Traumatic shock: the fifth shock.

Although, historically, shock associated with traumatic injury has been evaluated through knowledge of the 4 recognized shock patterns--cardiogenic, obstructive, distributive, and hypovolemic--many trauma practitioners view traumatic shock as a unique fifth shock pattern. Although secondary to a systemic inflammatory response syndrome triggered by endogenous danger signals, traumatic shock represents a unique pathological condition that begins with multiple, usually blunt, trauma and may conclude with multiple organ dysfunction syndrome and death. While varying mechanisms of injury may lead to

A multidisciplinary motorcycle safety initiative.

Motorcycle registrations are on the rise in the United States, especially among riders over 40 years of age. While motorcycle popularity has been increasing, so have injuries and fatalities. Unfortunately, motorcycle crashes have been increasing at a disproportionately high rate. In 2007, motorcycle fatalities reached the highest level since the Department of Transportation began collecting data in 1975. Because of the increasing number of motorcycle crashes in Ohio and Central Ohio, a multidisciplinary team consisting of the Grant Medical Center's trauma program, Franklin County Safe Communit

Temporal artery scanning falls short as a secondary, noninvasive thermometry method for trauma patients.

Oral and mandibular trauma pose barriers to oral thermometry. We sought to determine whether temporal artery (TA) scanning thermometry could be an accurate, noninvasive back up method of thermometry in patients with these types of traumatic injury. We compared 3 techniques of TA scanning, axillary thermometry, and oral thermometry in critical care patients. Our results indicate that TA scanning methods were, at best, comparable with axillary measurements. In addition, the performance of the TA scanners varied with body mass index, whereas axillary readings did not.

Severe and penetrating traumatic brain injury in the context of war.

Our data suggests that traumatic brain injury (TBI) may account for up to one third of battle-related injuries in today's war. Although the majority of these injuries are classified as mild in severity, service members with severe or penetrating TBI can be faced with many challenges. Injuries sustained on the battlefield require a slightly different approach than the TBI care that is traditionally seen in a civilian setting. This article presents the range of care that occurs beginning on the battlefield and continuing to state-of-the-art rehabilitation within the Department of Defense and Vet

Indications for use of recombinant factor VIIa: a case study with implications for research.

The purpose of this case study was to explore the current use of recombinant factor VIIa to control hemorrhage in the trauma patient. Despite aggressive blood product replacement and surgical intervention, uncontrolled bleeding and resultant coagulopathy are lethal to the trauma patient. What is the correlation between this drug and the hemorrhagic trauma patient? Quite simply--does it work? Curiosity may have killed the cat, but this case study clearly demonstrates the need for further research and satisfactory answers to questions illuminated by this case.

Improved organ procurement through implementation of evidence-based practice.

Michigan, like most other states in the nation, has a clear need for more organ donors for transplantation; at this time, there are more than 2,800 patients in the state awaiting organs. We have evaluated the effects of a process improvement program designed to increase the number of organ donors and the number of organs donated from appropriate trauma patients. In 2005, William Beaumont Hospital began working with the Michigan Hospital Association Keystone Center and more than 40 hospitals across Michigan to implement evidence-based practices in organ donation focused on 4 specific outcomes a

Nurses' knowledge and attitudes toward organ and tissue donation in a community hospital.

The author conducted a survey of 210 nursing staff members in a community hospital to identify their attitudes and knowledge about organ and tissue donation. Results showed that 32% (n = 68) of respondents identified the significant difference between organ and tissue donors and 53% (n = 112) described feeling uncomfortable discussing donation with families. Fifty-seven percent (n = 120) of those who had been involved with donor families in the past were more likely to feel comfortable discussing donation with family members. Other means for increasing awareness and referral are discussed also

Feasibility of Using Electronic Medical Records to Identify Postdischarge Mental Health Screening Documentation in Trauma Patients.

BACKGROUND: Injury leads to long-term physical and mental health challenges, yet inpatient screening for post-traumatic stress disorder and depression remains inconsistent, and postdischarge mental health is poorly understood, particularly in less resourced trauma centers where emerging symptoms may go unrecognized. OBJECTIVE: To evaluate the feasibility of using the electronic medical record to identify postdischarge mental health screening documentation in adult trauma patients within 6 months after discharge. METHODS: This retrospective descriptive feasibility study was conducted at a mid

Accuracy of Prehospital Head Injury Assessment: Emergency Medical Dispatcher Versus Emergency Department.

BACKGROUND: In adult patients with head trauma, the location and characteristics of the visible head injury have been associated with adverse outcomes, such as traumatic intracranial hemorrhage and cervical spine injuries. OBJECTIVE: This study aimed to evaluate the reliability of head injury location and characteristics between Emergency Medical Dispatcher (EMD) and Emergency Department (ED) assessments in patients contacting an EMD for head injuries. A secondary objective was to assess how this reliability varied depending on the individual contacting the EMD. METHODS: This study was a pla
