Varus tilt of the tibial plafond as a factor in chronic ligament instability of the ankle.

The authors performed a radiographic study of 136 patients with acute ligament sprains and 85 patients with chronic lateral ligament instability of the ankle. Varus angulation of the tibial plafond, varus angulation of the line passing both malleolar ends, and varus angulation of the medial malleolus were measured on the AP views, and dorsal angulation of the tibial plafond was measured on the lateral views. The varus angles of the tibial plafond, the line passing both malleolar ends, and the medial malleolus were larger as mean values in patients with chronic ligament instability than in the

Impingement syndrome of the ankle caused by a medial meniscoid lesion.

Meniscoid lesion of the ankle is a well-described condition involving the anterolateral aspect of the ankle joint. To our knowledge, there are no reports of this condition involving the medial aspect of the ankle in the literature. We present the case of a 27-year-old man with a chronic deltoid ligament rupture who had developed anteromedial impingement syndrome of the ankle. The patient was treated with an arthroscopic debridement of the lesion and experienced a complete recovery.

Syndesmotic ankle injuries in rodeo bull riders.

This report describes five male rodeo bull riders, aged 18 to 32 years, who sustained syndesmotic ankle injuries during competition. The mechanism of injury was identical in each case. As the rider attempted to escape after being thrown, the bull stepped on his lateral ankle, resulting in forced external rotation. All patients delayed seeking medical care, the interval between the injury and presentation ranging from 2 days to 4 weeks. All five patients sustained significant syndesmotic tears, two with associated fibular fractures. Three patients underwent surgical stabilization, and all rider

Bone bruises detected by magnetic resonance imaging following lateral ankle sprains.

Although bone bruises have been well described in the knee joint, little is known about their presence in the ankle joint. The present study attempted to document the association of bone bruises with lateral ankle sprains. Magnetic resonance (MR) images were obtained from 60 consecutive patients with lateral ankle sprains between April 1994 and June 1995. There were 29 men and 31 women, aged on average 25 years (range 12-68 years). All of the patients presented within 3 weeks of the sprain. MRI examinations were done within 3 weeks of the injury in 15, after 3-6 weeks in 21, and after 6-8 week

Injuries in military parachuting: a prospective study of 4499 jumps.

In this prospective study, the parachuting injuries which occurred during 2031 jumps in basic courses of free fall were compared with the injuries occurring during 2468 jumps for reserve paratroopers on training exercises. Fifty-eight injuries were recorded in 51 paratroopers. The ankle was most commonly affected, and 80 per cent of the injuries involved the lower extremity. Only 14 per cent of the injured troopers suffered severe injuries (fractures, knee ligament ruptures). The injury rate for paratroopers on basic courses (19.7 injuries per 1000 jumps) was significantly higher (P < 0.0001)

The effect of an ankle orthosis on ankle range of motion and performance.

Ankle joint orthoses are used for rehabilitation and/or prevention of ankle sprains. The purpose of this study was to determine the effect of the Malleoloc ankle joint orthosis on active and passive range of motion reduction and on a jumping and a figure-eight running test. Twelve subjects with a history of inversion ankle sprain and documented increased anterior translation in a drawer test participated in the study. Active and passive range of motion for inversion was determined with and without the orthosis and pre- and post-exercise. Additionally, performance tests for figure-eight running

Electromyographic response of peroneal muscles in surgical and nonsurgical injured ankles during sudden inversion.

Because the peroneal muscles are thought to act as a safeguard against lateral ankle injury, it is important to study their function. This study was undertaken to electromyographically assess the latency between sudden unexpected ankle inversion and the start of peroneal muscle activity. Three groups participated in the study, including subjects with lateral ankle sprain rehabilitated nonsurgically (N = 7), sprains rehabilitated following surgery (N = 6), and injury-free ankles (N = 11). Peroneal latency was assessed by dropping the subjects into inversion from a special platform. The platform

Arthrography, clinical examination, and stress radiograph in the diagnosis of acute injury to the lateral ligaments of the ankle.

We examined 188 consecutive patients, each of whom had an acute ankle sprain, using clinical stability assessment, stress radiographs, and arthrography to compare the reliability of these methods used in diagnosing lateral ligament ruptures of the ankle. Sixty-six of the ankles were treated operatively. Arthrography accurately detected ligament damage in all of the patients with this injury and revealed the extent of the injury in about 85%, whereas clinical examination and stress radiographs detected only about half of the injuries. The injuries and the results of these two methods were often

[Treatment of acute ankle sprains. Comparison of the efficacy and tolerance of 2 indomethacin-gel preparations].

