Precision oncology in breast cancer: Biomarker-driven treatment strategies and the expanding role of APPs.

Recent advances in breast cancer management have transformed treatment across molecular subtypes and expanded the role of precision oncology. Hormone receptor-positive (HR-positive), human epidermal growth factor receptor 2-positive (HER2-positive), HER2-low, and triple-negative breast cancer (TNBC) each demonstrate distinct biologic behavior requiring tailored therapeutic strategies. In HR-positive disease, cyclin-dependent kinase 4/6 (CDK4/6) inhibitors and next-generation selective estrogen receptor degraders have improved survival, while HER2-directed antibody-drug conjugates (ADCs) have e

Durable Intracranial Response to Sacituzumab Tirumotecan in HER2-Negative Salivary Duct Carcinoma With High TROP2 Expression: A Case Report.

BACKGROUND: Salivary duct carcinoma is an aggressive salivary gland malignancy in which systemic therapy is often guided by androgen receptor and HER2 status. Treatment options are limited for HER2-negative disease after progression on androgen-deprivation therapy and chemotherapy, particularly when active central nervous system (CNS) involvement develops. CASE: A 62-year-old man with androgen receptor-positive, HER2-negative salivary duct carcinoma developed progressive intracranial dural metastases involving the left temporal and occipital regions after surgery, adjuvant radiotherapy, andro

[Development of Antibody-polymer Conjugates for the Treatment of Intractable Cancers].

Antibody therapeutics have become a major modality for cancer treatment. Particularly, immune checkpoint inhibitors have shown remarkable efficacy against various cancers. However, similar to other antibody therapeutics, they have not demonstrated clinical efficacy against malignant brain tumors. The primary reason for this is the presence of the blood-brain tumor barrier (BBTB) in the endothelial cells of malignant brain tumors, which prevents antibodies from entering the tumor parenchyma. Additionally, treatment with immune checkpoint inhibitors is clinically challenged by the occurrence of

Brentuximab vedotin monotherapy for CD30-positive adult T-cell leukemia/lymphoma with HTLV-1-infected Hodgkin and Reed-Sternberg-like cells.

Adult T-cell leukemia/lymphoma (ATLL) is an aggressive T-cell malignancy associated with human T-lymphotropic virus 1 (HTLV-1) infection. In this case study, we present a male patient diagnosed with CD30-positive ATLL, a rare pathological subtype characterized by the presence of HTLV-1-infected Hodgkin and Reed-Sternberg-like (HRS-like) cells. The patient was considered unsuitable for intensive chemotherapy due to advanced age and severe comorbidities. Consequently, the patient was treated with brentuximab vedotin (BV) monotherapy. Following five cycles of BV, the patient achieved partial remi

Trastuzumab deruxtecan (DS-8201a), a HER2-targeting antibody-drug conjugate, demonstrates in vitro and in vivo antitumor activity against primary and metastatic ovarian tumors overexpressing HER2.

High-grade serous ovarian cancer (HGSOC) and ovarian clear cell carcinoma (CC), are biologically aggressive tumors endowed with the ability to rapidly metastasize to the abdominal cavity and distant organs. About 10% of HGSOC and 30% of CC demonstrate HER2 IHC 3 + receptor over-expression. We evaluated the efficacy of trastuzumab deruxtecan (T-DXd; DS-8201a), a novel HER2-targeting antibody-drug conjugate (ADC) to an ADC isotype control (CTL ADC) against multiple HGSOC and CC tumor models. Eleven ovarian cancer cell lines including a matched primary and metastatic cell line established from th

Emerging Toxicities of Antibody-Drug Conjugates for Breast Cancer: Clinical Prioritization of Adverse Events from the FDA Adverse Event Reporting System.

BACKGROUND: Antibody-drug conjugates (ADCs) are gaining widespread use in the treatment of breast cancer, although toxicity remains an underexplored issue in the real-world clinical setting. Individual case safety reports collected in large pharmacovigilance databases can advance our knowledge on their safety profile in routine clinical practice. OBJECTIVE: We prioritized adverse events (AEs) reported with ADCs approved for breast cancer using the Food and Drug Administration Adverse Event Reporting System (FAERS). METHODS: We assessed clinical priority of AEs reported in FAERS (February 201

Outcomes with loncastuximab tesirine following CAR T-cell therapy in patients with relapsed or refractory diffuse large B-cell lymphoma.

The efficacy of loncastuximab tesirine (lonca) following chimeric antigen receptor T-cell therapy (CAR-T) progression/failure is unknown. Hence, we sought to examine real-world use and outcomes of lonca following CAR-T in patients with relapsed or refractory (R/R) diffuse large B-cell lymphoma (DLBCL) in the USA. In this retrospective study, we included adults (age ≥ 18 years) with R/R DLBCL who received lonca monotherapy as third- (3 L) or fourth line (4 L) treatment after progressing on second line (2 L) or 3 L CAR-T, respectively. Post-CAR-T lonca outcomes included response rates (overall r

Effects of Rovalpituzumab Tesirine on Ventricular Repolarization in Patients With Small-Cell Lung Cancer.

Small cell lung cancer (SCLC) is a leading cause of cancer death worldwide, with few treatment options. Rovalpituzumab tesirine (Rova-T) is an antibody-drug conjugate that targets delta-like 3 on SCLC cells to deliver a cytotoxic payload directly to tumor cells. In this study, the cardiac safety profile of Rova-T was assessed by evaluating changes in QT interval, electrocardiogram (ECG) waveform, heart rate, and proarrhythmic adverse events (AEs) after treatment with Rova-T in patients with previously treated extensive-stage SCLC. Patients underwent ECG monitoring for 2 weeks after each of 2 i

BACKGROUND: Alzheimer's disease (AD) is a progressive neurodegenerative disorder and the most common cause of dementia. Diagnosing AD before symptoms arise will facilitate earlier intervention. The early diagnostic approaches are thus urgently needed. METHODS: The multifunctional nanoparticles W20/XD4-SPIONs were constructed by the conjugation of oligomer-specific scFv antibody W20 and class A scavenger receptor (SR-A) activator XD4 onto superparamagnetic iron oxide nanoparticles (SPIONs). The SPIONs' stability and uniformity in size were measured by dynamic light scattering and transmission

Development of new agents for peripheral T-cell lymphoma.

INTRODUCTION: Peripheral T-cell lymphoma (PTCL) is a relatively rare, heterogeneous group of mature T-cell neoplasms generally associated with poor prognosis, partly because of refractoriness against conventional cytotoxic chemotherapies. To improve the outcome of patients with PTCL, the clinical development of several novel agents is currently under investigation. AREAS COVERED: Since the first approval of pralatrexate (dihydrofolate reductase inhibitor) by the US Food and Drug Administration, belinostat, romidepsin (histone deacetylase inhibitors), and brentuximab vedotin (anti-CD30 antibod
