Diagnostic utility of endoscopic ultrasonography for preoperative rectal cancer staging estimation.

The preoperative staging accuracy of endoscopic ultrasonography (EUS) was assessed in 38 rectal cancer patients who underwent rectal EUS and curative surgery from July 1992 to September 1994. We used a GF-UM20 instrument with both 12- and 7.5-MHz transducers. Compared with the histological findings, the diagnostic accuracy rate for EUS was 76% (29/38) for the invasion depth and 85% (sensitivity) and 72% (specificity) for level one lymph node metastasis, resulting in an overall preoperative staging accuracy of 74% (28/38). The diagnostic accuracy of invasion depth was poor (only 45%: 5/11) in c

New radiographic techniques for colorectal cancer.

Before surgical staging and resection, patients with primary colorectal cancer undergo a thorough diagnostic evaluation. The initial imaging study to identify a primary site of disease often is endoscopy, which frequently is supplemented with a double contrast barium enema. Once the presence of colorectal cancer has been established, new radiographic imaging techniques, including computed tomography (CT), magnetic resonance imaging (MRI), and special ultrasound (US) may be used. Clinical questions addressed by these special imaging techniques include detection of liver and lymph metastases and

Colonoscopic polypectomy as a tool for management of colonic polyps and detection of new lesions.

Over the past 14 years 1,000 polyps were removed via colonscope and histologically examined. There were 869 neoplastic polyps including 125 carcinomas. Non-neoplastic polyps included 32 inflammatory polyps, 32 metaplastic polyps, 27 juvenile polyps and 17 Peutz-Jeghers type polyps. Ninety-eight focal carcinomas were adequately treated by polypectomy only, whereas 9 out of 26 invasive carcinoma required further operations with one residual carcinoma in the pararectal tissue and colonic wall respectively. The criteria requiring additional surgery in invasive carcinoma are 1) lymphatic permeation

[Screening for colorectal cancer with a test for occult blood in stool (author's transl)].

Results of an occult-blood-test in 1 072 persons are reported. The test, recently registered as KRYPTO-HAEM, was developed by the Central Institute of Molecular Biology of the Academy of Sciences of the GDR. 27 patients (2.5%) had positive findings and by clinical examination 1 carcinoma of the sigmoid colon and 2 adenomas were detected. In 1 case (adenoma of the rectum) the test had a false-negative result. The findings were compared with own experiences with the HAEMOCCULT-Test. The KRYPTO-HAEM-Test is recommended for screening for colorectal tumors.

Elective resection for colorectal cancer in Vermont: 1971--1975.

An analysis of 612 patients undergoing elective resection for colon and rectal cancer in Vermont during the period 1971 to 1975 indicates an increasing incidence of cancers of the cecum and ascending colon, an increased number of Duke's A cancers, a synchronous cancer rate of 6 percent, an overall operative mortality of 6.5 percent, a wide variance in postoperative stay and a relatively small experience per operating surgeon. Only one third of colorectal cancers can be visualized through the rigid sigmoidoscope. Better screening techniques that are simple, reliable, inexpensive and safe must b

Failure to demonstrate specificity of the morphological and histochemical changes in mucosa adjacent to colonic carcinoma (transitional mucosa).

Characteristic morphological and histochemical changes in mucosa adjacent to colorectal adenocarcinomas have been described. It has been suggested that this type of mucosa, labelled transitional mucosa (TM) because of its specific association with colorectal adenocarcinoma, is indicative of a premalignant change. In an investigation of mucosa adjacent to anal tumours extending into the rectum and mucosa from solitary ulcer syndrome and colostomies the mucosal alterations described in TM could be demonstrated. Thus TM is not specifically related to colorectal adenocarcinoma and probably arises

Elevated serum acid phosphatase levels with rectal carcinoid tumor.

A case of rectal carcinoid tumor with liver metastases is reported in which a markedly elevated serum acid phosphatase level was found. Tissue assays of the patient's tumor, liver metastasis, and uninvolved liver were performed which demonstrated very high tumor levels of acid phosphatase. The patient also had elevated plasma serotonin levels and urinary 5-hydroxyindole acetic acid levels and did not exhibit the carcinoid syndrome. Autopsy showed no prostate cancer or metastatic bone lesions. Serum acid phosphatase elevation may occur with carcinoid lesions of the rectum.

