PURPOSE: A retained placenta after vaginal delivery where manual removal of placenta fails is a clinical challenge. We present six cases that illustrate the heterogeneity of the condition and discuss the etiology and terminology as well as the clinical management.
METHODS: Members of the European Working group on Abnormally Invasive Placenta (EW-AIP) were invited to report all recent cases of retained placenta that were not antenatally suspected to be abnormally adherent or invasive, but could not be removed manually despite several attempts.

We assessed 5 patients with histologically/clinically confirmed placenta accreta after first-trimester abortion. In 4 patients, sonography showed an unclear endometrium, absence of an endometrium-myometrium interface, a well-vascularized hyperechoic lesion in the uterine body with a low resistive index, and myometrial thinning near the lesion. METHODS: A total of 182 cases of singleton pregnancies with placenta previa were reviewed. Uterine cervical varix (CV) is a very rare condition during pregnancy and may cause moderate to severe hemorrhage. We present the third reported case of huge CV coexisting with placenta previa in the English literature. The "bladder-opening" technique can be applied to cesarean hysterectomy for placenta previa percreta with bladder invasion. Cutting the bladder lateral wall with an automatic stapling/cutting apparatus enables direct visualization of the invaded bladder area and its resection. This may reduce the amount of hemorrhage and avoid accidental bladder injury.
