A brain stem circuit integrating reflexive and anticipatory salivation.

Salivation supports oral health, taste sensation, swallowing, and digestion, but the brain mechanisms that regulate this process remain largely unknown. Here, we identify a salivatory center in the mouse brain stem that integrates sensory and learned anticipatory signals to control salivation. We show that activating choline acetyltransferase-expressing neurons in the inferior salivatory nucleus (IS) is sufficient to trigger saliva secretion. Using fiber photometry, we monitor real-time IS neural responses to mechanosensory and gustatory stimulation and find tight correlation with salivatory o

Selective facilitation of short latency postural reflexes by instability.

We sought to determine if enhancement of postural reflexes occurs specifically in response to instability induced by leaning and the possible neural level at which this may be mediated. Healthy participants (n = 10; 37 ± 15 years) were asked to adopt leaning postures to increase postural instability. Recordings of postural, eye and neck muscles and from over the cerebellum were made during stable upright stance and with the feet plantarflexed or dorsiflexed. The latter postures were used to modulate tonic activity levels in the soleus and tibialis anterior (TA) muscles to a similar degree to l…

Precision Induction and Distinction of Coughing and Sneezing Reflexes in Mice.

Coughing and sneezing, as vital respiratory defensive reflexes, have seen research into their mechanisms constrained by the nonspecific stimulation inherent in traditional models. Existing methods, such as capsaicin or citric acid nebulization, can simultaneously activate coughing (mediated by tracheal C fibers) and sneezing (mediated by nasal trigeminal nerves), leading to confounded behavioral observations. Although previous studies have attempted to distinguish coughing and sneezing by analyzing respiratory patterns or acoustic signatures, these studies lack models that can specifically sti…

Neuroendocrine cells initiate protective upper airway reflexes.

Airway neuroendocrine (NE) cells have been proposed to serve as specialized sensory epithelial cells that modulate respiratory behavior by c … phosphate (ATP) to activate purinoreceptive sensory neurons that initiate swallowing and expiratory reflexes. Our work uncovers the broad molecular and biophysical diversity of NE cells across the airways and reveals mechanisms by which these specialized excitable cells serve as sentinels for activating protective responses.

Comparison of the blink reflex in classical and idiopathic trigeminal neuralgia.

BACKGROUND: Previous findings indicate that the blink reflex is useful to distinguish between primary (classical/idiopathic) and secondary trigeminal neuralgia. No prior studies have investigated whether the blink reflex could identify differences in electrophysiological responses between classical and idiopathic trigeminal neuralgia. With this in mind, we investigated the blink reflex in a cohort of classical and idiopathic trigeminal neuralgia patients. METHODS: Participants were consecutively enrolled in the study. According to magnetic resonance imaging findings, the patients were subgrou…

Modeling and simulation of vagal afferent input of the cough reflex.

We employed computational modeling to investigate previously conducted experiments of the effect of vagal afferent modulation on the cough reflex in an anesthetized cat animal model. Specifically, we simulated unilateral cooling of the vagus nerve and analyzed characteristics of coughs produced by a computational model of brainstem cough/respiratory neuronal network. Unilateral vagal cooling was simulated by a reduction of cough afferent input (corresponding to unilateral vagal cooling) to the cough network. All these attempts resulted in only mild decreases in investigated cough characteristi…

THE VALUE OF BLINK REFLEX IN EARLY DIAGNOSIS OF MULTIPLE SCLEROSIS.

The aim was to determine differences of blink reflex in clinically definite multiple sclerosis (CDMS) and clinically isolated syndrome (CIS) in patients presented with symptoms and signs of brainstem impairment. The study included 20 patients diagnosed with CDMS, 20 with CIS, and 20 healthy controls. We recorded latencies of early (R1) and late component ipsilaterally (R2) and contralaterally (R2'), and occurrence of irritative component (R3). We analyzed data on sex, age, signs of brainstem impairment and magnetic resonance imaging (MRI) findings for the presence of brainstem demyelinating le…

The visual-ocular and vestibulo-ocular reflexes in vestibular migraine.