The use of hyperbaric oxygen to treat actinic rectal fistula after SpaceOAR use and radiotherapy for prostate cancer: a case report.

BACKGROUND: In definitive radiation therapy for prostate cancer, the SpaceOAR® System, a hydrogel spacer, is widely used to decrease the irradiated dose and toxicity of rectum. On the other hand, periprostatic abscesses formation and rectal perforation are known as rare adverse effects of SpaceOAR. Nevertheless, there is a lack of reports clarifying the association between aggravation of abscesses and radiation therapy, and hyperbaric oxygen therapy (HBOT) is effective for a peri-SpaceOAR abscess and rectal perforation. CASE PRESENTATION: We report a case of a 78-year-old high-risk prostate c

Laparoscopic low anterior resection for rectal cancer with rectal prolapse: a case report.

BACKGROUND: Rectal cancer with rectal prolapse is rare, described by only a few case reports. Recently, laparoscopic surgery has become standard procedure for either rectal cancer or rectal prolapse. However, the use of laparoscopic low anterior resection for rectal cancer with rectal prolapse has not been reported. CASE PRESENTATION: A 63-year-old Japanese woman suffered from rectal prolapse, with a mass and rectal bleeding for 2 years. An examination revealed complete rectal prolapse and the presence of a soft tumor, 7 cm in diameter; the distance from the anal verge to the tumor was 5 cm.

Benign and malignant tumors of the rectum and perirectal region.

Although most rectal masses are histologically characterized as adenocarcinomas, the rectum and perirectal region can be affected by a wide variety of tumors and tumor-like conditions that can mimic the symptoms caused by rectal adenocarcinoma, including mucosal or submucosal rectal tumors such as lymphoma, gastrointestinal stromal tumor, leiomyosarcoma, neuroendocrine tumor, hemangioma, and melanoma, as well as tumors of the perirectal region such as developmental cyst, neurogenic tumor, osseous tumor, and other miscellaneous conditions. As a group, tumors of the rectum are considerably diffe

Rectourethral fistula and massive rectal bleeding from iodine-125 prostate brachytherapy: a case report.

Iodine-125 brachytherapy is an effective well-tolerated treatment for localized prostate cancer. Gastrointestinal complications of brachytherapy (minor rectal bleeding or tenesmus) are uncommon. Rectal ulceration or rectourethral fistulas after prostate brachytherapy are rare. We present a case of massive refractory rectal bleeding and rectourethral fistula, a complication of prostate brachytherapy never before reported. As a result of the patient's life-threatening symptoms aggressive surgical therapy was necessary.

Detection of a rectocele-like prolapse in the colonic J-pouch using pouchography: cause or effect of evacuation difficulties?

The functional outcome after a low anterior resection for rectal cancer is improved by a colonic J-pouch reconstruction. One functional problem with J-pouches is difficulty in evacuation, which is more common with large reconstructions. Since rectoceles are common findings on defecography in patients with evacuation difficulties, we proposed that a rectocele-like prolapse may be thus found in patients with large J-pouches. Pouchography was used to identify a rectocele-like prolapse (RP) in 26 patients with a 10-cm J-pouch (10-J group) and 27 patients with a 5-cm J-pouch (5-J group). Pouchograp

'Spontaneous ileorectostomy' complicating a leaking Hartmann's pouch. A case report.

Leakage from the closed rectal stump following Hartmann's procedure appears to be more common than previously suspected. A case of a leaking Hartmann's pouch further complicated by an ileorectal communication is reported. Non-operative attempts to seal this 'spontaneous ileorectostomy' failed.

Transanal endoscopic microsurgery.

Transanal endoscopic microsurgery (TEM) has emerged as a minimally invasive means of resecting rectal tumors. Developed in Germany and now being used with increasing frequency in the United States, TEM utilizes a 40-mm operating rectoscope, which is sealed with an airtight facepiece. Carbon dioxide is constantly infused, thereby distending the rectum and maintaining visibility. A variety of instruments, such as tissue graspers, a high-frequency knife, suction, and needle holders, are inserted through the facepiece. Adenomas that are small, large, or even circumferential, as well as selected ca
