The surgical handling of central cleft hands may involve various procedures to restore acceptable function. Stabilization of the merged cleft is usually achieved through reconstruction of the intermetacarpal ligament. However, no attention has been paid to the agenesis of the metacarpophalangeal collateral ligaments adjacent to the cleft, which is responsible for an excessive laxity of the joint in the frontal plane. We propose an autologous ligamentoplasty aimed at stabilizing the metacarpophalangeal joint while taking into account the specificities of the immature skeleton. A tendon graft is

End-stage degenerative joint disease of the ankle and concomitant ipsilateral Stage II posterior tibial tendon insufficient flat foot is a well known entity. Despite this, treatment options have not been discussed in the orthopaedic literature. A case series consisting of five patients was conducted to determine the efficacy of our treatment proposal. Our surgical treatment included an ankle fusion and concomitant flat foot reconstruction with a medializing calcaneal osteotomy, lengthening calcaneal osteotomy, and flexor digitorum longus transfer. At the final followup visit all patients were

Thumb pain secondary to arthritis at the basal joint of the thumb is a common condition, especially in women, and can be quite disabling. An accurate diagnosis can be readily made from the history and examination. Radiographs are used to stage the severity of the arthritis. Splinting is the mainstay of conservative care. Reconstructive procedures for each stage of the disease are aimed at restoring thumb motion and strength. Partial or complete trapeziectomy with tendon interposition and ligament reconstruction to stabilize the metacarpal base is used for advanced disease. Secondary metacarpop

We present an unusual case of polydactyly of the thumb. The patient, despite having a fully developed nail bed, had a duplication at the metacarpophalangeal level consisting of a single bony phalanx. The Wassel classification of polydactyly, which is the most commonly cited classification scheme, does not include this particular anomaly. In addition, there has been no reference to this type of polydactyly in the literature.

Three patients were treated for snapping scapholunate subluxation in four wrists, with disabling pain in three wrists. Conventional roentgenographic techniques were not helpful in diagnosing any of these wrist problems. No complete description of the precise abnormality encountered was found in the surgical or roentgenographic literature. Three of the involved wrists have had surgical correction with good early results.

The complex adult acquired flatfoot deformity involves progressive collapse of the foot with attenuation of medial soft tissues such as the … n, and progressive deformity. Some treatment options directly address the pathologic areas, such as tendon transfer for posterior tibialis tendon dysfunction and spring ligament reconstruction. Others such as calcaneal osteotomies secondarily counteract the primary ligamentous dysfunction and realign the foot to neutralize deforming forces. Selective fusions of t …

OBJECTIVE: To report a new approach to lower lip reanimation, which can be performed in the outpatient clinic setting.
STUDY DESIGN: Cross- … tients reported overall improvements in smiling, oral competence, and speech articulation following tendon transfer. Photographic grading by the judging panel found satisfactory smile symmetry postprocedure.
CONCLUSIONS: The palmaris longus tendon sling procedure provides an effective, additional option for reanimation of the paralyzed lower lip.

Upper limb spasticity is a frequent and disabling consequence of cervical spinal cord injury. In tetraplegic patients, spasticity may either support residual function or severely compromise surgical reconstruction, hygiene, comfort, and independence. Over the past decade, advances in neurophysiology, diagnostic tools, and surgical techniques have substantially modified the management of spastic upper limbs in tetraplegia. This review synthesizes recent advances with a particular focus on surgical decision-making, selective neurotomy, and combined tendon procedures, and highlights their impact

Surgical options may include release or lengthening of the spastic muscles, imbrication or tendon transfers to augment the weak extensors and abductors, joint procedures when needed and neurectomies in selected cases. Most patients improve with the surgical treatment and both patients and caregivers are satisfied with the outcomes. Alleviating the thumb- …

Acquired hallux varus deformity secondary to postburn contracture is a rare condition. It causes cosmetic disfigurement, pain, and inability to wear a shoe. Soft-tissue procedures and tendon transfers have been described for correction; however, these may require multiple operations and the outcome may be poor. We report a 6-year-old neglected case of hallux varus deformity secondary to burn contracture successfully managed by contracture release with pivotless distraction technique using a biplanar distractor and skin grafting with Z-plasty in the same procedure. The deformity was corrected t…

In this review article, the authors give an overview of the currently available soft tissue and bony procedures in the treatment of the adult acquired flexible flatfoot. Instead of starting from the classification for posterior tibial tendon dysfunction, described by Johnson and Storm, the authors address the flatfoot from a more anatomical point of view. Based on this, they will try to define a treatment algorithm.

The surgical treatment of flatfoot deformity has evolved during the past three decades. Soft tissue procedures alone fail to reestablish anatomic bony alignment, and bony procedures alone fail to provide dynamic support to the arch. The goal of any procedure is to reestablish the inherently stable bony configuration with adequate soft tissue balance (tendon transfer) to maintain stability in the dynamic situation. Therefore, a combination of procedures, such as soft tissue procedures combined with calcaneal displacement osteotomies and/or lateral column lengthening, may provide optimal results…

Toe-to-hand transfer is a well-established reconstructive option for certain congenital hand anomalies. It is the only technique which can add growth potential to the immature skeleton. Toe transfer is best suited for constriction ring amputations, which have relatively normal proximal anatomy. Transfers should be performed early in life to avoid lack of cortical integration of the new part. Anatomic variations of both hand and foot are often encountered, which influence both operative approach and functional prognosis. Indications, techniques, and complications are reviewed.

The authors report the medium term results (41 months) in 88 patients submitted to surgery for reconstruction of the anterior cruciate ligament according to the method currently in use at our Clinic which uses the semitendinosus and gracilis tendons. The results were evaluated according to a score card method and in 28 patients a KT-1000 arthrometer was also used. The method gives a probability of full success in approximately 85% of cases, with significantly better results in patients not submitted to meniscectomy. A comparison with other methods of autogenous transplant and prosthetic substi

Variable impairment occurs following birth associated brachial plexus injury. Muscle transpositions have evolved in response to patient need for functional reconstruction. Loss of external rotation limits the ability to perform activities with the arm in an overhead position. A modification of the combined latissimus dorsi and teres major transfer is described by means of which increased excursion of the transfer may be achieved.
