Downregulation of serum IGF-1 for treatment of early worsening of diabetic retinopathy: a long-term follow-up of two cases.

In 2003, we reported on 2 cases of nonproliferative and proliferative diabetic retinopathy, subsequent to HbA1c reduction by intensive insulin therapy (so-called early worsening of diabetic retinopathy). This acute condition could partly be reversed by discontinuation of intensive insulin therapy, whereby glycemia increased and serum IGF-1 concentration decreased [Ophthalmologica 2003;217:373-377]. On review 7 years later, both type-2 diabetic patients were on insulin therapy but had failed to achieve good glycemic control. One patient had mild background retinopathy on both eyes, with visual

Reduction of hyperopia and astigmatism after superficial keratectomy of peripheral hypertrophic subepithelial corneal degeneration.

PURPOSE: To report the development of progressive hyperopia and astigmatism resulting from peripheral hypertrophic subepithelial degeneration which was treated with superficial keratectomy of the lesion. METHODS: Case report. RESULTS: A patient with peripheral hypertrophic subepithelial degeneration of both corneas was followed up for a 6-year period. During this time, the patient experienced a slowly progressive increase in hyperopia and astigmatism in both eyes. In the left eye, extension of the degeneration paracentrally resulted in significant hyperopia and astigmatism and a best spectac

[A retrosellar arachnoid cyst as a rare cause of homonymous hemianopsia].

BACKGROUND: Arachnoidal cysts are cystic lesions filled with a cerebrospinal-fluid-like content within the leptomeninges. Usually arachnoidal cysts represent a congenital malformation. Clinical signs (increased intracranial pressure and/or neurological defects) may become apparent in children or young adults. PATIENT: A sixteen year old boy was referred to our outpatient clinic because of blurred vision in both eyes since five days. The patient complained of cephalgia and of an increased sleep requirement in the last two weeks. Morphological findings were within the normal range. The visual a

Toxoplasmosis neuroretinitis.

BACKGROUND: Neuroretinitis is a distinct clinical entity consisting of moderate to severe visual loss, optic nerve head edema, macular exudate in a stellate pattern, and variable vitreous inflammation. Although the etiology is usually postviral or idiopathic, an acute infectious cause occasionally is demonstrated. METHODS: Five juvenile or young adult patients with neuroretinitis are presented with serologic evidence of Toxoplasma gondii infection. Four of the five patients were treated with systemic antibiotics and corticosteroids; one patient was not treated. RESULTS: With a mean follow-up

[Proposals for a new partial permanent incapacity rate for aphakia].

Thanks to modern methods of correction in aphakia, (precornea lenses, and intra-ocular implants), the patient is less and less concerned by the major inconveniences linked to aphakia. It is, therefore, desirable to fix, in common law, the rate of partial permanent incapacity, according to whether the aphakic patient is well or poorly compensated by a prothesis. The residual global incapacity rate, would be the sum of the two following figures: 1 - A basic disability figure, of five to eight per cent taking into account the discomfort brought by the accessory consequences of the aphakic conditi

Unusual ocular manifestations of ethomoidal mucocele: a case report.

A 42-year-old female patient had vision loss and chronic epiphora in her left eye. Her best-corrected visual acuity was 10/10 in the right eye and 0.3/10 in the left eye. The anterior segment examination results were normal. In fundus examination, choroidal folds were detected. Optical coherence tomography showed elevation on the macula and choroidal folds. Ultrasonography revealed a T-sign. Magnetic resonance imaging revealed an ethmoidal mucocele that compresses the orbital tissues. Surgical treatment was performed in the otorhinolaryngology department. Postoperatively, choroidal folds recov

A case of Behçet's disease complicated by bilateral posterior ischemic optic neuropathy.

We report a case involving bilateral posterior ischemic optic neuropathy with Behçet's disease in a 49-year-old woman. The patient presented because of abrupt visual disturbances. An ophthalmic examination, including fluorescein angiography, visual field testing, and visual-evoked potentials, was performed. Her visual acuity was 20/100 bilaterally and posterior ischemic optic neuropathy was suspected. She was treated with systemic methylprednisolone; however, disc pallor developed and the visual acuity remained 20/200 bilaterally. Optic neuropathy can be bilateral, affect both eyes simultaneou

Transscleral sulcus fixation of a small-diameter iris-diaphragm intraocular lens in combined penetrating keratoplasty and cataract extraction for correction of traumatic cataract, aniridia, and corneal scarring.

We present the case of a 36-year-old Hispanic man who presented with photophobia and hand motion acuity from a lacerated cornea. Primary repair had been performed 13 years earlier. In addition to a densely scarred cornea and a fibrotic, partially resorbed cataract, more than 300 degrees of iris loss was noted. The patient was treated with penetrating keratoplasty, cataract extraction, and implantation of a transsclerally fixated, small-diameter aniridic intraocular lens (IOL). Despite the significant iris loss, a small-diameter IOL was chosen over a standard larger aniridic IOL to allow safer,
