PURPOSE: No consensus has been reached regarding the best treatment for repair of fractures of the severely atrophic mandible (<10 mm vertical height). The most complete Cochrane review of the published data could not substantiate a single best practice recommendation for treating these fractures. The present study reports the results of such treatment by placing miniplates at the inferior border of the mandible of 23 fractures of the mandibular body in 16 patients.
MATERIALS AND METHODS: This was a consecutive case series during a 30-year period performed by a single surgeon at several pract

We evaluated the results following the use of the Volkov-Oganesyan-Povarov hinged distraction apparatus to correct equinocavovarus deformity of the foot in sixty-five adults (eighty-three feet). The deformity was secondary to ischemic and neuropathic changes after trauma to the foot and ankle, poliomyelitis, Charcot-Marie-Tooth disease, or untreated clubfoot. The deformity often was associated with severe neurotrophic changes, extensive scarring of the skin, callosities, or osteomyelitis of the foot, all of which usually preclude operative reconstruction. The average age of the patients was tw

PURPOSE: Tibial tubercle osteotomy (TTO) is often employed for certain patellofemoral instability (PFI) cases, though its indications and effectiveness are not widely accepted. This systematic review gathers recent studies comparing isolated medial patellofemoral ligament reconstruction (iMPFLR) to MPFLR combined with TTO in managing PFI and to offer recommendations for clinicians when selecting TTO. This review proposes that MPFLR combined with TTO is superior to iMPFLR and that the combined procedure does not increase the incidence of postoperative complications.

Prosthodontic rehabilitation of a patient with an atrophic edentulous mandible presents a significant challenge in restoring esthetics and function. The purpose of this clinical report is to describe fracture of an atrophic edentulous mandible opposing maxillary natural dentition in association with endosseous dental implants. The patient received two wide-diameter implants in the anterior mandible for an implant-assisted mandibular overdenture, in which the implants penetrated the inferior border of the mandible for bicortical stabilization. Three months following implant placement surgery, t

OBJECT: The treatment of vertebral osteomyelitis includes antibiotics with or without surgical intervention. The decision to place instrumentation into an infected spinal column remains controversial. The use of recombinant human bone morphogenetic protein-2 (rhBMP-2) in patients with osteomyelitis is also extremely controversial. The authors review their experience in performing corpectomy and fusion with titanium cages and rhBMP-2 in patients with vertebral instability and/or neurological compromise due to vertebral osteomyelitis.
METHODS: Data obtained in 15 patients treated between 2001 a

BACKGROUND: The Coalition (Globus Medical) hybrid PEEK-titanium cage has been widely utilized in our practice for Anterior Cervical Discectomy and Fusion (ACDF) over a 5-year period. A preliminary review of all the cases that we performed in these five years show to us an unexpectedly high rate of postoperative cage subsidence with the Coalition (Globus Medical) stand-alone cage. This prompted a formal investigation, the purpose of this study was, therefore, to systematically determine the true rate of subsidence for the Coalition cage in our cohort and to identify the independent risk factors

INTRODUCTION: Variable-angle locking systems offer increased surgical flexibility by permitting off-axis screw insertion, with some designs allowing angulation up to 15°. However, clinical challenges such as screw loosening and insufficient mechanical stability data persist. This study aims to evaluate the impact of different plate hole design parameters on the locking strength of plate-screw head interface under various screw insertion angles, providing insights to optimize VALS performance in fracture fixation.
METHODS: Variable-angle locking system with Point-Loading Thread-In mechanism wa

BACKGROUND: Clavicular fracture or sternoclavicular luxation is observed in 10% of all polytrauma patients and is frequently associated with concomitant intrathoracic life-threatening injuries. Posterior sternoclavicular luxation is well known to induce underlying great vessels damage. The gold standard treatment usually is a combined orthopedic and cardiovascular surgical procedure associating vascular repair, clavicular open reduction, and internal fixation.
METHODS: A 59-year-old wheelchair ridden, institutionalized woman, known for psychiatric disorder, severe scoliosis, malnutrition, and

INTRODUCTION: In 2009-2010, military physicians hypothesized that a new pattern of spinal injury had emerged, resulting from improvised explosive device assault on up-armored vehicles, associated with a high rate of point of first contact fracture and neurological injury-the combat burst fracture. We sought to determine the incidence of all thoracolumbar (TL) burst fractures and combat burst fractures in 2009-2010 as compared to two antecedent years.
METHODS: A screening process identified all individuals who sustained TL burst fractures in the time-period studied. Demographics, injury-specif

BACKGROUND: Displaced lateral malleolar fractures are often treated with reduction and surgical stabilization. However, there has not been a comprehensive laboratory comparison to determine the most appropriate device for treating these patients. This study subjected a range of contemporary lateral fibular plates to a series of mechanical tests designed to reveal performance differences.
METHODS: Forty fresh frozen lower extremities were divided into 4 groups. A Weber B distal fibula fracture was simulated with an osteotomy and stabilized using 1 of 4 plate systems: a standard Synthes one-thi

INTRODUCTION: Unstable intra articular fractures of distal radius are frequently being managed with open reduction and internal fixation. Volar locking plate based on polyetheretherketon (PEEK) polymer has developed lately as an alternative to conventional metallic devices. The advantage of this kind of plates include the lack of metal allergies, radiolucency, low artefacts on MRI or imaging scans and the high resistance associated to loading forces. The aim of this study was to evaluate the clinical and radiological results using a new Carbon-PEEK volar locking plate compared with titanium vo

BACKGROUND: Transforaminal lumbar interbody fusion (TLIF) augmented with pedicle screw instrumentation has become a favored surgical treatment for management of lumbar degenerative disease and spondylolisthesis. Iliac crest bone graft has traditionally been used for interbody arthrodesis. Many substitutes for autograft have been suggested to reduce iliac crest graft morbidity. The present study was aimed to determine fusion rate and clinical outcome with local bone autograft drill shavings harvested during minimally invasive (MI) TLIF.
