PURPOSE: To investigate the spatial resolution requirements in digital radiography of scaphoid fractures.
MATERIAL AND METHODS: Included in the study were 60 scaphoid radiographs with and 60 without fractures of the scaphoid bone. The film-screen images were digitized using pixel sizes of 115, 170, and 340 microns along with 170 microns with a 10:1 wavelet compression. The digital images were displayed on a 1280 x 1024 x 8 bits monitor, and 5 observers evaluated the images in 5 randomized sessions. The results for each pixel size were then compared to the film-screen images by ROC analysis.

INTRODUCTION: The goal of this work is to determine, using the analysis of our own patient group and current professional publications, in which cases fractures of the scaphoid waist can be treated conservatively by short-term plaster fixation with a low risk of non-union developing.
MATERIAL AND METHOD: A group of 19 patients (17 men and 2 women) with the mean age 31 years (range 20-43, SD 7.3) with fractures of scaphoid waist were indicated for conservative treatment. The diagnosis was made on the basis of X-ray and CT examination. Conservative therapy consisted of applying plaster fixation

In scaphoid nonunion, pseudarthrosis and sclerotic change occur at the nonunion site. These changes make a distal fragment look like a trapezium on plain radiographs and we called this phenomenon the double trapezia sign. The purpose of this study was to estimate the diagnostic reliability of the double trapezia sign and its clinical and prognostic implications for the scaphoid nonunion.A retrospective review of 124 patients who underwent surgical treatment because of scaphoid nonunion between January 2007 and December 2017 was performed. Two hand surgeons and 1 musculoskeletal radiologist rev

The aim of the present anatomical study was to assess the dorso-radial approach for percutaneous fixation of scaphoid wist fractures. Through the anatomical snuffbox, cannulated screws or 1.2 mm K-wires were inserted into the scaphoids of 20 fresh-frozen cadavers. No tendon injuries were observed. There were two lesions of the radial artery, and three lesions of the sensory branches of the radial nerve. After dissection and three-dimensional CT reconstruction, the K-wire or screw position was described in relation to the scaphoid centroid and its longitudinal axis. The mean distance between th

We describe the case of a 58-year-old woman with AVN of the left trapezium which was treated surgically. This paper aims to review the etiologies leading to the pathology and the different therapeutic options. Avascular necrosis (AVN) of the carpal bones most commonly involves the lunate and the scaphoid . AVN of the trapezium is extremely rare. To our knowledge only 3 cases have been published in the literature so far and they were all treated differently.

BACKGROUND: Incorrect screw placement and penetration in screw fixation of scaphoid fractures are found in 5 to 30 %. Therefore, optimizing of screw placement is desirable, especially because an exact central position of the screw in the proximal fragment leads to a significant higher stability as a more peripheral position.
PATIENTS UND METHODS: 36 patients with an acute non-displaced scaphoid fracture were included in this randomized prospective study. 18 patients underwent navigated, the other 18 conventional percutaneous screw fixation of an acute non-displaced scaphoid fracture through a

We experienced two cases of simultaneous fracture of the scaphoid and the lunate. In two cases, both scaphoid and lunate fractures existed on the same plane, which may help us to understand the mechanism of proximal fracture of the scaphoid.

OBJECTIVE: To evaluate the effect to the fixation stability of central screw placement during scaphoid fracture surgery.
METHODS: We designed oblique osteotomies for 32 identical sawbone scaphoids and fixed each specimen with a cannulated screw. Sawbone scaphoids were divided into 4 groups, according to position of the osteotomy (distal waist portion or proximal waist portion) and the position of the screw (central or eccentric). We performed Computed Tomography scanning to one specimen, and then the central zone of the scaphoid was established from volume data by using the preoperative plann

One of the most frequent skeletal injuries to the body is a fracture of the distal forearm or wrist. These injuries commonly arise from a fall onto an outstretched hand. Plain radiographs are the mainstay in the initial evaluation of these injuries. Factors affecting injuries to the forearm and wrist depend on the age of the patient, severity of injury, exact mechanism and direction of force to the injury. However, wrist injuries in particular can be subtle on plain radiographs and therefore difficult to identify. It is essential to identify injuries early so that appropriate management is imp

Fractures of the scaphoid must be treated promptly to minimize the risk of malunion and nonunion. Although most scaphoid fractures have been reported to heal well with cast immobilization, advances in surgical techniques have greatly changed the decision-making process for treatment. It is worthwhile to review the different management options for acute fractures.

