Alirocumab Treatment and Achievement of Non-High-Density Lipoprotein Cholesterol and Apolipoprotein B Goals in Patients With Hypercholesterolemia: Pooled Results From 10 Phase 3 ODYSSEY Trials.

BACKGROUND: Non-high-density lipoprotein cholesterol (non-HDL-C) and apolipoprotein (apo) B are better predictors of atherosclerotic cardiovascular disease risk than low-density lipoprotein cholesterol alone. US and European lipid management guidelines support non-HDL-C and apoB as targets for lipid-lowering therapy. METHODS AND RESULTS: This analysis evaluated the efficacy of alirocumab, a proprotein convertase subtilisin/kexin type 9 inhibitor, on non-HDL-C and apoB. Data were derived from 4983 patients enrolled in 10 randomized, placebo- or ezetimibe-controlled Phase 3 ODYSSEY trials. Prim

Increased visfatin levels are associated with higher disease activity in anti-Jo-1-positive myositis patients.

BACKGROUND: Treatment with proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors reduces low-density lipoprotein cholesterol (LDL-C) levels and decreases the incidence of major ischaemic events in clinical trials. However, less is known about the efficacy of PCSK9 inhibition in clinical practice. This study aimed to describe the change in LDL-C levels over time and LDL-C goal achievement in patients with/without atherosclerotic cardiovascular disease (ASCVD), who were prescribed evolocumab in clinical practice, and to describe adherence to and persistence with treatment. METHODS: P

Differential Expression of PCSK9 Modulates Infection, Inflammation, and Coagulation in a Murine Model of Sepsis.

INTRODUCTION: Proprotein convertase subtilisin/kexin type 9 (PCSK9) targets lipoprotein receptors for degradation, thereby reducing hepatic lipid clearance. PCSK9 inhibition reduces mortality in septic mice, presumably through increased hepatic clearance of pathogen lipids due to increased lipoprotein receptor concentrations. However, PCSK9 overexpression in vivo has not been studied in sepsis. Therefore, this study aimed to evaluate the effects of differential PCSK9 expression on systemic infection, inflammation, and coagulation in sepsis. METHODS: Wild-type, PCSK9 knockout (KO), and transge

Efficacy and safety of evolocumab in reducing lipids and cardiovascular events.

BACKGROUND: Evolocumab, a monoclonal antibody that inhibits proprotein convertase subtilisin-kexin type 9 (PCSK9), significantly reduced low-density lipoprotein (LDL) cholesterol levels in short-term studies. We conducted two extension studies to obtain longer-term data. METHODS: In two open-label, randomized trials, we enrolled 4465 patients who had completed 1 of 12 phase 2 or 3 studies ("parent trials") of evolocumab. Regardless of study-group assignments in the parent trials, eligible patients were randomly assigned in a 2:1 ratio to receive either evolocumab (140 mg every 2 weeks or 420

In vitro processing of anthrax toxin protective antigen by recombinant PC1 (SPC3) and bovine intermediate lobe secretory vesicle membranes.

Protective antigen (PA), an 83-kDa protein produced by Bacillus anthracis, requires proteolytic activation at a tetrabasic site (RKKR167) before it can combine with either edema factor or lethal factor on the cell surface. The complex is then endocytosed and the target cell intoxicated. Previous work has demonstrated that furin, a ubiquitously distributed, subtilisin-like protease, can perform this cleavage. In this study, another member of the furin family, PC1 (SPC3), was tested as a putative processing enzyme for PA. Recombinant PC1, partially purified from the medium of stably transfected

Involvement of protein disulfide isomerase in subtilase cytotoxin-induced cell death in HeLa cells.

Subtilase cytotoxin (SubAB) is a member of bacterial AB5 toxin produced by certain enterohemorrhagic E. coli strains which cleaves host chaperone BiP in endoplasmic reticulum (ER), leading to ER stress-mediated cytotoxicity. Previous study suggested that protein disulfide isomerase (PDI), an enzyme which catalyzes the formation and breakage of disulfide bonds in proteins, regulates AB5 toxin such as cholera toxin by unfolding of A subunit, leading to its translocation into cytosol to induce disease. Although SubAB targets ER and has similar A subunit to that of other AB5 toxins, it is unclear

Alteration in the processing of the ACRBP/sp32 protein and sperm head/acrosome malformations in proprotein convertase 4 (PCSK4) null mice.

Proprotein convertase 4 (PCSK4) is a member of a family of proprotein convertases that convert inactive precursor proteins into their mature and active forms. PCSK4 is expressed by testicular germ cells and localizes to the sperm acrosome, suggesting roles in fertilization. Mice lacking PCSK4 exhibit a profound fertility defect; yet, to date, few substrates for PCSK4 are known. In this study, two-dimensional differential in-gel electrophoresis analysis was carried out in order to identify proteins that are altered in spermatozoa from PCSK4 null mice. Herein, we report that the sperm fertilizat

Maturation of the trans-Golgi network protease furin: compartmentalization of propeptide removal, substrate cleavage, and COOH-terminal truncation.

