[Subcutaneous rupture of the long thumb extensor tendon. Etiology, clinical aspects and therapy].

Subcutaneous ruptures of the extensor pollicis longus tendon are mostly due to an earlier wrist trauma and rarely caused by a degenerative systemic disease. The tendon usually ruptures close to the distal edge of the retinaculum extensorum where it is vulnerable in various respects and in addition exposed to increased mechanical strain. Functional loss of the extensor pollicis longus greatly impairs the function of the entire hand and requires surgical therapy. Transfer of the extensor indicis tendon has proven successful in 95% and thus appears to be the most appropriate therapeutic procedure

Traumatic injuries: imaging of peripheral musculoskeletal injuries.

The current dominant role of conventional radiography must be reassessed at increasingly shorter intervals in view of the continuing emergen … n of the body. Sonography is beginning to play an increasingly important role in trauma. Muscle and tendon injuries are the most common indications, but worthwhile information can be gained of the shoulder, elbow, hip, and knee joints, supplementing conventional or digital radiography. Magnetic resonance imaging effectively visualizes traumatic changes of the ske …

Imaging of triathlon injuries.

Injuries in triathletes are common and are mostly overuse injuries. Rotator cuff tendinitis is the most common complaint from swimming, but the incidence of tendinopathy and rotator cuff tears on magnetic resonance imaging is comparable in triathletes without and with shoulder pain. Cycling injuries are mainly to the knee, including patellar tendinosis, iliotibial band syndrome, and patellofemoral stress syndrome, and to the Achilles tendon and the cervical and lumbar spine. Running is associated with most injuries in triathletes, during both training and racing, causing the athlete to discont

The Evaluation of Equine Allogeneic Tenogenic Primed Mesenchymal Stem Cells in a Surgically Induced Superficial Digital Flexor Tendon Lesion Model.

Background: Tendon injuries are very common in horses and jeopardize the athletic performance, and due to the high risk of reinjury may lead to early retirement. The use of mesenchymal stem cells for the treatment of equine tendon disease is widely investigated because of their regenerative potential. The objective of this study is to investigate the safety and efficacy of equine allogeneic tenogenic primed mesenchymal stem cells (tpMSCs) for the management of tendinitis in horses. Methods: A core lesion was surgically induced in the superficial digital flexor tendon of both forelimbs of eight…

Delaminated Patellar Tendon Rupture in a Pediatric Patient: A Case Report.

CASE: We present a 12-year-old boy with partial delamination of the patellar tendon in the coronal plane and bipolar avulsion fracture of the tibial tubercle and patella after a planting injury while skateboarding. Pediatric patellar tendon rupture with associated bipolar avulsion fractures is rare. Furthermore, to the best of our knowledge, a delamination injury pattern of the patellar tendon has not been described. CONCLUSION: This type of extensor mechanism injury has not been reported in the literature. Repair with Krackow sutures and suture-bone tunnel technique, with consideration of th

Distal biceps repair using the lacertus fibrosus as a local graft.

BACKGROUND: We report a case series of 15 patients who underwent a new surgical technique that uses a transfer of the lacertus fibrosus to a … had no lacertus harvest complications. CONCLUSIONS: When operative treatment is chosen for biceps tendon injuries and if the lacertus fibrosus is intact, transfer of the lacertus to augment repair of the distal biceps provides predictable outcomes without the potential complications associated with allograft or autograft tendon reconstruction.

Extensor Pollicis Longus Tendon Rupture Following a Cat Bite: A Case Report and Review of Literature.

This case report describes the first reported occurrence of extensor pollicis longus (EPL) tendon rupture caused by a cat bite. Animal bites, particularly from cats, can cause various complications, including damage to tendons and bones. In our case, a 43-year-old female suffered an EPL rupture in her dominant hand after being bitten by her cat. The patient underwent EPL reconstruction by means of extensor indicis proprius (EIP) using standard techniques, with a positive functional outcome reported. The case highlights the importance of considering the risk of tendon injuries following animal …

Independent evaluation of a recently described Achilles tendon repair technique.

BACKGROUND: We investigated a previously reported technique for the repair of acute Achilles tendon ruptures using the percutaneous Achillon suture system (Intega Life Sciences Corporation, Plainsboro, NJ). METHODS: Twenty-five patients with Achilles tendon ruptures were studied prospectively with a minimum of 12 months followup. A single 2- to 3-cm horizontal incision and the Achillon suture system were used. Early rehabilitation and an active range-of-motion brace were instituted. RESULTS: There were no wound problems, sural nerve injuries, or re-ruptures. All patients were able to return

Biologics in Achilles tendon healing and repair: a review.