AIM: Unintended scaphoid excision is a rare but serious complication that can occur during surgical procedures involving the trapezium, including but not limited to trapeziectomy. Although prosthetic replacement is increasingly favored in the treatment of rhizarthrosis, trapeziectomy (with or without ligamentoplasty) remains a widely used and effective option. To reduce the risk of inadvertently removing the scaphoid, we rely on the anatomical intersection between the radial artery branch and the first dorsal compartment tendons as a landmark for identifying the scapho-trapezial joint.

Scapholunate (SL) instability is the most common form of carpal instability, and if untreated, it may progress through dorsal intercalated segment instability to scapholunate advanced collapse. Contemporary understanding emphasizes a dual stabilizing system in which the SL interosseous ligament (SLIL) acts as the intrinsic primary restraint, reinforced by extrinsic "critical" secondary stabilizers; isolated SLIL division does not by itself produce static malalignment until these secondary restraints are also compromised. Against this background, wrist arthroscopy has become a widely used refer

BACKGROUND Scaphocapitate syndrome is a rare clinical entity consisting of a combined scaphoid and capitate fracture along with a 90- or 180-degrees rotation of the proximal capitate fragment. The syndrome is scarcely described in the literature, with proximal migration of the capitate fragment being reported only by Mudgal et al in 1995. Concurrent compression of the median nerve is a highly unfortunate event, suggesting a unique case presented here. CASE REPORT We present a unique case of scaphocapitate fracture-dislocation in a 25-year-old man with volar dislocation of the capitate's fragme

CASE: A 29-year-old young active man with ununited necrosed proximal fifth of scaphoid with chronic scapholunate ligament disruption was managed by excision of proximal pole fragment and interosseous scapholunate reconstruction using modified Brunelli triple ligament tenodesis technique with satisfying outcome at 6 months and return to sports instructor job by the end of 1 year.
CONCLUSION: Meticulous understanding and algorithmic itemwise approach of injury components can lead to optimal management of complex unstable wrist injuries such as scapholunate dissociation. To the best of our knowl

INTRODUCTION: Non-dissociative carpal instability (CIND) may lead to severe functional impairment. Destabilisation of the scapho-trapezial-trapezoidal (STT) ligament complex seems to result in a CIND.
MATERIALS AND METHODS: In one group with eight cadaver arms, distal scaphoid pole was resected with the adjacent ligaments. In the other eight cadavers, hemitrapeziectomy was performed followed by total trapeziectomy. CT scans were performed in different wrist positions, and the changed positions of the scaphoid, lunate and capitate were measured in comparison to non-operated wrists.
RESULTS: M

OBJECTIVES: Gantry-free cone-beam CT (CBCT) allows for comfortable patient positioning due to an open scanner architecture. Since CBCT without gantry is not yet established for clinical wrist trauma imaging, this study's aim was to investigate its diagnostic value in the preoperative workup of patients with distal radius and scaphoid fractures.
METHODS: Within a 12-month period, 113 patients with severe wrist trauma underwent both radiography and CBCT with the same gantry-free multi-use scanner before surgery. Two radiologists retrospectively analyzed all datasets for the morphology of distal

PURPOSE: Early diagnosis and treatment of scaphoid fractures is critical to achieve union and prevent wrist degenerative arthritis. Plain radiographic measurements can guide the early approach to suspected fractures. Specifically, the ability to predict fracture or fracture stability from plain radiographs may allow the traumatologist to expand initial referral to three-dimensional imaging. We evaluated the ability of four measured angles to predict presence of a scaphoid fracture and stability.
METHODS: Fifty patients with a scaphoid fracture and 50 patients without fracture were evaluated f
