Five patients with adult onset acid maltase deficiency are described. All patients had developed their initial pelvic girdle symptoms late in the second or early in the third decade and some years later developed signs of respiratory insufficiency. Typically they were tall, had weak and wasted paraspinal and gluteal muscles with lower limb weakness. All were orthopnoeic with marked diaphragmatic weakness. In all patients characteristic glycogen vacuoles were seen in peripheral blood lymphocytes. Muscle biopsy was characteristic in only 1 out of 4 cases and showed minimal or nonspecific feature

Six cases of bilateral abductor palsy with stridor are presented. Following unilateral intralaryngeal arytenoidectomy, four had adequate airways and reasonable voices. Two required bilateral arytenoidectomy, one acquired an adequate airway at the expense of a weakened voice, and the other still requires a tracheostomy. There was only one minor complication. Decannulation and discharge from hospital were usually rapid.

The object of this paper is to analyse our experience at the Hospital for Sick Children, Great Ormond Street (London, U.K.) in the management of 10 patients with posterior laryngeal clefts during the past 5 years. Of these 10 patients, 4 had minor clefts not causing significant laryngeal incompetence. The other 6 patients had clefts extending below the cords requiring repair. Other congenital abnormalities were prevalent in this series, particularly oesophageal abnormalities. The frequency of this association is stressed, the problems of diagnosis and management discussed and recommendations m

One hundred thirty-six cases of subglottic hemangioma in infants have been reported in the English -language literature; four additional cases are reported here. The clinical presentations, diagnostic work-ups, treatments, and outcomes are analyzed. Twenty-one patients died, 17 of whom underwent autopsy. The histology of the laryngeal pathology from seven of these patients was reviewed by the authors. Possible correlation between the pathological findings and the clinical presentation is made. Approaches to diagnosis and treatment are suggested based on these findings.

Two acromegalic patients presented with the additional complaint of excessive daytime sleepiness. Subsequent sleep laboratory evaluation revealed that each patient had concurrent obstructive sleep apnea. In each patient, endoscopic examination of the upper airway revealed that on inspiration, the soft tissue of the posterior and lateral hypopharynx invaginated into the laryngeal vestibule before any posterior movement of the tongue. Neither enlargement of the tongue nor in movement in relation to respiration appeared to be the primary factors in the etiology of the upper airway obstruction. Af

123 patients suffering from carcinoma of the larynx were treated by radical radiotherapy at Queen Mary Hospital in Hong Kong between 1970 and 1977. The results are presented. 69(56.1%) were in the supraglottic, 53(43.1%) in the glottic, and 1(0.8%) in the subglottic regions. The incidence of cigarettes, heroin, and opium smoking, and that of alcohol consumption is analysed. The overall actuarial 5-year survival rate for supraglottic cancer was 47.7%, and 80.4% for glottic cancer. The solitary patient with subglottic cancer was alive 9 years after radical radiotherapy.

Pressure-flow relationships of an artificial airway-mechanical ventilator system were investigated. Endotracheal tubes varying from 7 to 9.5 mm. internal diameter and tracheostomy tubes from 7 to 10 mm. internal diameter were tested over flow-rates from 0.3 to 1.4 L. per second using a tidal volume of 500 ml. The effect of partial obstruction within these airways by suction catheters and the bronchofiberscope was examined. Depending on the internal diameter and the flow-rate, the driving pressure necessary to overcome the resistance of the tubes ranged from 5 to 20 cm. H2O. Partial obstruction

OBJECTIVE: The current report describes two rare cadaveric findings of a left sided brachiocephalic trunk (BCT) in relation to the trachea, … mpression.
CONCLUSION: A high-riding BCT is of paramount clinical importance, as it may complicate tracheotomy, thyroid surgery and mediastinoscopy, leading to fatal complications. BCT injury leads to a massive bleeding during neck dissection (level VI), when the vessel crosses the anterior tracheal wall.
