Adhesion formation in experimental chronic radiation enteropathy.

We have studied the late changes associated with radiation enteropathy in mice over a period of 224 days following single or split doses of gamma radiation delivered to the total abdomen (TAI). We focused on the importance of adhesion formation as a cause of strictures and gut-associated deaths following TAI. Gut-associated peritoneal adhesions were found in mice 2-7 months after receiving 13.5-17.5 Gy TAI and appeared to constitute the most consistent serious late effect of irradiation. There was a good correlation between adhesion formation and death for both the single and split-doses of ra…

Radiation-induced lung injury: a hypersensitivity pneumonitis?

Radiation pneumonitis occurs 6 to 12 weeks after thoracic irradiation, and is thought to be due to direct radiation-induced lung injury. Four patients who developed pneumonitis after unilateral thoracic irradiation for carcinoma of the breast were studied with bronchoalveolar lavage, gallium scan of the lung, and respiratory function tests. On the irradiated side of the chest, all four patients showed an increase in total cells recovered from the lavage fluid and a marked increase in the percentage of lymphocytes. When results for the unirradiated lung were compared with results for the irradi…

Panretinal photocoagulation for radiation-induced ocular ischemia.

We present preliminary findings on the effectiveness of panretinal photocoagulation in preventing neovascular glaucoma in eyes with radiation-induced ocular ischemia. Our study group consisted of 20 patients who developed radiation-induced ocular ischemia following cobalt-60 plaque radiotherapy for a choroidal or ciliary body melanoma. Eleven of the 20 patients were treated by panretinal photocoagulation shortly after the diagnosis of ocular ischemia, but nine patients were left untreated. In this non-randomized study, the rate of development of neovascular glaucoma was significantly lower (p …

Glottic cancer. Surgical salvage for radiation failure.

Radiation therapy is a well-established treatment for early squamous cell carcinoma of the glottic larynx. However, the management of patients suspected to have persistent or recurrent disease may present both diagnostic and therapeutic dilemmas, particularly when laryngeal edema persists months after conclusion of radiation therapy. We reviewed the outcome of 212 patients with T1s, T1, and T2NOMO carcinoma of the glottic larynx who had been treated for cure with radiation therapy, with surgical salvage reserved for radiation failures. Twenty-four patients (11%) had recurrences after radiation…

Hypopituitarism following radiation therapy of pituitary adenomas.

Pituitary function was evaluated before and after supervoltage radiotherapy for a pituitary adenoma in 35 patients; 22 had had prior surgica … years. In contrast, in patients who had previously been treated surgically and who did not receive radiation, deficiencies of these hormones developed in only 13, 13, and 0 percent during four years of observation. It is concluded that supervoltage radiotherapy for pituitary adenoma, especially when preceded by surgical treatment, frequently produces, duri …

Lichen sclerosus et atrophicus following radiation therapy.

Several years after postmastectomy radiation therapy, lichen sclerosus et atrophicus (LSA) developed in two patients within the radiation fields. This sequence, to our knowledge, has not previously been described. The etiology of LSA is largely unknown, but trauma has been implicated as a provoking factor in some cases, and LSA might be an isomorphic response to the trauma of radiation therapy.

Radiation exposure during dynamic hip screw operation.

Exposure to radiation by surgeon and patient was measured in ten cases during insertion of a dynamic hip screw, using image intensified screening. There were low levels of irradiation of the thyroid and eyes of the surgeon. However, the dominant hand of the surgeon was often exposed to higher levels. The use of a memory during screening significantly reduced the level of radiation measured (P less than 0.05). Recommendations are made for reducing radiation exposure by the surgeon, and a method is described to measure the irradiation of surgeons' hands.

Early cardiac changes related to radiation therapy.

To investigate the incidence and severity of possible radiation-induced cardiac changes, 21 women without heart disease were investigated serially by echocardiography and by measuring systolic time intervals before and up to 6 months after postoperative radiation therapy because of breast cancer. Radiation was associated with a decrease in fractional systolic shortening of the left ventricular (LV) minor-axis diameter, from 0.35 +/- 0.05 to 0.32 +/- 0.06 (p less than 0.005), and in the systolic blood pressure/end-systolic diameter ratio, from 4.4 +/- 1.2 to 3.9 +/- 0.9 mm Hg/mm (p less than 0.…

Early glottic cancer: surgery or radiation therapy?

From 1970 to 1979 at the University Otorhinolaryngological Clinic, at the Radiology Institute of the University and at the Radiation Therapy Department of the Hospital of Florence, 385 early glottic cancers were treated: 300 by surgery and 85 with radiation therapy as the primary therapy. The policy of treatment in this period was in most cases surgery; those patients who could not be operated were treated with radiation therapy. The local control actuarial rates at 5 years were 85%, 70% and 65% for the T1a, T1b and T2 patients treated by surgery, and 86%, 83% and 57% for those treated with ra…

Primary radiation therapy for juvenile nasopharyngeal angiofibroma.