Injuries of the Achilles tendon are relatively common with potentially devastating outcomes. Healing Achilles tendons form a fibrovascular scar resulting in a tendon which may be mechanically weaker than the native tendon. The resulting strength deficit causes a high risk for reinjury and other complications. Treatments using biologics aim to restore the normal properties of the native tendon and reduce the risk of rerupture and maximize tendon function. The purpose of this review was to summarize the current findings of various therapies using biologics in an attempt to improve the prognosis

Distal pectoralis major tears: sonographic characterization and potential diagnostic pitfalls.

OBJECTIVES: Distinction between musculotendinous tears of the pectoralis major and distal tendon avulsions is important, as the latter typically requires surgical repair. The objective of this study was to characterize the sonographic appearances of surgically proven distal tendon avulsion tears of the pectoralis major. METHODS: A retrospective search of the radiology database (2001-2011) revealed 22 cases of pectoralis major tears on sonography, of which 9 had surgical correlation. Sonograms were retrospectively characterized by 3 fellowship-trained musculoskeletal radiologists in consensus

The scientific basis for advances in flexor tendon surgery.

During the last 40 years, there has been an enormous amount of basic scientific research designed to improve our knowledge of the structure of tendons, the biomechanics of their action, their biologic response to injury and repair, the mechanical characteristics of various tendon suture methods, and the effect of postrepair motion stress on tendon strength and healing. These investigative efforts have given rise to improved methods of tendon repair and protocols for the early application of passive and active wrist and digital motion as a means to more rapidly increase the strength and gliding

Evaluating the effect of VersaWrap tendon protector on functional outcomes in operative tendon repairs.

BACKGROUND: Tendon repairs often result in adhesion formation which can cause persisting functional deficits. Close proximity of healing tissues increases friction during tendon excursion, often leading to tendon tethering postoperatively. Despite continued improvements in techniques for tendon repairs, there is currently no consensus on the most effective modality to reduce adhesion formation. The VersaWrap Tendon Protector is a bioresorbable hydrogel that is FDA-cleared for use in tendon repair by separating healing tendons from surrounding tissues and improving tendon gliding. We conducted

Direct Observation of a Single Peroneal Myotendinous Unit Anatomic Variant: A Case Report and Systematic Review.

CASE: A 29-year-old woman with acute peroneal tendon subluxation underwent superior retinacular repair. On exposure, a single peroneal myotendinous unit was encountered, as opposed to the usual presence of independent peroneal tendons arising from separate muscle bellies. At 3-year follow-up, she has had no recurrence with full return to activity and no limitations. CONCLUSION: Multiple peroneal myotendinous variants have been described; however, this report is the first to describe direct intraoperative observation of a single peroneal myotendinous unit. Whether this anatomic variant contrib

Distal biceps tendon ruptures occur with the almost extended elbow and supinated forearm - an online video analytic study.

BACKGROUND: Distal biceps tendon ruptures can lead to significant restrictions in affected patients. The mechanisms of injury described in scientific literature are based exclusively on case reports and theoretical models. This study aimed to determine the position of the upper extremities and forces involved in tendon rupture through analyzing video recordings. METHODS: The public YouTube.com database was queried for videos capturing a clear view of a distal biceps tendon rupture. Two orthopedic surgeons independently assessed the videos for the activity that led to the rupture, the arm posi

The Transosseous Internal Four Strand Technique: A New All-Inside Technique for Zone 1 Flexor Tendon Repairs.

BACKGROUND: Multiple techniques for the repair of flexor tendon injuries in zone 1 have been proposed over time. While pull-out suture techniques and bone anchor seem to be stronger than internal suture techniques, they are associated with a higher complication rate. We therefore developed an alternative internal suture repair with similar biomechanical stability to those of pull-out sutures and bone anchors. METHODS: Twenty porcine distal phalanges and deep flexor tendons were randomized to 2 groups of 10 each. The tendons were transsected at the level of the distal interphalangeal joint. In…

Mechanical properties of the different rotator cuff tendons in the rat are similarly and adversely affected by age.

Rotator cuff tendon tears and tendinopathies are common injuries affecting a large portion of the population and can result in pain and joint dysfunction. Incidence of rotator cuff tears significantly increases with advancing age, and up to 90% of these tears involve the supraspinatus. Previous literature has shown that aging can lead to inferior mechanics, altered composition, and changes in structural properties of the supraspinatus. However, there is little known about changes in supraspinatus mechanical properties in context of other rotator cuff tendons. Alterations in tendon mechanical p

The treatment of Doyle types II and III mallet finger with the encircling fixation of a transplanted palmaris longus tendon.

OBJECTIVE: This study aimed to investigate the clinical effect of the encircling fixation of a transplanted palmaris longus tendon in the treatment of Doyle types II and III mallet finger. METHODS: There were 115 cases of mallet finger deformity with Zone 1 extensor tendon rupture and defect. After debridement by first intention, the tendon bundles of the palmaris longus tendon were used to pass through the subcutaneous tunnel on the volar side of the base of distal phalanx, forming an encircling binding, crossing on the dorsal side. The tail of the tendon was then overlapped with the proxima
