Follmann balanitis - A very rare presentation of primary syphilis.

Syphilis is a sexually transmitted infection caused by Treponema pallidum. The primary stage of the disease manifests as a chancre. Balanoposthitis as a presenting feature of primary syphilis is a very rare presentation. A 26-year-old male presented with asymptomatic erythematous plaques involving the glans and prepuce after unprotected intercourse with a known female. Routine investigations, serology, and dark field examination were normal. Fontana-Masson stain revealed spirochetes and a diagnosis of syphilitic balanitis of Follmann was entertained. This rare presentation accounts for only 0.…

[Syphilis].

Syphilis is a sexually transmitted infection caused by the spirochaete Treponema pallidum. Syphilis re-emerged worldwide in the late 1990s, and hereafter increasing rates of syphilis were seen also in Denmark. Currently, around 700 cases are diagnosed yearly in Denmark, and syphilis is mainly encountered among men, who have sex with men. However, because of the risk of congenital infection screening of pregnant women has been introduced. Syphilis gives rise to highly variable symptoms such as chancre, skin rashes and fever or latent infection. The recommended treatment is intramuscular adminis…

[The many faces of neurosyphilis].

Syphilis is an infectious disease caused by Treponema pallidum. We describe two patients with chronic syphilis causing neurosyphilis. The first had had several brain infarctions due to the presence of meningovascular syphilis. Second patient suffered from motor and psychiatric symptoms caused by syphilis. The symptoms of our patients were alleviated by antibiotic therapy. Recognition of the multifaceted symptom picture of syphilis is increasingly important, because the occurrence of the disease has increased in our country over the last few decades. An early enough treatment can prevent perman…

Laryngeal syphilis: a case report.

Syphilis is a sexually transmitted disease caused by the spirochete Treponema pallidum. The complexity of the disease gained it the moniker "the great imitator"; it was William Osler who said, "He who knows syphilis, knows medicine." In 1866, Patrick Watson of Edinburgh, Scotland, reported a case of a 36-year-old man in whom syphilis destroyed the larynx.(1) The diagnosis was made postmortem. It was once believed that this was the first reported total laryngectomy, but the credit should actually be given to Christian Albert Theodor Billroth who performed this surgery on a patient with laryngea…

Syphilis--an appraisal of the Groote Schuur Hospital antenatal screening programme.

Syphilis is a major public health problem in pregnant women. Since congenital syphilis is preventable, an adequate screening programme is an essential facet of antenatal care. At Groote Schuur Hospital, Cape Town, the Venereal Disease Research Laboratory and Treponema, pallidum haemagglutination tests are employed for antenatal screening. A survey of the current literature on serological testing for syphilis does not reveal clear guidelines for an optimal antenatal screening programme. The high prevalence of syphilis in pregnant women is confirmed by this study, which shows the current program…

[Early syphilitic hepatitis. Two cases].

Two patients are presented as cases of secondary syphilitic hepatitis. Clinical and biological signs coincided with usual description of this affection. Syphilitic serological tests were all very positive. These two patients were homosexual men. In one case, we have found treponema in hepatic biopsy, underelectron microscope only, which confirmed the diagnosis. Evolution of both patients was improved by anti-syphilitic penicillin the rapy. Thus, this is an important diagnosis, which should not missed, because of the good prognosis of this affection with specific treatment, and because of the i…

Induction of acquired cellular resistance following transfer of thymus-dependent lymphocytes from syphilitic rabbits.

Syphilitic rabbits have previously been shown to resist challenge with Listeria monocytogenes. Thirty days after rabbits were infected with Treponema pallidum, transfer of 4 to 6 times 10-8 viable spleen cells along with T. pallidum conferred resistance to Listeria on normal recipients. Treatment of the spleen cells with anti-thymus serum and complement inhibited or abolished their ability to transfer resistance to Listeria. These results support the hypothesis that the ability of syphilitic rabbits to suppress the growth of Listeria reflects stimulation of cell-mediated immunity during active…

Erosive gastritis as a manifestation of secondary syphilis.

In a case of secondary syphilis with gastric involvement, the clinical history and roentgenographic and gastroscopic examiniations initially suggested an infiltrative malignant neoplasm. However, macroscopic and histologic examination of the partial gastrectomy specimen demonstrated an erosive, inflammatory lesion. Subsequently, numerous Treponema pallida were demonstrated in formalin-fixed eroded gastric mucosa by silver impregnation and fluorescent antibody technics. Although the true incidence of syphilitic gastritis is unknown, it may occur commonly during the secondary stage of syphilis a…

Progressive exacerbation of neurological symptoms in a patient with neurosyphilis following T9-T10 intradural space-occupying lesion resection: a case report.

