Sudden unexpected death due to a brainstem glioma in an adult.

Sudden death due to undiagnosed central nervous system tumors is an uncommon, but well-described occurrence. Most of the tumors in these circumstances are supratentorial and occur in a wide spectrum of ages. Brainstem tumors are more rare and occur predominantly in the pediatric and adolescent populations. We present the case of a 48-year-old man who died suddenly and unexpectedly of a brainstem glioma. This case is unusual because of his age and the paucity of antecedent symptoms.

The defecation reflex in rats: fundamental properties and the reflex center.

While pharmacological and physiological studies in rats are now increasing, physiological properties of their defecation have been scarcely investigated. This study was performed to define the properties of defecation in decerebrate rats, with special reference to the pontine defecation reflex center, which has been postulated in dogs. Intraluminal pressure was recorded from the colon and rectum with balloon-pressure transducer method using balloons of 15-20 mm in length and 0.1-0.3 ml in volume. Distention of a balloon in the descending colon and rectum with an additional injection of 0.03-0.

Surgically treated aneurysm of the trunk of the persistent primitive trigeminal artery--case report.

A 62-year-old man presented with a very rare cerebral aneurysm arising from the trunk of the persistent primitive trigeminal artery (PPTA) manifesting as subarachnoid hemorrhage. Angiography showed a saccular aneurysm at the curved mid-section of the trunk of the left PPTA of the adult type. The patient underwent surgery via the left subtemporal approach, and the aneurysm was successfully eliminated by clipping without sequelae. Aneurysm formation at this point was probably due to hemodynamic stress. The perforating arteries from the PPTA may be important in supply of the hindbrain, so PPTA pa

Idiopathic central pontine myelinolysis in childhood.

Central pontine myelinolysis (CPM) is rare in childhood with only a few cases reported in world literature. We report a 7-year-old male who presented with acute ataxia, swallowing difficulties, dysarthria, and radiological features consistent with the disorder. He improved remarkably with oral prednisolone therapy and was almost back to normal by 2 weeks. A review of the literature is also included.

Serial MRI of extrapontine myelinolysis of the basal ganglia: a case report.

We study a patient with extrapontine myelinolysis (EPM), without central pontine myelinolysis (CPM), who exhibited changes in abnormalities observed with MRI. MRI in the acute stage of the bilateral basal ganglia showed a high signal intensity on T2-weighted images, and a low signal on T1-weighted images. The low signal on T1-weighted images of the lesion in the acute stage changed to a high signal in the subacute stage. This change may reflect the involvement of microhemorrhagic lesions caused by vascular endothelial injury associated with EPM. We propose that vascular endothelial injury due

Analysis of ballistic movements in ataxic hemiparesis following a pontine stroke.

Although cerebellar-like ataxia is a well known component of the ataxic hemiparesis (AH), the mechanism of hypermetria in AH has not been established. We describe a patient presenting a left AH following a right pontine infarction. We investigated the ballistic flexion movements of both wrists and the associated agonist and antagonist electromyographic (EMG) activities, before and after addition of inertial loads. At the time of motion analysis, neurological examination showed cerebellar-like dysmetria of the left side but the patient had recovered a normal strength. In the basal state (withou

[Pure motor hemiplegia with ipsilateral lingual palsy caused by pontine infarction].

A 36 year old diabetic man developed a pure motor hemiplegia (PMH) associated with an ipsilateral lingual palsy. Magnetic resonance imaging revealed a pontine infarct. Lingual palsies have never been reported in patients with PMH so far, but may be associated with other lacunar syndromes such as the "dysarthria-clumsy hand syndrome". This observation supports the hypothesis that corticohypoglossal pathways may have bilateral and assymetrical projections.

[A case of the brainstem tactile hallucinosis due to pontine hemorrhage].

A 53-year-old man with hypertension was admitted to our hospital, for somnolence, horizontal gaze palsy, right hemiparesis and right sensory disturbance. Brain CT scan revealed a high density area from the left lower pontine tegmentum to the left tegmentum of the lower midbrain. As he became alert, he reported visual hallucination. Two weeks after onset of stroke, he complained tactile hallucination on his right half body with sensory disturbance. After disappearance of the visual hallucination, the tactile hallucination had been persisted. This is the first report of peduncular hallucinosis w

Unilateral symptomatic palatal myoclonus: MRI evidence of contralateral inferior olivary lesion.