Evidence is presented of the effectiveness and relative lack of serious toxicity of external beam megavoltage radiation therapy (RT) as primary treatment for juvenile nasopharyngeal angiofibroma. The importance of careful radiological evaluation of tumor extent prior to irradiation is stressed, and only moderate dose RT is required. Fifty-five patients have been treated by RT and followed for from 3 to 26 years. Forty-four of 55 patients (80%) had permanent tumor control following a single course of 3000 cGy to 3500 cGy over 3 weeks. Surgical resection or a second course of RT controlled the t…

Symptomatic radiation-induced pericarditis in Hodgkin's disease.

This is a retrospective review of 193 evaluable patients treated with radiation therapy to the mediastinum for Stages I, II and III-A Hodgkin's disease. Eligible patients were those receiving 3000 rad or more to the mediastinum and no chemotherapy prior to the radiation. During the study period, 13 patients developed symptomatic pericarditis. The interval post treatment to the development of symptoms was six to 34 months. The incidence of pericarditis was studied as a function of: (1) the dose of radiation at a depth of 2 cm, 5 cm and the midplane of the mediastinum; (2) the ratio of anterior …

A radiopharmacological study without human radiation exposure.

The development, study and control of new drugs today is hardly conceivable without nuclear medicine studies. Nuclear physicians on ethical commissions bear great responsibility in the planning and execution of such studies. In order to protect subjects and patients those nuclear techniques are therefore to be welcome which do not include exposure to radiation. Nuclear techniques used in in-vitro diagnostics (RIA) and the determination of naturally occurring nuclides incorporated in the human body belong to this category. With the aid of a clinico-pharmacological study of a new combination of …

[Human bone marrow burden in dermatologic radiation].

The exposure to radiation of the entire red bone marrow was determined in three dermatologic X-ray treatment situations by means of a phantom in which the bones were supplied with 55 measuring sites. In each measurement the field was in the area of the left cheek. The parameters of radiation were in measurement 1/2/3: voltage = 50/43/43 kV, filter = 1.0/0.6/0.6 mm Al, field = 12.5/12.5/3.0 cm diameter, focus-skin distance = 30/30/15 cm, tissue half-value thickness = 13.0/7.5/5 mm. Related to a surface dose of 400 rad, the radiation exposure of the red marrow was 1555 mrad in measurement 1, 556…

Microvascular studies in human radiation bowel disease.

The microvasculature was investigated in the normal bowel (n = 43 patients) and in radiation bowel disease (n = 18 patients). Tissue samples obtained from postoperative colectomy specimens in which the intramural vessels had been perfused with barium sulphate suspension were examined. Microradiography was used to study vascular pattern which was abnormal in radiation bowel disease. A recently described radiograph fluorescence system was used to estimate barium concentration, and hence microvascular volume. The radiation group showed a highly significant reduction in barium concentration (p les…

Favre-Racouchot syndrome associated with radiation therapy.

A 56-year-old woman developed Favre-Racouchot syndrome involving her face and scalp primarily at the sites of x-ray irradiation for therapy of an astrocytoma. The patient had not had comedones prior to radiotherapy and did not have a history of excessive sun exposure. The patient showed an excellent response to topical retinoic acid gel. To the best of our knowledge, this is the first case of Favre-Racouchot syndrome developing after radiation therapy to be reported in the literature; its pathogenesis is discussed in this paper.

[Monitoring of radiation via CT (author's transl)].

The results presented in this article lead to the conclusion that it would be appropriate to demand control or monitoring of radiation via CT whenever complicated exposure to radiation is required. Such control appears mandatory in order to guarantee a sufficiently high tumour dosage and satisfactory protection or minimum burdening of the surrounding tissue. At first sight, it may seem that such CT operations are relatively costly. However, once the teamwork has been properly established, both the time required and the cost involved remain within reasonable limits and will probably not be grea…

Complications after radiation therapy for cervical carcinoma.

A series of 1 139 patients with invasive carcinoma of the cervix uteri who received a complete course of treatment by means of external irradiation followed by intracavitary curietherapy was evaluated in order to assess the results of treatment and, in particular, the radiation complications induced. Complications developed in 145 instances (12.7%). Severe complications occurred in only 3 per cent. Twenty-four cases had more than one complication. Rectosigmoidal injuries accounted for 72 per cent of all complications, and appeared earlier than did bladder complications and within the first two…
