Surgical removal of giant acoustic neurinomas involving the skull base. Report of two cases.

Neurinomas arising from the peripheral branch of the acoustic nerve distal to the internal auditory canal in the temporal bone are rare. Two advanced skull-base neurinomas are described which were situated mainly in the temporal petrous bone, and extended to the parapharyngeal space anteriorly, to the lateral cervical portion inferiorly, into the sphenoidal sinus medially, and into the middle and posterior cranial fossae compressing the brain stem. Both patients had been deaf for several years without other neurological deficits. The operative findings revealed that the fifth, seventh, and cau

[The roof of the anterior ethmoid: a locus minoris resistentiae in the skull base].

Because of its special anatomic features, the roof of the anterior ethmoid is a preferred area for frontobasal fractures as well as an area … features of their topography like connection with the dura, fixation of the dura to the bone of the skull base and the variations of the thickness of the surrounding bony wall studied with regard to their surgical relevance. Not the dome of the ethmoidal roof proves to be the most critical point for lesions to occur, but the area where the anterior ethmoidal …

Surgical therapy of internal carotid artery lesions of the skull base and temporal bone.

Five cases of carotid artery lesions at the skull base or temporal bone, treated surgically, are presented. These lesions include mycotic aneurysms and carotid stenosis secondary to angiofibroma and a glomus caroticum. One case demonstrated an anomalous carotid anatomic pattern with a persistent stapedial artery. A description is provided of the technique of subtotal petrosectomy with permanent anterior displacement of the facial nerve, and middle ear obliteration, thus achieving a safe exposure of the temporal course of the internal carotid artery. The cases indicate that cooperation between …

Artificial intelligence-based prediction of cerebrospinal fluid leak in skull base surgery: a scoping review.

BACKGROUND: Cerebrospinal fluid (CSF) leak remains one of the most significant complications of skull base surgery, particularly in endoscopic endonasal and transsphenoidal approaches. Accurate risk stratification may improve surgical planning and reconstruction strategies. Artificial intelligence (AI)-based models have been increasingly investigated for CSF leak prediction; however, the evidence remains methodologically heterogeneous. This scoping review aimed to map the current literature and summarize data sources, modeling approaches, validation strategies, and reported performance. METHO…

The Impact of Aprepitant on Post-Operative Nausea and Vomiting in Endoscopic Skull Base Surgery.

OBJECTIVE: Post-operative nausea and vomiting (PONV) is common following endoscopic skull base surgery despite multimodal prophylaxis and carries a risk of post-operative bleeding and cerebrospinal fluid (CSF) leakage. This study aims to investigate if pre-operative administration of the neurokinin-1 (NK-1) antagonist, aprepitant, reduces PONV in patients undergoing endoscopic skull base surgery. METHODS: A retrospective review of 58 patients undergoing endoscopic skull base surgery was performed. Demographic variables and clinical variables such as anesthesia type, procedural time, intraoper…

The copy-number events in skull base chordoma stratify tumours into four biologically coherent groups.

Chordoma, a rare sarcoma of notochordal origin, exhibits slow growth and local aggressiveness. While copy-number (CN) events are recognized as key chordoma drivers, no comprehensive classification, based on CN, has yet been developed. Here, we establish a robust, reproducible genomic subtyping of chordoma, based on CN events. Two independent skull base chordoma cohorts (N = 32, N = 71) were analyzed, utilizing distinct analytical platforms, DNA methylation microarrays and whole-genome sequencing, both controlled for B-allele frequencies. Samples were clustered using unsupervised hierarchical m…

Pediatric sinusitis complicated by skull base osteomyelitis and neurovascular narrowing-two cases and literature review.

Central skull base osteomyelitis with sellar epidural abscess and neurovascular narrowing are a unique combination of imaging findings resulting from complicated acute paranasal sinusitis. Nonspecific clinical presentation and insidious onset with atypical initial imaging findings often pose a diagnostic challenge. We present two cases of acute sinusitis, which progressed to central skull base osteomyelitis and additional intracranial complications. Maintaining a high index of suspicion and a multidisciplinary approach during preoperative workup are essential for timely management.

Early Endoscopic Repair of Traumatic Skull Base Dural Injuries: Challenging the Dogma of Conservative Management.

INTRODUCTION: Traditional management of traumatic skull base dural injury has often favored observation and conservative therapy, despite the risk of delayed intracranial complications. This paradigm originated when operative intervention required craniotomy and carried significant morbidity. The objectives of this study were to evaluate outcomes of early endoscopic repair for traumatic skull base dural injury and to assess whether radiographic evidence of dural violation, including pneumocephalus, may support consideration of early intervention. METHODS: A retrospective review was performed …

Preoperative carotid stent reinforcement for recurrent skull base tumors: technical note and early feasibility series.

OBJECTIVE: Recurrent skull-base malignancies that circumferentially encase the internal carotid artery (ICA) in irradiated fields carry a major risk of vascular injury during repeat surgery. When balloon test occlusion (BTO) shows inadequate collateral supply, high-flow extracranial-intracranial bypass is often required to enable resection. We evaluated staged, segment-specific endoluminal ICA reinforcement with covered and open-cell stents to permit carotid-preserving re-resection. METHODS: We conducted a prospective single-center study of patients with recurrent skull-base tumors previously

Giant skull base collision tumors in NF2-related schwannomatosis: longitudinal outcomes of planned selective resection.

OBJECTIVES: Giant skull base collision tumors in neurofibromatosis type 2-related schwannomatosis (NF2-SWN) pose a complex surgical challenge in young patients with lifelong tumor burden. Gross total resection is often impossible without severe neurological morbidity, and data on operative timing and durability are limited. We assessed long-term outcomes of planned, selective resection prioritizing neurological preservation. METHODS: We queried a prospective institutional database for NF2-SWN patients undergoing surgery for giant (≥ 4.0 cm) intracranial skull base collision tumors (≥ 2 neopla…

Applications and prospects of artificial intelligence and digital medicine in pediatric nasal skull base tumors.

Pediatric nasal skull base tumors are rare and difficult to diagnose early due to their deep anatomical location and children's limited ability to describe symptoms. When the tumors progress to advanced stages, nonspecific symptoms such as nasal congestion, nosebleeds, and facial swelling are easily confused with sinusitis or trauma, complicating diagnosis and treatment. Moreover, the special tumor anatomical location and the narrow nasal cavity in children increase the risk of tumor involvement with the surrounding structures. Given children's longer life expectancy and higher postoperative q…

Anatomical Feature of Nasal Cavity for Endoscopic Endonasal Skull Base Surgery in Patients With GHoma.

BACKGROUND: Endoscopic endonasal skull base surgery (EESS) is widely performed for pituitary tumors. In growth hormone-secreting adenomas (GHomas), the procedure is technically more demanding than for other pituitary neuroendocrine tumor (PitNET) subtypes due to restricted instrument maneuverability. Few studies have evaluated the influence of nasal cavity morphology on this difficulty. OBJECTIVE: To identify anatomical factors contributing to the technical challenges of EESS for GHomas by comparing nasal cavity structures between GHoma and non-GHoma PitNET patients. STUDY DESIGN: Retrospect…
