Effects of Telerehabilitation on Patient Adherence to a Rehabilitation Plan: Protocol for a Mixed Methods Trial.

BACKGROUND: Strong evidence supports beginning stroke rehabilitation as soon as the patient's medical status has stabilized and continuing following discharge from acute care. However, adherence to rehabilitation treatments over the rehabilitation phase has been shown to be suboptimal. OBJECTIVE: The aim of this study is to assess the impact of a telerehabilitation platform on stroke patients' adherence to a rehabilitation plan and on their level of reintegration into normal social activities, in comparison with usual care. The primary outcome is patient adherence to stroke rehabilitation (up

Characteristics of swallowing disorders in patients with dysphonia.

INTRODUCTION: Coexistence of dysphagia with voice disorders is a topic rarely raised in the literature. Particular attention is paid to the aspect of dysfunction of laryngeal and pharyngeal muscles. AIM: The aim of the study was to analyse cases of patients with dysphonia in relation to coexistence of swallowing disorder. MATERIAL AND METHOD: The material of the study included 515 patients hospitalised due to dysphonia in 2018. Patients whose interview indicated swallowing difficulties were subjected to additional diagnosis for dysphagia (FEES, extended speech therapy test, SEMG). R esults:

Speaking Quality Improvement: Leveraging the PDSA Framework to Improve Dialysis Outcomes.

Improving patient care in the dialysis setting begins with empowering nurses and interdisciplinary teams to lead quality improvement initiatives. As dialysis care becomes more complex and regulatory metrics intensify through the ESRD Quality Incentive Program, the need for continuous improvement is critical. The Plan-Do-Study-Act (PDSA) cycle offers a practical, iterative framework for driving meaningful change in dialysis settings. A discussion of each phase followed by a detailed example addresses the current educational gap and illustrates how this framework supports quality outcomes. By em

The Trimodality PET/CT-MRI (3.0 T) System with a Dedicated Shuttle Detects Local Recurrence in a Case Classified as Indeterminate by Means of PSMA PET/CT Alone at Prostatic Fossa.

In the following case study, we evaluate a prostate cancer patient with persistent biochemical relapse and negative conventional imaging. Following interdisciplinary team discussion, it is decided to assess the patient by a 11 C-Choline and 68 Ga-PSMA positron emission tomography/computed tomography (PET/CT) scan. Notably, doubtful findings due to physiological urinary activity in the 68 Ga-PSMA-11 PET/CT scan were clarified with PET/CT-magnetic resonance imaging (MRI) images. The added value of the trimodality PET/CT-MRI system was very useful in detecting recurrence in a case classified as i

Taxing experiences: theorizing negative incidents at the frontlines.

Negative interactions with citizens can leave frontline workers stressed, anxious, and traumatized. Here, we construct a theoretically interdisciplinary framework for understanding frontline negative incidents that was thus far lacking. Drawing from 1302 descriptions of Dutch and Belgian frontline tax officials' most negative experience with citizens, we identify four overarching categories: citizen aggression, facing tragedy, violations of professional norms, and absence of support. We show the importance of non-aggression incidents and reveal how the nature, direction, and impact of these in

Interaction order and anxiety disorder: A 'Batesonian' heuristic of speaking patterns during psychotherapy.

This paper describes speaking practices enacted by young female in-patients during psychotherapy sessions. The patients are in treatment for anxiety and panic disorders (social phobias). The practices involve prosodic, lexical and pragmatic aspects of utterance construction. An effect that they share is that the speaker's embodied presence in her talk and her epistemic commitment to it are reduced as the utterance progresses. The practices are interpreted in light of Bateson's interactional theory of character formation: as elements of a self-sustaining system Angst (anxiety). The study has gr

Melanins: an old problem revisited or the usefulness of infrared spectroscopy.

