From hidroacanthoma simplex to poroid hidradenoma: clinicopathologic and immunohistochemic study of poroid neoplasms and reappraisal of their histogenesis.

BACKGROUND: Poroid neoplasms comprise classic poroma (P), hidroacanthoma simplex (HS), dermal duct tumor (DDT), and poroid hidradenoma (PH). The 3 latter are rarely reported. Poroid cells in P have recently been identified as keratinocytes of the lowermost acrosyringium and the sweat duct ridge. OBJECTIVES: To investigate a large cohort of poroid neoplasms to better define the clinical and pathologic aspects of HS, DDT, and PH. To analyze the expression of discriminatory keratins in all 4 poroid neoplasms. METHODS: 202 P, 11 HS, 17 DDT, 31 PH, and 5 composite tumors were examined under light…

Volume repletion after exercise-induced volume depletion in humans: replacement of water and sodium losses.

Sodium and water loss during, and replacement after, exercise-induced volume depletion was investigated in six volunteers volume depleted by 1.89 +/- 0.17% (SD) of body mass by intermittent exercise in a warm, humid environment. Subjects exercised in a large, open plastic bag, allowing collection of all sweat secreted during exercise. For over 60 min beginning 40 min after the end of exercise, subjects ingested drinks containing 0, 25, 50, or 100 mmol/l sodium (trials 0, 25, 50, and 100) in a volume (ml) equivalent to 150% of the mass lost (g) by volume depletion. Body mass loss and sweat elec…

Effect of endoscopic thoracic sympathetic block on plantar hyperhidrosis.

HYPOTHESIS: Endoscopic thoracic sympathetic block at T4 (ESB4) provides excellent results in patients with primary hyperhidrosis (HH) of the upper limb. Most patients have combined palmo-plantar or palmo-axillary-plantar HH. This study evaluates the clinical outcome of patients with upper limb HH with special emphasis on plantar sweating and patients' quality of life. DESIGN: Review of a prospectively gathered database. SETTING: Tertiary care university teaching hospital. PATIENTS: The cohort included 73 patients (50 women and 23 men; mean age, 30.2 years). Twenty-six patients had palmar; 3…

20-Year Efficacy of Endoscopic Thoracic Sympathectomy for Primary Hyperhidrosis: A Cohort Study.

Background: Primary focal hyperhidrosis, characterized by excessive sweating primarily affecting the hands and axillae, can significantly impact quality of life. Bilateral thoracic sympathectomy is the gold standard treatment, providing permanent resolution of palmar hyperhidrosis. Most studies evaluating outcomes and patient satisfaction after sympathectomy focus on short- to medium-term follow-up, typically up to 5 years. This study aimed to assess anxiety, satisfaction, and sweat redistribution 20 years after bilateral endoscopic thoracic sympathectomy. Methods: Between January 2002 and Dec…

Incidence, severity, and impact of hyperhidrosis in people with lower-limb amputation.

To assess the incidence and severity of self-reported hyperhidrosis in patients with amputation and understand its effects on prosthetic fit or function, a cross-sectional survey of patients at two amputee clinics was performed. Responses from 121 subjects with lower-limb amputation were analyzed. Of these subjects, 66% reported sweating to a degree that it interfered with daily activities, as measured by the Hyperhidrosis Disease Severity Scale. There was a significant association between sweating and interference with prosthetic fit and function. Sweating was more severe in cases of transtib…

Immunohistochemical differentiation of four benign eccrine tumors.

BACKGROUND: The histogenesis and differentiation of eccrine tumors, including cylindroma, poroma, spiradenoma and syringoma, remains controversial. This controversy may be because of sporadic and incomplete studies of these neoplasms. METHODS: Ten examples each of normal eccrine structures and of four benign eccrine tumors are analyzed with antibodies to cytokeratin (CK) 7, CD34, CK6, CK10, smooth muscle actin (SMA) and CD10. These markers represent two different immunohistochemical stains for each part of the eccrine structure; CK7 and CD34 stain the secretory coil, CK6 and CK10 stain the st

It is necessary to confirm whether or not sweat ducts or sweat glands express Dsgs, because sera from PV patients exhibit high background in the dermis.

[Cystic fibrosis revealed by dehydration with hypochloronatremic alkalosis in 3 infants and a neonate].

