Symptoms of narcolepsy/cataplexy developed in a monozygotic twin at the age of 56 years, 25 years after the onset of multiple sclerosis. The diagnosis of narcolepsy/cataplexy was confirmed by polygraphic recordings demonstrating sleep-onset periods of rapid eye movements (REM), increase in REM time per 24 hours, and disturbed nocturnal sleep. Frequent catapletic attacks were almost completely controlled by clomipramine. These symptoms may constitute one of the paroxysmal syndromes in multiple sclerosis. Visual evoked potential (VEP) latency to a sinusoidal grating pattern was measured in each eye of 103 patients with multiple sclerosis (MS) and compared with results in a control group of 56 patients hospitalized for other neurological conditions. Of the 50 patients classified as having definite MS, 90% showed prolonged latency (over 131 msec) in one or both eyes. A young woman with firm evidence of early multiple sclerosis (MS), who had never had measles, developed a clinically and serologically evident measles infection. Follow-up 1 year later confirmed the MS diagnosis. Thus, if measles can have a causal role in MS, it is probably not the only agent with this ability.

The National Research Council Twin Registry comprises 16,000 pairs of white male twins, both members of which had been in military service, mainly in World War II. All their available military and Veterans Administration records and their responses to a 1965 to 1970 NRC questionnaire have been coded as to disease. In a retrospective survey of acute optic neuritis 144 cases in adults were found and, of these, 127 were reviewed, five had died of causes related to multiple sclerosis, and 12 were lost to follow-up. There was a statistically significant seasonal variation in the incidence of optic neuritis. When the life-table method of analysis was used, the probability of developing multiple sclerosis rises to 78% 15 years after an episode of optic neuritis. At review, 49 (73%) of the 67 patients with multiple sclerosis were independent and leading active lives.

Bladder and urethral function was studied in 52 patients with multiple sclerosis using gas-cystometry combined with sphincter electromyography. 96% of the patients showed abnormalities of either detrusor or sphincter function. The abnormal patterns discovered indicated demyelinating lesions in the corticospinal as well as in the reticulospinal tract. Urgency and urge-incontinence were the predominant symptoms, whereas no signs of involvement of the upper urinary tract found. The use of gas-cystometry provides a fast and differentiated method for assessment of the detrusor function.

This case study presents a young woman who was admitted to our facility in an emaciated physical condition. She had multiple pressure sores and severe contractures about the neck, trunk, and lower extremities, necessitating dependence in all care. Slow but consistent progress was noted trhoughout a four-month aggressive rehabilitation program. The various treatment approaches and resulting functional gains are detailed.

The diagnostic value of the pattern-evoked response has been assessed in 73 patients referred because of suspected multiple sclerosis. Altogether 52 had delayed responses. Fifty-one patients in the group satisfied McAlpine's criteria for diagnosing definite, probable, or possible multiple sclerosis.