This mini review remembers the implication, thirty nine years ago, of professor Wegmann in a fascinating adventure on the serendipitous study of melanin, after that a case of Dubin-Johnson's icterus had been submitted to him. To this investigation have been applied, one after the other and often simultaneously, for the first time, the diverse technologies of pathology, histochemistry, histoenzymology, biochemistry and infrared spectrography, having permitted to detect for the first time melanin in a human liver, associated with a melaninuria. But it was thanks to a marvellous collaboration wit

[Laparoscopy-assisted harvesting of free small intestine transplants for reconstruction of voice and deglutition after total laryngopharyngectomy--an animal experiment study].

Based on animal trials the presented study results describe a laparoscopic assisted removal of an extra-long free jejunal graft to reconstruct voice and swallowing after a total laryngo-pharyngectomy in cases with advanced malignancies of the larynx including invasion in the upper oesophagus. With a microvascular anastomosed small bowel segment, we have been able to reconstruct parts of the pharynx and to create a speaking syphon as devised by Ehrenberger in an one-stage procedure. The concept of avoidance of an extensive laparotomy may result in an decreased morbidity of this surgical procedu

Neuropathic Mechanisms in Phantom Pain: A Review of Pain Pathways and Peripheral Localization.

PURPOSE OF REVIEW: Phantom limb pain (PLP), the perception of pain in a limb no longer present, is a phenomenon affecting over 80% of amputees. Its prevalence induces severe pain and psychological distress causing a significant decline in the quality of life of amputees. PLP's complex interplay of peripheral and central mechanisms remains the leading cause for limited understanding and management of the condition. This article aims to examine PLP within the framework of pain localization by reviewing previous studies and investigating underexplored dimensions of PLP. RECENT FINDINGS: With the

The world is an unjust place: The impact of lifetime adversity on belief in conspiracy theories.

Conspiracy mentality - the tendency to view important events as the product of secret plots by powerful groups - is known to arise from collective victimization and discrimination. Yet less is understood about how adversities such as illness, financial loss, or trauma, unrelated to social identity, foster such beliefs. Drawing on social and clinical perspectives, we conducted three preregistered studies (total n = 1284) to test whether lifetime adversity contributes to conspiracy mentality via perceived injustice. Study 1 (n = 275) supported this hypothesis, showing an indirect link between li

Wealth, health, cats and Toxoplasma gondii: An ecological correlational study of subjective well-being across 93 countries.

National happiness is related not only to socioeconomic factors but also to biological and ecological factors. This ecological correlational study examined associations between national happiness, cat density, Toxoplasma gondii exposure, and socioeconomic indicators across 93 countries. We hypothesized that countries with higher socioeconomic indicators (e.g., GDP, health expenditure, income level) would exhibit higher happiness levels. In contrast, we predicted that higher T. gondii exposure, both acute (IgM) and chronic (IgG), would be negatively associated with national happiness. Data on h

[Impact of the high-altitude environment on preserved ratio impaired spirometry and its underlying mechanisms].

Preserved ratio impaired spirometry (PRISm) is an intermediate phenotype between normal lung function and chronic obstructive pulmonary disease (COPD), and it carries a high risk of progressing to COPD. Due to unique factors such as hypobaric hypoxia, the detection rate of PRISm is significantly higher in high-altitude environments than in plain areas. High-altitude hypoxia may contribute to the development and progression of PRISm through multiple mechanisms, including induction of systemic inflammatory responses, oxidative stress injury, abnormal activation of hypoxia-inducible factor (HIF)

Advancing Placental Pathology With Artificial Intelligence: Toward Reproducible and Predictive Diagnostics.

Placental pathology remains underdeveloped in its application of artificial intelligence (AI) and digital pathology, despite established diagnostic guidelines and increasing clinical demand. Determining which placentas require histologic examination continues to strain staffing and resources, while histologic interpretation is limited by high interobserver variability and a lack of scalable analytic tools. This review synthesizes current applications and future opportunities for AI in placental pathology across multiple domains, including gross examination, villous maturation, lesion detection
