Stay awake! Understanding, diagnosing, and successfully managing narcolepsy.

Sleep is a physiologic state that performs an essential restorative function and facilitates learning and memory consolidation. When sleep i … ep. Hypersomnia can result from several primary sleep disorders, including narcolepsy, sleep apnea, restless legs syndrome, idiopathic hypersomnia, and periodic limb movement disorder. The effects of some of these sleep disorders and other chronic illnesses on daytime sleepiness are measured using the Epworth Sleepiness Scale. Narcolepsy was found to cause some of the h …

Putting the periodicity back into the periodic leg movement index: an alternative data-driven algorithm for the computation of this index during sleep and wakefulness.

OBJECTIVE: To evaluate an alternative index for periodic leg movements during sleep (PLMS) and wakefulness (PLMW) expected to be similar to the standard index when leg movement activity is genuinely periodic, but significantly lower when periodicity is low. SUBJECTS AND METHODS: One-hundred-and-seven subjects with restless legs syndrome (RLS) were retrospectively identified and included (47 males, 60 females, mean age 56.9 years), along with 63 controls (33 males, 30 females, 42.2 years). Night-to-night variability was analysed in a subgroup of 17 subjects with RLS. PLMW were evaluated in a s…

Association of low ferritin with PLM in the Wisconsin Sleep Cohort.

OBJECTIVE: The origins of periodic leg movements (PLMs), a strong correlate of restless legs syndrome (RLS), are uncertain. This study was performed to assess the relationship between PLMs and peripheral iron deficiency, as measured with ferritin levels corrected for inflammation. METHODS: We included a cross-sectional sample of a cohort study of 801 randomly selected people (n = 1008 assays, mean age 58.6 ± 0.3 years) from Wisconsin state employee agencies. A previously validated automatic detector was used to measure PLMs during sleep. The patients were categorized into RLS symptoms-positiv…

Case report: A novel WASHC5 variant altering mRNA splicing causes spastic paraplegia in a patient.

Background: Hereditary spastic paraplegia (HSP) is a progressive upper-motor neurodegenerative disease. Mutations in the WASHC5 gene are associated with autosomal dominant HSP, spastic paraplegia 8 (SPG8). However, due to the small number of reported cases, the exact mechanism remains unclear. Method: We report a Chinese family with HSP. The proband was referred to our hospital due to restless leg syndrome and insomnia. The preliminary clinical diagnosis of the proband was spastic paraplegia. Whole-exome sequencing (WES) and RNA splicing analysis were conducted to evaluate the genetic cause of

Characterization of Neuropathic Pain in Primary Sjögren's Syndrome with Respect to Neurophysiological Evidence of Small-Fiber Neuropathy.

OBJECTIVE: To determine whether clinical features of neuropathic pain differ with respect to the presence of small-fiber neuropathy (SFN) in patients with primary Sjögren's syndrome (pSS). METHODS: We compared the clinical presentation of neuropathic pain between 15 patients with pSS and SFN detected by neurophysiological tests (laser-evoked potentials, cold and warm detection thresholds, sympathetic skin responses, and electrochemical skin conductance) and 15 patients with pSS but no neurophysiological evidence of SFN. RESULTS: The patients with SFN had more intense squeezing and pressure s

Lesson From a Case of Cervical Meningioma Misdiagnosed as Parkinsonism.

INTRODUCTION: Lesion localization based on patient's manifestation is a fundamental step in making a neurological diagnosis. However, it has been reported that diagnosticians are vulnerable to the effects of various cognitive biases during diagnostic processes. CASE REPORT: A 69-year-old man with right-hand stiffness visited the Movement Disorder Clinic with the history of periodic limb movement syndrome and restless leg syndrome. His sensory and deep tendon reflex examination results were normal. Brain magnetic resonance imaging was normal. Corticobasal degeneration was considered as a possi

RLS shows increased resting state sympathetic activity and decreased sympathetic response capability.

BACKGROUND: Restless Legs Syndrome (RLS) is a sensorimotor disorder characterized by painful discomfort, an urge to move the legs and circadian and sleep-wake disturbances. The pathophysiology is complex and not fully understood, with inhibitory dysfunction of dopaminergic neurons or increased sympathetic activity being discussed. Aim of this study was to shed light on the association between dysfunction of the autonomic nervous system and RLS. METHODS: 6 RLS patients diagnosed according to consensus criteria were compared with 9 gender- and age-matched healthy controls. Patients were clinica…

The Impact of Sleep Disorders on Cardiovascular Health: Mechanisms and Interventions.

