Tuberculosis control in the era of the HIV epidemic: risk of tuberculosis infection in Tanzania, 1983-1998.

SETTING: In Tanzania, a national tuberculosis programme (NTP) was established in 1979 based on the principles currently known as the World Health Organization DOTS strategy. From the period 1983-1987 to 1994-1998, notification rates of smear-positive tuberculosis increased from 32 to 69 per 100,000 population, mainly due to the human immunodeficiency virus (HIV) epidemic. OBJECTIVES: To estimate the trend in the annual risk of tuberculosis infection and to establish to what extent the opposing forces of improved tuberculosis control and HIV have had an impact on tuberculosis transmission. ME

Negative association between Helicobacter pylori infection and reflux esophagitis in older patients: case-control study in Japan.

BACKGROUND: Recent studies have clarified a close association between H. pylori infection and gastritis, peptic ulcer disease, and gastric cancer, but there is little information concerning the relationship between H. pylori infection and reflux esophagitis (RE). We investigated the relationship between H. pylori, RE, and corpus gastritis. SUBJECTS AND METHODS: Ninety-five patients with RE and 190 sex- and age-matched asymptomatic healthy controls demonstrating no localized lesions in the upper GI tract were studied and evaluated for H. pylori infection, histologic gastritis, serum gastrin, a

Sexual behaviour patterns and other risk factors for HIV infection in rural Tanzania: a case-control study.

OBJECTIVE: To examine the association between HIV infection and patterns of sexual behaviour and other risk factors in a rural Tanzanian population in a case-control study, nested within a randomized trial of improved sexually transmitted disease treatment. METHODS: All HIV-positive patients from the baseline survey of the randomized trial were eligible as cases. Cases (n = 338) and controls (a random sample of one in eight HIV-negative persons; n = 1078) were interviewed about risk factors for HIV infection using a structured questionnaire. RESULTS: A significantly higher HIV prevalence was

Nosocomial transmission and infection control aspects of parasitic and ectoparasitic diseases. Part III. Ectoparasites/summary and conclusions.

As a rule, both the standard of hygiene and sanitation prevalent in hospitals in the United States and the rarity of parasitic diseases compared to viral, bacterial, and fungal infections, reduce the hazard of nosocomial acquisition of parasites to relatively trivial levels. However, abetted by the resultant low index of suspicion on the part of clinical staff, certain parasitic microorganisms may at times cause significant morbidity and even mortality in both normal and immunocompromised patients, as summarized in this review. Also, the nosocomial acquisition of parasites may be somewhat unde

Field observations on adult zebu bulls immunised against anaplasmosis by infection and treatment plus integrated tick control.

Fourteen adult zebu bulls which were negative for Anaplasma marginale infection both serologically and on blood smear examinations were infected with a virulent Nigerian isolate of A. marginale. Forty days following the immunising infection when clinical reactions were established groups of seven animals were treated with either two doses of imidocarb dipropionate (2 X 5 mg/kg given intramuscularly 14 days apart) or a single intramuscular dose of long-acting oxytetracycline (5 mg/kg). Following clinical recovery, two weeks after the first treatment the immunised animals and seven susceptible c

National Trends in Healthcare-Associated Infections and Evaluation of Infection Prevention and Control Policies in Uzbekistan (2015-2024).

Background/Objectives: Healthcare-associated infections (HAIs) disproportionately burden low- and middle-income countries, but no such description has been published for Uzbekistan. We describe national trends in reported HAIs over 2015-2024 with their regional and clinical distribution and review the country's infection prevention and control (IPC) regulations against the eight World Health Organization (WHO) Core Components. Methods: We analysed the complete national series of reported HAI cases for 2015-2024 (ten annual counts; 817 cases in 2024, disaggregated by region and HAI type), with

Infection prevention and control programme and COVID-19 measures: Effects on hospital-acquired infections in patients with cirrhosis.

