Contribution of histological, serological, and genetic factors to the clinical heterogeneity of adult-onset coeliac disease.

OBJECTIVE: Although the factors predisposing to coeliac disease (CD) are largely understood, it remains unclear what determines the clinical heterogeneity of the disease. The aim of this study was to explore the contribution of histological, serological, and genetic factors to disease presentation. MATERIAL AND METHODS: The study was designed as a retrospective chart review of 384 unrelated Caucasian patients diagnosed with CD after the age of 16 at a single UK centre. RESULTS: We found that 8.8% of IgA-competent CD patients were endomysial antibody (EMA)-negative. Compared with the EMA-posi

Immunological Manifestations in GALE Deficiency: Extending the Spectrum Beyond Thrombocytopenia and Galactosemia.

BACKGROUND: Mutations in the UDP-galactose 4’-epimerase (GALE) gene disrupt the Leloir pathway of galactose metabolism and play a key role in protein and lipid glycosylation by providing essential nucleotide sugar donors. While GALE deficiency is known to cause galactosemia and thrombocytopenia, its impact on the human immune system remains largely unexplored. We aimed to characterize a novel immunodeficiency phenotype associated with the homozygous p.R51W GALE variant. METHODS: We performed a comprehensive immunological evaluation of seven pediatric patients (aged 2.5–10 years) homozygous for

Homozygous DIAPH1 Deficiency Without Cortical Blindness Presenting with EBV-Associated Hodgkin Lymphoma.

Background:Diaphanous-related formin 1 (DIAPH1) is a cytoskeletal protein belonging to the formin family. Homozygous loss-of-function variants in DIAPH1 have been associated with seizures, cortical blindness, mitochondrial dysfunction, and combined immunodeficiency. Here, we report a rare case of homozygous DIAPH1 deficiency presenting with the classical clinical features of the disorder but without cortical blindness.Case Presentation:An 11-year-old male patient presented with a productive cough. His computed tomography scan revealed diffuse bronchiectasis and chronic parenchymal infiltrative

Rapidly Progressive IgA Nephropathy in a Patient With a History of Breast Cancer: A Case Report and Literature Review.

IgA nephropathy (IgAN), or Berger's disease, is the most common primary glomerulopathy worldwide. It is characterized by dominant mesangial deposition of immunoglobulin A1 (IgA1), typically of the galactose-deficient form (Gd-IgA1), which triggers the formation of circulating immune complexes and subsequent glomerular inflammation. Clinical presentation includes hematuria, proteinuria, and, in severe cases, progressive renal failure. A rare variant is the extracapillary or crescentic form, which follows an aggressive course. We report the case of a 52-year-old woman with a history of Stage II

Serum steroid metabolome pattern is associated with the response to prednisolone therapy of male patients with IgA nephropathy.

IgA nephropathy (IgAN) is characterized by the deposition of galactose-deficient IgA1 (Gd-IgA1)-containing immune complexes into kidney mesangium leading to glomerular inflammation and injury. In patients with active IgAN non-responding to renin-angiotensin system blocking drugs (RASBs), corticosteroids (CSs) are recommended. Although CSs reduce significantly serum levels of Gd-IgA1, IgA-containing immune complexes, and proteinuria, their clinical effect in IgAN is variable. Because IgAN patients exhibit abnormal serum concentration of several endogenous steroid metabolites, some of which exhi

Significance of KM55 immunohistochemical staining in the diagnosis and prognosis of IgA nephropathy.

BACKGROUND: Galactose-deficient IgA1 (Gd-IgA1) plays a crucial role in IgA nephropathy (IgAN). The monoclonal antibody KM55 has emerged as a simplified method for detecting Gd-IgA1; however, the clinicopathological significance of immunohistochemistry for Gd-IgA1 remains underexplored. This study evaluated the prognostic and clinicopathological significance of KM55 immunohistochemistry in IgAN. METHODS: A total of 114 native kidney biopsies showing at least mild mesangial IgA positivity on immunofluorescence were retrospectively analyzed. Patients were categorized as having IgAN or non-IgAN d

Dioscin attenuates Gd-IgA1 overproduction by inhibiting the IL-6-JAK2/STAT3 signaling axis in IgA nephropathy.

BACKGROUND: Inflammation is a key driver in IgA nephropathy (IgAN) pathogenesis. Recent studies have demonstrated a significant association between pro-inflammatory cytokine–triggered inflammatory cascades, particularly those involving interleukin-6 (IL-6), and disease progression in IgAN. However, the underlying molecular mechanisms remain to be fully elucidated. Dioscin (DIO), a major active compound in Yiqi Qingjie Formula— a standardized formula for IgAN treatment at Guang’anmen Hospital—has shown well-defined anti-inflammatory and immunomodulatory properties. METHODS: Peripheral blood mon

Contemporary review of IgA nephropathy.

