in the community and generally linked to contaminated domestic hot water systems, hotels, or other environmental sources or of unknown origin. L. pneumophila is a leading cause of pneumonia, especially in high-risk patients such as those over 50 who are immunocompromised or people with pre-existing illnesses. This study examines the factors linked to L. pneumophila acquired in the community in adult patients with hospitalization due to pneumonitis. METHODS: This case-control study included 140 hospitalized adult patients admitted to th

Thoracic ultrasound use in hospitalized and ambulatory adult patients: a quantitative picture.

INTRODUCTION AND OBJECTIVES: Thoracic ultrasound (TUS) has been established as a powerful diagnostic and monitoring tool in the Intensive Care Unit (ICU). However, studies outside the critical care setting are scarce. The aim of this study was to investigate the value of TUS for hospitalized or ambulatory community patients. MATERIALS AND METHODS: This was a retrospective study conducted from 2016 to 2020 in the TUS clinic at Heraklion University Hospital. TUS examination was performed using a standard ultrasound machine (EUB HITACHI 8500), and a high-frequency microconvex probe (5-8 MHz). Pa

Identifying Local Facilitators and Barriers to Screening Mammography Within a Rural Acute Care Hospital Service Area.

INTRODUCTION: Women living in rural areas are more likely to be diagnosed with advanced-stage breast cancer than their urban counterparts. The advanced stage at diagnosis is potentially attributable to lower rates of mammogram screening. We aimed to elucidate factors affecting women in decision-making about mammogram screening in a rural area in Wisconsin served by a critical access hospital. METHODS: We conducted an observational cross-sectional mixed-methods study, collecting data from various sources using 3 methods. Hospitalization experience of ethno-racial older adults with dementia and caregivers: scoping review.

BACKGROUND AND OBJECTIVES: Dementia and hospitalization are complex, interwoven experiences, particularly for older adults from ethno-racial communities. Systemic racism, stigma, health care provider bias, and the lack of culturally and linguistically appropriate care impede access to equitable care while admitted to the hospital. This scoping review aims to describe the existing literature on the hospitalization experiences of ethno-racial older adults (age ≥ 60 years) with dementia and their caregivers and identify challenges in receiving care in hospitals. RESEARCH DESIGN AND METHODS: A sc

The impacts of COVID-19 hospitalizations on non-COVID-19 deaths and hospitalizations: A panel data analysis using Brazilian municipalities.

The COVID-19 pandemic in Brazil has brought many challenges, particularly regarding the management of hospital capacity, and a new demand for healthcare that added to the preexisting demands, such as neoplasms, cardiovascular diseases and births. In this paper, we estimate the impact of the pandemic on the number of deaths and hospitalizations for other diseases. We construct a monthly panel data of deaths and hospitalizations for various causes by the municipality of residence and relate them to COVID-19 hospitalizations using regression models that control for municipalities fixed-effects an

Effectiveness of Nurse-Led Case Management in Community Settings Among Older Adults: A Systematic Review and Meta-Analysis.

INTRODUCTION: Integrated, people-centred care requires continuity and coordination across services, and nurses increasingly contribute to case management; however, evidence on its effectiveness for older adults living at home remains limited. This systematic review aimed to evaluate the effectiveness of nurse-led case management for community-dwelling older adults compared with usual care. METHODS: PubMed, CINAHL, Embase and the Cochrane Central Register of Controlled Trials were searched from inception to 13 August 2025, for randomised controlled trials comparing nurse-led case management wi

Dissemination Routes of Carbapenem and Pan-Aminoglycoside Resistance Mechanisms in Hospital and Urban Wastewater Canalizations of Ghana.

Wastewater has a major role in antimicrobial resistance (AMR) dynamics and public health. The impact on AMR of wastewater flux at the community-hospital interface in low- and middle-income countries (LMICs) is poorly understood. Therefore, the present study analyzed the epidemiological scenario of resistance genes, mobile genetic elements (MGEs), and bacterial populations in wastewater around the Tamale metropolitan area (Ghana). Wastewater samples were collected from the drainage and canalizations before and after three hospitals and one urban waste treatment plant (UWTP). From all carbapenem

The role of neurology in the development of community healthcare within the Italian national health service. The position of the Italian society of neurology (SIN).

Neurological disorders rank within the leading causes of disability and premature death in Europe and Italy, and their prevalence is expected to rise due to the aging population. In Italy, the current approach to neurological care is primarily centered in hospitals. However, an effective community-based neurology care system would ensure that patients receive appropriate care in the right setting, from the right healthcare professional, and at the right time. This "precision" approach would also alleviate the burden on acute neurology facilities. To address the growing demand for neurological