We have cloned a bovine cDNA encoding the trans-Golgi network (TGN) protease furin and expressed it via recombinant vaccinia viruses to investigate intracellular maturation. Pulse-chase labeling reveals that the 104-kD pro-furin bearing high mannose N-glycans is rapidly processed into the 98-kD protease whose N-glycans remain sensitive to endoglycosidase H for a certain period of time. Furthermore, in the presence of brefeldin A, pro-furin cleavage occurs. From these data we conclude that the ER is the compartment of propeptide removal. Studies employing the ionophore A23187 and DTT show that

INTRODUCTION: As one of the most common chronic liver diseases worldwide, metabolic dysfunction-associated steatotic liver disease (MASLD) imposes an escalating burden on global health systems. However, the efficacy of Guizhi Fuling decoction (GZFLD) against MASLD and its potential pharmacodynamic material basis remain elusive. OBJECTIVES: This work aims to explore the potential bioactive constituents of GZFLD against MASLD by integrating a multitarget affinity ultrafiltration strategy with UPLC-Q-TOF-MS/MS. METHODS: The efficacy of GZFLD against MASLD was initially evaluated using an in vit

Real-world efficacy of inclisiran: data from a tertiary lipid clinic.

Small-interfering RNA (siRNA)-based therapies, such as inclisiran, offer a novel approach to reducing low-density lipoprotein-cholesterol (LDL-C) and preventing cardiovascular disease (CVD). Inclisiran inhibits proprotein convertase subtilisin/kexin type 9 (PCSK9) synthesis, enhancing LDL-receptor recycling and LDL-particle clearance. Although clinical trials have established its efficacy with LDL-C reductions of 44-52%, real-world studies have reported variable reductions over shorter follow-up periods. This study aimed to assess the long-term efficacy and safety of inclisiran in a diverse re

Inhibition of PCSK9 protects against myocardial ischemia-reperfusion injury by suppressing ferroptosis via negative regulation of the P65/MMP9 pathway.

Ferroptosis is a critical driver of cardiomyocyte death during myocardial ischemia-reperfusion (I/R) injury. Proprotein convertase subtilisin/kexin type 9 (PCSK9) is well-established as a key regulator of cholesterol metabolism; however, its role in modulating cardiomyocyte death, particularly ferroptosis, in the context of I/R injury remains poorly defined. Plasma PCSK9 level was determined by ELISA in patients with acute myocardial infarction 12 h after PCI. Left anterior descending coronary artery ligation was established to induce myocardial I/R injury in male SD rats. Alirocumab or vehicl

Sex Differences in Coronary Atheroma Regression After Intensive Lipid-Lowering Therapy: A Pooled Analysis.

BACKGROUND: Intensive lipid-lowering therapy with high-intensity statins, with or without proprotein convertase subtilisin/kexin type 9 inhibitors, reduces coronary plaque burden. However, whether men and women experience similar atheroma regression and plaque compositional changes remains uncertain. OBJECTIVES: The objective of the study was to quantify sex-specific changes in percent atheroma volume (PAV), total atheroma volume (TAV), and plaque composition after intensive lipid-lowering, using pooled individual-participant data from four serial intravascular ultrasound trials. METHODS: Da

Reversal of tumour immune evasion via enhanced MHC-Class-I antigen presentation by a dual-functional RNA regulated system.

BACKGROUND: Motivating the immune system to target tumour cells plays an increasingly prominent role in the treatment of hepatocellular carcinoma (HCC), but challenges such as low overall response rates persist in current clinical practice. Tumour cell MHC-Class-I (MHC-I) downregulation and antigen loss are typical mechanisms of immune evasion. To this end, a dual-functional RNA-based strategy was conceived for HCC immunotherapy. METHODS: MHC-I expression on HCC and paratumour tissues from patients was assessed, and the correlations between MHC-I regulators and HCC prognosis were analyzed. Sm

Silencing FAF2 mitigates alcohol-induced hepatic steatosis by modulating lipolysis and PCSK9 pathway.

BACKGROUND: Chronic alcohol consumption leads to lipid accumulation, oxidative stress, cellular damage, and inflammation in the liver, collectively referred to as alcohol-associated liver disease (ALD). FAF2/UBXD8/ETEA (Fas-associated factor 2) is a ubiquitin ligase adaptor protein that plays a crucial role in the ubiquitin-mediated degradation of misfolded proteins in the endoplasmic reticulum. A recent genome-wide association study indicated an association between FAF2 and ALD; however, the exact contribution of FAF2 to ALD pathogenesis remains unclear. METHODS: FAF2 was knocked down using

Evaluating the Cardiometabolic Efficacy and Safety of Lipoprotein Lipase Pathway Targets in Combination With Approved Lipid-Lowering Targets: A Drug Target Mendelian Randomization Study.

BACKGROUND: Therapies targeting the LPL (lipoprotein lipase) pathway are under development for cardiometabolic disease. Insights into their efficacy-both alone and in combination with existing lipid-lowering therapies-modes of action, and safety of these agents are essential to inform clinical development. Using Mendelian randomization, we aimed to (1) evaluate efficacy, (2) explore shared mechanisms, (3) assess additive effects with approved lipid-lowering drugs, and (4) identify secondary indications and potential adverse effects. METHODS: We selected triglyceride-lowering genetic variants