IgA nephropathy (IgAN) is considered the most common primary glomerulonephritis worldwide with a predilection for Asian-Pacific populations and relative rarity in those of African descent. Perhaps 20%-50% of patients progress to kidney failure. The pathogenesis is incompletely understood. Mesangial deposition of immune complexes containing galactose-deficient IgA1 complexed with anti-glycan IgG or IgA antibodies results in mesangial cell activation and proliferation, inflammatory cell recruitment, complement activation, and podocyte damage. Diagnosis requires a biopsy interpreted by the Oxford

Endothelial nitric oxide synthase inhibits the development of autoimmune-mediated vasculitis in mice.

OBJECTIVE: Many different genes or mediators have been implicated in promoting the development of vasculitis, although little is known regarding the mechanisms that normally act to suppress lesion formation. Endothelial nitric oxide synthase (eNOS) has been shown to inhibit vascular inflammation in many different model systems, but its roles in the pathogenesis of vasculitis have not been elucidated. This study was undertaken to determine the functions of eNOS in the initiation and progression of vasculitic lesion formation. METHODS: MRL/MpJ-Fas(lpr) mice lacking the gene for eNOS (Nos3(-/-)

Transcutaneous immunization with toxin-coregulated pilin A induces protective immunity against Vibrio cholerae O1 El Tor challenge in mice.

Toxin-coregulated pilin A (TcpA) is the main structural subunit of a type IV bundle-forming pilus of Vibrio cholerae, the cause of cholera. Toxin-coregulated pilus is involved in formation of microcolonies of V. cholerae at the intestinal surface, and strains of V. cholerae deficient in TcpA are attenuated and unable to colonize intestinal surfaces. Anti-TcpA immunity is common in humans recovering from cholera in Bangladesh, and immunization against TcpA is protective in murine V. cholerae models. To evaluate whether transcutaneously applied TcpA is immunogenic, we transcutaneously immunized

Immunoglobulin class switch recombination is impaired in Atm-deficient mice.

Immunoglobulin class switch recombination (Ig CSR) involves DNA double strand breaks (DSBs) at recombining switch regions and repair of these breaks by nonhomologous end-joining. Because the protein kinase ataxia telengiectasia (AT) mutated (ATM) plays a critical role in DSB repair and AT patients show abnormalities of Ig isotype expression, we assessed the role of ATM in CSR by examining ATM-deficient mice. In response to T cell-dependent antigen (Ag), Atm-/- mice secreted substantially less Ag-specific IgA, IgG1, IgG2b, and IgG3, and less total IgE than Atm+/+ controls. To determine whether

Toxic effects of subchronic T-2 toxin exposure on systemic immune deficiency in developing juvenile rats.

T-2 toxin, a potent immunosuppressive mycotoxin produced by Fusarium species, has not yet been fully elucidated regarding its immunotoxic effects under chronic exposure during developmental stages. To address the limitations of previous studies that primarily focused on single-organ assessments or acute toxicity evaluations in adult animals, this study established a 28-day subchronic T-2 toxin exposure model in Wistar juvenile rats (0-0.8 mg/kg body weight). We systematically evaluated its developmental immunotoxicity through three integrated dimensions: 1) immune organs (thymus, spleen), 2) i

Decreased vitamin A levels in common variable immunodeficiency: vitamin A supplementation in vivo enhances immunoglobulin production and downregulates inflammatory responses.

BACKGROUND: Vitamin A has a broad range of immunological effects, and vitamin A deficiency is associated with recurrent infections. Common variable immunodeficiency (CVI) is a group of B-cell deficiency syndromes with impaired antibody production and recurrent bacterial infections as the major manifestations, but the immunological dysfunctions may also include T cells and macrophages. In the present study we examined the possible role of vitamin A deficiency in CVI. PATIENTS AND METHODS: We analysed plasma vitamin A levels in 20 CVI patients and 16 controls, and examined the relationships bet

Immunodeficiencies in children with chronic post tympanic otorrhoea.

INTRODUCTION: A minority of children treated with ventilation tubes develop chronic otorrhoea. To test the hypothesis that this condition might be caused by an underlying primary immunodeficiency, the immunological status was examined in a group of children with longstanding otorrhoea. MATERIAL AND METHODS: Eighteen children who had suffered from otorrhoea for a minimum of six months and who did not respond to relevant therapy were included. Thorough cleansing and suction was performed including removal of ventilation tubes. Swabs were obtained for microbiology and blood was collected for imm

Hypogammaglobulinemia at Diagnosis is Associated With Inferior Survival and Higher Risk of Infections in Diffuse Large B Cell Lymphoma.

There are some evidences that hypogammaglobulinemia in newly diagnosed diffuse large B cell lymphoma (DLBCL) is a predictor for inferior outcome, but the risk for infection-related admissions specifically related to hypogammaglobulinemia is not known. The aim was to explore if hypogammaglobulinemia in untreated DLBCL in a Swedish cohort was associated with inferior outcome, and to assess the relationship between low immunoglobulin (Ig) levels and infections. Using data from the Swedish Lymphoma Register, we retrospectively identified patients above18 years diagnosed with DLBCL, receiving anthr