BACKGROUND & AIMS: Bacterial infections affect survival of patients with cirrhosis. Hospital-acquired bacterial infections present a growing healthcare problem because of the increasing prevalence of multidrug-resistant organisms. This study aimed to investigate the impact of an infection prevention and control programme and coronavirus disease 2019 (COVID-19) measures on the incidence of hospital-acquired infections and a set of secondary outcomes, including the prevalence of multidrug-resistant organisms, empiric antibiotic treatment failure, and development of septic states in patients with

Helicobacter pylori infection rates in dyspeptic Serbian HIV-infected patients compared to HIV-negative controls.

Helicobacter pylori infection does not belong to the spectrum of opportunistic infections in people living with HIV (PLHIV). To evaluate the Helicobacter pylori infection prevalence rate trends in HIV co-infected individuals in comparison to the HIV-negative population, we compared histopathological findings of H. pylori positive gastritis (gastritis topography and histopathology) between 303 PLHIV and 2642 HIV-negative patients who underwent esophagogastroduodenoscopy (EGD) between 1993 and 2014 due to dyspeptic symptoms. The prevalence of H. pylori infection was significantly higher in HIV-n

Aetiology and prevalence of mixed-infections and mono-infections in non-gonococcal urethritis in men: a case-control study.

OBJECTIVES: Chlamydia trachomatis (CT) and Mycoplasma genitalium (MG) cause the majority of non-gonococcal urethritis (NGU). The role of Ureaplasma urealyticum (UU) in NGU is unclear. Prior case-control studies that examined the association of UU and NGU may have been confounded by mixed infections and less stringent criteria for controls. The objective of this case-control study was to determine the prevalence and aetiology of mixed infections in men and assess if UU monoinfection is associated with NGU. METHODS: We identified 155 men with NGU and 103 controls. Behavioural and clinical infor

Association of CMV, HBV, or HCV co-infection with vaccine response in adults with well-controlled HIV infection.

Even after CD4 count recovery on antiretroviral therapy, HIV infection is associated with decreased response to most vaccines compared to the general population. Chronic infections with viruses such as cytomegalovirus (CMV), hepatitis B virus (HBV), and hepatitis C virus (HCV), which are more prevalent in HIV-infected populations, have been linked to immune dysfunction and decreased vaccine response in the general population. However, whether co-infection with these other viruses contributes to the decreased vaccine response seen in adults with well-controlled HIV infection is unknown. We cond

Chemoprophylaxis of Onchocerca infections: in a controlled, prospective study ivermectin prevents calves becoming infected with O. ochengi.

Onchocerciasis ('River Blindness'), caused by the filarial nematode Onchocerca volvulus is of major public health importance in West Africa. Ivermectin, a drug originally developed for veterinary use, is now being incorporated in control strategies but whilst it has potent efficacy against L1 larvae (microfilariae), ivermectin is not lethal to adult (L5) O. volvulus, nor to adults of the related cattle parasite O. ochengi. We have exploited this model to determine if ivermectin has prophylactic activity against naturally transmitted, O. ochengi infections in a controlled, prospective study in

Development of infection control and preventive nursing assessment tool for central line-associated bloodstream infection in selected hospitals in China: an exploratory sequential mixed-method study.

BACKGROUND: Extended hospital stays, higher hospital costs, and increased mortality are common central line-associated bloodstream infection (CLABSI) outcomes. Nurses' roles in preventing CLABSI are vital. Improving patient outcomes requires action to ensure intensive care unit (ICU) nurses' strict adherence with nursing practice guidelines. AIM: The aim of this study was to develop and validate a nursing assessment tool to enhance CLABSI prevention and infection control in Chinese ICUs. STUDY DESIGN: We used an exploratory sequential mixed-method design using a three-stage sequential proced…

Resistance to mycobacterial infection: a pattern of early immune responses leads to a better control of pulmonary infection in C57BL/6 compared with BALB/c mice.

In this study, we have compared the immunological responses associated with early pulmonary mycobacterial infection in two mouse strains, BALB/c and C57BL/6 known to exhibit distinct differences in susceptibility to infection with several pathogens. We infected mice via the intranasal route. We have demonstrated that BALB/c was less able to control mycobacterial growth in the lungs during the early phase of pulmonary infection. Our results showed that during the early phase (day 3 to week 1), BALB/c mice exhibited a delay in the production of TNF and IFN-gamma in the lungs compared to C57BL/6