Vestibular migraine is a disease from the border of neurology and otoneurology. The diagnosis depends on patient history. There are no valuable laboratory or imaging tests, therefore the examination of visual-ocular and vestibulo-ocular reflexes is very useful in this group of patients. The material was collected from patients diagnosed due to dizziness at the Department of Otolaryngology and Oncology with the Subdepartment of Audiology and Phoniatrics, CM UMK in Bydgoszcz in 2019-2021. We considered patients with vestibular migraine and analyzed videonystagmographic tests in this study. The m…

Harnessing the Inflammatory Reflex for the Treatment of Inflammation-Mediated Diseases.

Treating diseases nonpharmacologically, using targeted neurostimulation instead of systemic drugs, is a hallmark of the burgeoning field of bioelectronic medicine. In this review, we provide a brief overview of the discovery and function of the prototypical neuroimmune reflex, the "inflammatory reflex." We discuss various biomarkers developed and used to translate early physiological discoveries into dosing parameters used in experimental settings, from the treatment of animal models of disease through a proof-of-concept clinical study in rheumatoid arthritis (RA). Finally, we relate how uniqu…

Reflex testing. A key tool for the elimination of hepatitis C.

Hepatitis C is a major cause of liver cirrhosis and hepatocellular carcinoma, as well as the primary indication for liver transplant in Europe. The highly effective direct-acting antivirals currently available make it possible to achieve the hepatitis C elimination targets set by the World Health Organization. For this, population screening and reflect testing are fundamental strategies.

The origins of the Lacto-Bolo reflex: the mythology of lactate in sepsis.

The use of lactate as a marker of the severity of circulatory shock was popularized by Dr. Weil in the 1970's. Dr. Weil promoted the idea that blood lactate concentration increased in circulatory shock due to anaerobic metabolism following decreased oxygen delivery. This concept becomes entrenched with 1992 ACCP/SCCM consensus conference definition of sepsis. Since then, the central role of lactate in the definition and management of septic shock has only been expanded and become more ingrained. This review will discuss the wisdom of such an approach, an updated model describing the origins of

The oculocardiac reflex and depth of anesthesia measured by brain wave.

BACKGROUND: The oculocardiac reflex (OCR), bradycardia that occurs during strabismus surgery is a type of trigemino-cardiac reflex (TCR) is blocked by anticholinergics and enhanced by opioids and dexmedetomidine. Two recent studies suggest that deeper inhalational anesthesia monitored by BIS protects against OCR; we wondered if our data correlated similarly. METHODS: In an ongoing, prospective study of OCR/TCR elicited by 10-s, 200 g square-wave traction on extraocular muscles (EOM) from 2009 to 2013, anesthetic depth was estimated in cohorts using either BIS or Narcotrend monitors. The depth…

Electrical Colon Stimulation Reflexively Increases Colonic Activity.

OBJECTIVES: Most individuals with spinal cord injury have neurogenic bowel dysfunction, which includes slowed colonic motility and has a significant impact on their health and quality of life. Bowel management typically includes mechanical rectal distension to evoke a recto-colic reflex and promote bowel emptying. Electrical stimulation could replace this mechanical distension. The purpose of this study was to determine the feasibility of evoking colonic activity using electrical stimulation. MATERIALS AND METHODS: Acute experiments were conducted in eight neural-intact cats under chloralose …

Brainstem reflexes are hyperactive in patients with drug-induced akathisia.

Akathisia is a sensori-motor phenomenon which is generally encountered as an adverse effect of antidopaminergic medications suggesting involvement of dopaminergic pathways. We recently showed nociceptive flexor reflex was altered in akathisia as compared to restless legs syndrome and therefore, these findings may indicate co-involvement of pathways other than dopaminergic ones. To examine functional status of different pathways, we investigated auditory startle reflex (ASR), startle response to somatosensory input (SSS), and trigemino-cervical reflex (TCR) in a group of patients with akathisia…
