The treatment of steroid-resistant minimal change nephrotic syndrome (MCNS) continues to pose a therapeutic challenge. We conducted a randomised prospective controlled trial to evaluate the efficacy of i.v. cyclophosphamide compared with oral cyclophosphamide in 13 children with biopsy-proven steroid-resistant MCNS. All 7 patients receiving i.v. cyclophosphamide achieved remission; this was sustained in 4 patients, while 3 relapsed. However, even these 3 patients subsequently became steroid sensitive. In five previously not treated patients with active primary idiopathic polymyositis/dermatomyositis and severe dysphagia persisting for 3-8 weeks treatment with a series of 4-5 intravenous pulses a 1 g methylprednisolone was provided. In four patients dysphagia disappeared within 2-14 days, in one patient within 30 days marked improvement was recorded. Two children, 1 with idiopathic nephrotic syndrome and 1 with endo-extracapillary glomerulonephritis, presented an episode of seizures and transient blindness at different times after i.v. pulse methylprednisolone (IVPMP) treatment. We have studied the pattern of variations in extent of thymidine incorporation during pulse-labelling in cultured chick pineal glands. During the first 24-30 h in culture the extent of pulse-labelling varied in parallel with the cycle in cumulative incorporation. An improved technique has been developed for determination of sulfites in food by differential pulse polarography. A Teflon sleeve is fitted to the dropping mercury electrode capillary so that SO2 is purged from the sample and simultaneously detected at peak potential. Bound sulfite in the sample is released at room temperature by addition of base in the absence of oxygen. Patients treated with barbiturate coma for elevated intracranial pressure after head injury may suffer brain death. Since such patients have an iatrogenically induced absence of neurological function, brain death cannot be diagnosed clinically. Methotrexate therapy has been effective in the treatment of RA with short term experience suggesting little serious adverse reactions. Our review of 168 patients receiving methotrexate has identified nine patients with probable or possible methotrexate-induced pulmonary toxicity, giving a prevalence of 5% and an incidence of 3.9 per 100 patients per year. A 36-yr-old patient suffering from chronic ulcerative colitis complicated by severe pyoderma gangrenosum of the leg is presented. Pyoderma gangrenosum rapidly progressed and failed to respond to a combined treatment of bed rest, prednisone, sulfasalazine, povidone-iodine, compresses, and debridement. Pyoderma gangrenosum responded and disappeared with no recurrence during the following months after the combined treatment of methylprednisolone, dapsone, and zinc sulfate.

A retrospective study of 191 patients undergoing 203 Burgess type below knee amputations for end-stage peripheral vascular disease showed that the only clinical feature which associated significantly with wound healing was the lowest palpable pulse. The pulse level was noted preoperatively and was compared with the healing rates in the amputation stumps. When the femoral pulse was absent, 79% of below knee amputations failed. When the femoral pulse was palpable, only 29% wound breakdown occurred. When the popliteal or more distal pulses were palpable, only 10% failure was recorded.

The 8-h double pulse oral glucose test is proposed as an alternative screening procedure for determining the adequacy of growth hormone (GH) release. The second pulse of glucose is timely in suppressing GH release and delaying it for a more predictable controlled elevation.
