Using a high-resolution real-time arc sector scanner, fetal cardiac activity was detected in ten normal intrauterine pregnancies, including one triplet gestation, between 41 and 43 days of gestation. Fetal heart rates ranged from 96 to 120 beats/min (mean 110) and were detected contiguous with the yolk sac as a tiny blinking, flashing, and/or rocking echo with a regular rhythm. 30 patients with severe aortic valve stenosis presented in severe congestive heart failure within the first 2 months of life. Systolic time intervals (STI) were measured in 20 control subjects and 20 cases of lepromatous leprosy in lepra reaction. Significant differences in the pre-ejection period (PEP), PEP/LVET and isovolumic contraction (IVCT) were observed between the groups. The abnormalities of STI observed in patients of lepra reaction are characteristic of left ventricular dysfunction in patients of lepra reaction.

Isoproterenol (IPR) induces myocardial lesions in R. pipiens adapted to +25 degrees C, but not to +8 degrees C. Extracardiac sounds secondary to cardiac pacing are well known, but the murmurs originating in the heart after pacemaker insertion are rare. A patient who developed musical systolic murmur following temporary transvenous endocardial pacemaker insertion is described. It is concluded that the murmur was produced by vibrations caused by the pacing catheter and the tricuspid valve or some other intracardiac structure.

Paroxysmal supraventricular tachycardia (PSVT) is associated with altered hemodynamics. We describe the echocardiographic features of myocardial dysfunction during a prolonged episode of PSVT in an 11-year-old male. Echocardiographic examination in a patient with primary pulmonary hypertension demonstrated midsystolic closure of the aortic valve, which has often been described in hypertrophic cardiomyopathy with or without obstruction, in discrete subaortic stenosis, and in ruptured aneurysm of the right coronary sinus of Valsalva. A possible mechanism for this finding in primary pulmonary hypertension and differential diagnosis from other diseases are discussed.

A possibility of the artificial ventricle regulation using information on the atrial contraction is discussed. Correlations between atrial wall tension and ventricle tension as well as the pressure under radom heart rhythm variations were studied. The wall tension of the appropriate heart portion was recorded by the arched tensiometers, and intraventricular pressure under cavity catheterization by the electric manometer. Application of a small dose of glyceryl trinitrate paste (0.25 in) resulted in severe left ventricular dysfunction with the appearance of pulsus alternans in a patient with alcoholic cardiomyopathy. The echocardiogram showed a reduction in the amplitude and velocity of motion of the left ventricular posterior wall on alternate beats. Leg elevation promptly reversed the pulsus alternans, and the echocardiographic abnormality. Glyceryl trinitrate paste should be used with caution and under close supervision in patients with liver dysfunction.

Intracoronary administration of contrast materials causes myocardial depression which is related to serveral physiochemical properties of the contrast solution. The role of variations in ambient calcium ions (Ca++) in mediating this effect was evaluated in 19 anesthetized dogs. Sodium meglumine diatrizoate caused decreases in left ventricular peak systolic pressure (LVPSP),-12.6 +/- 3.2%, and dp/dt at a left ventricular pressure (LVP) of 40 mm Hg, -14.3 +/- 4.1%.