In a controlled randomized double-blind study, the effectiveness and tolerance of two gels containing the active substance indomethacin used in the treatment of acute sprains of the ankle were compared. Reduction in swelling and the evaluation of pain served as the criteria for the success of treatment. Over a treatment and observation period of two weeks, the parameters decreased rapidly and equally in both groups. At no time were statistically remarkable differences observed for any of the parameters. With respect to the main target parameter, the two preparations were seen to be of equal th

[The lower ankle joint. Anatomic and pathophysiologic observations].

The lower ankle joint is highly variable in ligamental guidance. It has to be considered in unity with the upper ankle joint. Biomechanical investigations have shown the posterior lower ankle joint to be relatively unstable primarily in supination position. Radiological techniques are available to procedure radiographic evidence to that clinical instability of the lower ankle joint. A surgical approach to instability of both upper and lower ankle joints is proposed in this paper and is discussed in some detail.

Lesion of the ligamentum bifurcatum in ankle sprain.

In a consecutive study of 106 patients with a history of ankle sprain, 40.5% showed clinical signs of damage to the ligamentum bifurcatum of the midfoot. A significantly larger percentage of these patients complained of maximum tenderness localized to the instep compared with the patients without signs of damage to the ligamentum bifurcatum (P less than 0.001). Radiologic examination revealed 20 lesions related to damage to the ligamentum bifurcatum: laxity in the transverse tarsal joint, 12; avulsion from the anterior process of the calcaneus, 5; avulsion from the cuboid, 2; avulsion from the

Inversion lateral ankle trauma: differential diagnosis, review of the literature, and prospective study.

This study is comprised of 71 patients with inversion injuries. The purpose of this article is to present a logical and orderly approach in evaluating these injuries. A comprehensive review of the literature concerning diagnostic techniques and mechanisms of injury related to inversion ankle trauma is presented. Inversion ankle injuries are frequently misdiagnosed as simple sprains when frequently there are other pathologies present. The standard approach of evaluating inversion ankle injuries is often inadequate.

Prediction of overexertion injuries using biomechanical and psychophysical models.

This paper summarizes a three year epidemiological study conducted in five large industrial plants in order to evaluate the validity of two alternative modeling approaches to overexertion injury prediction. Detailed biomechanical and psychophysical job evaluations were performed for 55 industrial jobs comprised of 2934 potentially stressful manual materials handling tasks. The medical experiences of 6912 incumbent workers were monitored retrospectively for two years and prospectively for one year to establish a data base for comparison of the different models. The results show that each of the

[Sacroiliac distorsion or subluxation--a medically established concept? X-ray diagnosis--bone scintigraphy].

50 patients with complaints in the region of the sacroiliacal joints and the hip, but negative radiographs following injury to the pelvis were investigated by bone scan. Scintigraphic diagnosis always showed involvement of the pelvis ring other than the apparent fractures. Together with the clinical symptoms scintigraphic findings are interpreted as sacroiliacal strain or subluxation. Additional injuries to the acetabulum and to the lumbosacral joints may be present. The patients' complaints are explained, the injuries are localized and documented.

Ankle instability: current concepts, diagnosis, and treatment.

Our approach to the problem of ankle sprains and instability is reviewed. After diagnostic evaluation (including stress roentgenography, arthrography, or tenography if necessary), most of our patients are treated either by taping if they have a grade 1 or 2 sprain or by casting if they have a grade 3 disruption. If the injury should progress to chronic instability despite this treatment regimen, which is an unusual occurrence, satisfactory reconstructive procedures are available. Indications for acute primary repair are limited.

Peroneal tendon dislocations: a review of the literature.

Acute anterior dislocation of the peroneal tendons from behind the peroneal groove is uncommon but not rare. The injury must be evaluated carefully to determine whether conservative treatment will suffice or surgical intervention is required. Choice of a surgical procedure depends on the fibular groove, the nature of the retinaculum, and the amount of damage to the area.

[Radiologic examination of the knee and ankle in stress positions (author's transl)].

The serious serious sequelae of sprains of the knee and ankle makes it imperative for the therapist to have an early and precise diagnosis. The authors propose a means of exploring these joints with the aid of a simple set up. This new technology allows the radiologist, secure from direct or indirect radiation, to carry out a standardized examination of the joint, usually unilaterally, and to make his interpretation more easily and more precisely. This method of x-raying in the stress position has its major indication in emergencies, but is equally useful in non emergencies, accomplishing a ve