Symptomatic palatal myoclonus is a rare syndrome of segmental myoclonus which is thought to occur after damage to certain brainstem or cerebellar structures. We report two patients with unilateral PM, who showed hypertrophy of the inferior olivary nucleus on magnetic resonance imaging. In the first patient this was due to a left pontine tegmental hemorrhage, and a right-sided oculo-palato-facial myoclonus developed one month after the episode. The second one had a basilar artery occlusion with ischemic infarcts at the basis pontis, dorso-lateral aspects of the left pons, and left cerebellar de…

Pontine myelinolysis following liver transplantation: a report of two cases.

Two cases of central pontine myelinolysis (CPM) following orthotopic liver transplantation (OLT) are reported. Several months after the onset of this neurological syndrome, the two patients are still alive but with severe neurological sequelae. Some patients undergoing emergency OLT present a high risk of CPM because of pre-existing malnutrition, encephalopathy, and hepatic insufficiency. All of these are associated with an inevitable abrupt rise in sodium serum concentration due to intraoperative compensation of blood losses with high-sodium content blood products. Whenever the renal capacity

Type I familial amyloid polyneuropathy and pontine haemorrhage.

A Portuguese female, aged 47 years, who had emigrated to Spain, was admitted to the hospital in 1991 for pontine haematoma. The patient, four siblings and her father were affected by a peripheral neuropathy, indicating autosomal dominant inheritance. The patient presented in the 2nd decade with sensory and motor neuropathy beginning in the lower extremities. Alternating constipation and diarrhoea, and urinary incontinence became uncontrollable. She had to be colostomised, and, eventually, confined to a wheelchair from the age of 43. Neurological examination showed bilateral facial involvement,

Pontocerebellar hypoplasias. An overview of a group of inherited neurodegenerative disorders with fetal onset.

Cerebellar hypoplasia is common to a variety of congenital disorders. Both stable conditions and progressive (degenerative) disorders may cause cerebellar hypoplasia. Pontocerebellar hypoplasia (PCH) is distinct from cerebellar hypoplasias in general, because the ventral pons is affected. Reviewing both clinical and neuropathological evidence, two specific neurogenetic entities are delineated. It is proposed to call these, respectively, type 1 (PCH-1) and type 2 (PCH-2). In type 1 the hallmark is the presence of spinal anterior horn degeneration similar to Werdnig-Hoffmann disease. Presentatio…

Central pontine myelinolysis: a case study.

Central pontine myelinolysis is a disorder affecting the myelin in the central pontine region of the brain. The disorder usually has a rapid onset which can result in death, even with the appropriate interventions. This paper presents the case of a seventy-one year old patient who developed central pontine myelinolysis and survived. The nursing challenges will be discussed as encountered in a primary nursing setting using a multidisciplinary approach.

Signal processing in brainstem auditory neurons which receive giant endings (calyces of Held) in the medial nucleus of the trapezoid body of the cat.

In the medial nucleus of the trapezoid body (MNTB), each principal neuron receives one large axonal ending (a calyx of Held) and many small … sult from antidromic activation of the neurons producing C2 spikes. Spontaneous and sound-evoked responses in dozens of cats anesthetized by barbiturates or Ketamine always had a C2 spike following each C1 spike. This implies that there is normally one-to-one spike transmission from the calyx of Held input to the MNTB principal neuron output. The small …

The projection of the nucleus reticularis tegmenti pontis and adjacent regions of the pontine nuclei to the central cerebellar nuclei in the cat.

The projection of the nucleus reticularis tegmenti pontis (NRTP) and the pontine nuclei (NP) to the central cerebellar nuclei (CCN) was investigated by means of anterograde transport of tritiated leucine. Although termination was found in all the CCN, it was most pronounced in the lateral nucleus and the lateral aspect of the posterior interposed nucleus. The extreme lateral aspect of the anterior interposed nucleus and the caudal part of the fastigial nucleus received a projection of modest intensity. Termination in the infracerebellar nucleus and group Y is likely to be present but could not