This comprehensive review article explores the intricate mechanisms at work and possible remedies for the connection between sleep issues an … y for momentary relief. Cardiovascular illness has been linked to the sensorimotor problem known as restless legs syndrome (RLS), which causes a strong impulse to move the legs. Sleep disruption caused by RLS-related leg movements leads to sympathetic activation, elevated blood pressure, impaired vascular function, and potential iron deficiency. Treating the underlying …

Sleep-Related Disorders in Patients with CKD and Kidney Transplant Recipients.

Sleep disorders such as insomnia, restless legs syndrome (RLS), and sleep apnea (SA) are common in patients with CKD. These conditions tend to become more prevalent and more severe as kidney function deteriorates and when a patient reaches ESKD. The prevalence of insomnia in the general population ranges from 4% to 29% compared with ( 1 ) 30%-67%, ( 2 ) 39%-54%, ( 3 ) 41%-79%, and ( 4 ) 9%-49% in patients with CKD, on hemodialysis, on peritoneal dialysis (PD), or in kidney transplant recipients (KTRs), respectively. RLS occurs in approximately 1%-15% of the general population compared with ( 1…

Attention-deficit/hyperactivity disorder and sleep disorders in children.

Attention-deficit/hyperactivity disorder (ADHD) is a neurocognitive and behavior abnormality commonly seen in childhood and adolescence. Symptoms and consequences of ADHD and sleep problems frequently overlap, and their relationship is complex and bidirectional. To avoid inappropriate diagnosis and inadequate management, mental health professionals should assess sleep problems and disorders in children, adolescents, and adults with ADHD-related symptoms and in those with a diagnosis of ADHD. Screening for other psychiatric comorbidities and the side effects of medications, such as psychostimul

Lower limb motor restlessness in Asperger's disorder, measured using actometry.

The movement disturbances and brain imaging findings in Asperger's disorder (AD) suggest a dopaminergic deficit in movement regulation. Movement disorders of different etiologies have been quantified and specified with actometry. We compared 10 AD patients with 10 healthy controls, measuring their rest-activities by actometry. The lower limb motor activity was significantly higher in the AD group. They also displayed a rhythmic, periodic movement pattern similar to akathisia. These findings suggest a hypothesis of idiopathic akathisia and a special sensitivity to adverse effects of neuroleptic

Tiredness and somnolence despite initial treatment of obstructive sleep apnea syndrome (what to do when an OSAS patient stays hypersomnolent despite treatment).

From a database of 4,129 patients with sleep-disordered breathing (SDB), we found 207 subjects (43 women) that still complained of daytime tiredness, fatigue, and/or sleepiness despite treatment. In 25 subjects the sleepiness developed 1 to 36 months following treatment and was related to noncompliance (8 subjects), significant weight increase and/or inappropriate treatment (10 subjects), or development of new medical problems (7 subjects). In the remaining 182 subjects, sleepiness was noted within 1 month after what was judged appropriate treatment for SDB. In this group, the reason for persi

Current understanding of nociplastic pain.

Nociplastic pain by the "International Association for the Study of Pain" is defined as pain that arises from altered nociception despite no clear evidence of nociceptive or neuropathic pain. Augmented central nervous system pain and sensory processing with altered pain modulation are suggested to be the mechanism of nociplastic pain. Clinical criteria for possible nociplastic pain affecting somatic structures include chronic regional pain and evoked pain hypersensitivity including allodynia with after-sensation. In addition to possible nociplastic pain, clinical criteria for probable nociplas

[Nighttime eating disorders--clinical symptoms and treatment].

Nighttime eating is categorized as either night eating syndrome (NES) or the sleep-related eating disorder (SRED). Both diseases are often connected with an increase of the body mass, obesity, and with psychiatric disturbances. NES is characterized by evening hyperphagia, abnormally increased food intake after the evening meal, nocturnal awakings with ingestions, morning anorexia, and insomnia. Patients suffering from NES are aware of their nocturnal ingestions. It is suggested that NES is an abnormality in the circadian rhythm of meal timing that occurs in people with normal circadian rhythm