BACKGROUND--Acute dehydration with hypochloronatremic metabolic alkalosis is a classical complication of cystic fibrosis of the pancreas. Its progressive development as a revealing manifestation of the disease is rare as is its appearance in newborns. Case n. 1.--A 13 month-old girl was admitted because of status epilepticus. She was severely dehydrated (20% weight loss) but had normal diuresis. Investigations showed metabolic alkalosis, hypochloronatremia and hypokalemia. All the manifestations disappeared within 5 days with treatment but three sweat tests were abnormal. Case n. 2.--A 7 month…

Comparison of physiological and subjective strains of two protective coveralls in two short physically simulated demanding tasks.

A total of 15 physically active male students carried out a normalised task (T(N)) and a task of access by ladder and scaffolding (T(L)) in two impermeable protective coveralls and a reference sports wear to compare the physiological and subjective strains. Heart rate (HR) was recorded and sweat loss was checked. Subjective evaluations of comfort parameters, acceptable exposure durations and physical exhaustion were recorded at the end of the tasks. Results show that both protective clothing, compared to the sport wear, increase significantly HR, RPE and CR10 in both tasks. However the strains…

An update on cystic fibrosis and implications for anesthesia.

Cystic fibrosis is a recessive inherited disorder that affects about 30,000 Americans. The majority of infants born with cystic fibrosis now reach adulthood, making surgical procedures performed in this patient group more common. The disease is a heterogeneous disorder caused by widespread dysfunction of exocrine glands and involving salivary, sweat, digestive, and pulmonary secretions. The broad, variable, and sometimes confusing array of clinical manifestations of cystic fibrosis include obstructive pulmonary disease, pancreatic insufficiency, abnormally high sweat electrolyte concentrations…

Chronic idiopathic anhidrosis.

We describe the cases of eight patients with chronic idiopathic anhidrosis. These patients were heat intolerant and became hot, flushed, dizzy, dyspneic, and weak but did not sweat when the ambient temperature was high or when they exercised. Four patients had preganglionic sudomotor lesions and in the remaining 4 the lesion appeared to be postganglionic. The patients did not have orthostatic hypotension, other evidence of generalized autonomic failure, or symptomatic somatic neuropathy. One patient regained thermoregulatory sweat function and no patient's condition progressed to generalized a…

Obstructive azoospermia as a diagnostic criterion for the cystic fibrosis syndrome.

In some male patients with findings characteristic of cystic fibrosis, but normal sweat chloride concentrations, the demonstration of obstruction azoospermia has been pivotal in diagnosis. It is proposed that cystic fibrosis be diagnosed if the patient has at least two of three major criteria (marked rise in sweat chloride concentration, chronic obstructive pulmonary disease and pseudomonas infection, and unexplained obstructive azoospermia) or has one major criterion together with one of several minor criteria--including positive family history and childhood onset of exocrine pancreatic insuf…

Studies on the mechanism of topical anhidrosis due to polyvalent cations.

Synopsis Although eccrine sweat glands are present in the foot pads of many mammals, and are abundant in the general body skin of many of the higher primates, it is only in man that these glands have a thermoregulatory function and are cosmetically significant. The expansion in sales of various antiperspirant products containing complexes of aluminium and zirconium has resulted in many studies, whose aim is to elucidate the mode of action of these substances. Sweat production in the rat foot pad in vivo has been studied, and its modification following topical application of various polyvalent …

Hyperhidrosis: what is it and why does it occur?

Hyperhidrosis is excessive sweating in response to heat or emotional stimuli beyond physiologic need. The ailment is not new and has been described in the literature dating back several centuries. It can be classified as either primary or secondary based on its etiology. Mechanisms that cause excessive sweating can be traced to the sympathetic nervous system, part of the autonomic nervous system. It has been speculated that the primary abnormality is central, and that the hypothalamic sweat center that controls the palms, axillae, and soles is distinct in hyperhidrosis individuals.

Thoracic sympathectomy for palmar hyperhidrosis: report of 457 cases.

A thoracic sympathetic ganglionectomy was performed on 457 patients with palmar hyperhidrosis, resulting in a complete cessation of the excessive palmar sweating but not a total abolition of the sweating of the upper extremity, forehead, face, neck, upper chest and upper back. This procedure does not produce a Horner's syndrome. The present data provide evidence that sympathetic pathways for controlling sweating of the above-mentioned skin areas come mainly through the second thoracic ganglion, and therefore from the spinal cord segment below T1 in man.