BACKGROUND: Neurosyphilis is a disease of the central nervous system (CNS) caused by Treponema pallidum (syphilis spirochete), which usually presents with a variety of nonspecific symptoms and is easily misdiagnosed. Although Treponema pallidum can remain latent after treatment, reactivation may occur in the presence of a compromised or immunosuppressed immune system. This article reports a case of reactivation of neurosyphilis after spinal surgery, with the aim of exploring the possible role of immunosuppression in the process. CASE PRESENTATION: The patient was a 75-year-old female admitted…

Biophysical and biochemical evidence for the role of acetate kinases (AckAs) in an acetogenic pathway in pathogenic spirochetes.

Unraveling the metabolism of Treponema pallidum is a key component to understanding the pathogenesis of the human disease that it causes, syphilis. For decades, it was assumed that glucose was the sole carbon/energy source for this parasitic spirochete. But the lack of citric-acid-cycle enzymes suggested that alternative sources could be utilized, especially in microaerophilic host environments where glycolysis should not be robust. Recent bioinformatic, biophysical, and biochemical evidence supports the existence of an acetogenic energy-conservation pathway in T. pallidum and related treponem…

Neurosyphilis Diagnosed on the Basis of Pupillary Abnormalities: A Case Report.

Neurosyphilis is a condition in which Treponema pallidum invades the central nervous system and may occur at any stage of syphilis. Early diagnosis and treatment are crucial, as advanced disease can result in locomotor ataxia and progressive paralysis. We report a case of neurosyphilis identified on the basis of pupillary abnormalities. A man in his 20s was referred to our hospital with difficulty with near vision in the right eye, after bilateral pupillary dilation had been detected during an eye examination for contact lens prescription at another clinic. Brain MRI revealed no abnormalities,…

Clinical study showing a lower abundance of Neisseria in the oral microbiome aligns with low birth weight pregnancy outcomes.

OBJECTIVES: The objective of this study was to examine the association between the oral microbiome and pregnancy outcomes, specifically heal … ed by 16S rRNA sequencing. RESULTS: Women with PD showed an enrichment in the genus Porphyromonas, Treponema, and Filifactor, whereas women with healthy gingiva showed an enrichment in Streptococcus, Actinomyces, and Corynebacterium, independently of the birth status. Although no significant difference was found in the beta diversity between the 4 groups, …

Evaluation of immunoglobulin G enzyme immunoassay for serodiagnosis of yaws.

A commercially available enzyme immunoassay (EIA), the Captia Syphilis-G immunoglobulin G (IgG) EIA, for the detection of IgG antibodies to Treponema pallidum was evaluated for use as a screening test for yaws (Treponema pallidum subsp. pertenue). The IgG EIA was compared with the fluorescent treponemal antibody absorption (FTA-ABS) test. All sera were also examined by the T. pallidum hemagglutination test and the Venereal Disease Research Laboratory test. Serum samples from 271 subjects (23 control serum samples from an area nonendemic for yaws, 58 control serum samples from an area endemic f…

Ocular syphilis in antibiotic era.

Syphilis is a sexually transmitted multisystem disease caused by Treponema pallidum. Although easily treatable in early stages, if unrecognized, the disease usually progresses through its natural stages (i.e., primary, secondary, early latent, late latent, and tertiary) with devastating neurological, cardiovascular, and ocular sequelae. Syphilitic eye involvement may occur in any stage of the disease, affects mainly the posterior segment of the eye, and may cause blindness. Syphilitic eye involvement is especially severe in patients co-infected with human immunodeficiency virus. Ocular syphili…

Borrelia burgdorferi colonizes the mammary glands of lactating C3H mice: does not cause congenital Lyme disease.

Transplacental transmission of syphilis causing spirochete, Treponema pallidum subspecies pallidum, from mother to child results in congenital syphilis, an ever-expanding devastating disease worldwide. Although adverse effects of untreated gestational Lyme disease, caused by a related spirochete, Borrelia burgdorferi on fetus viability and development have been observed, cases of congenital Lyme disease are not reported. In this study, we show that B. burgdorferi colonizes mammary glands of C3H mice only postpartum; however, neither transmission of these spirochetes from dams-to-pups occurs no…

Liver Cholestasis Secondary to Syphilis in an Immunocompetent Patient.

Liver involvement is a known feature of secondary syphilis. The prevalence of hepatitis in secondary syphilis ranges broadly from 1 to 50%. We report a case of a 37-year-old man with type 1 diabetes mellitus and sickle cell trait presenting with jaundice and acute liver cholestasis. Abdominal ultrasound revealed mild hepatic fatty infiltration. RPR and Treponema pallidum IgG results were positive with a reflex titer of 1:64. Liver biopsy revealed chronic hepatitis with normal hepatic architecture, Kupffer cell hyperplasia, hepatic cholestasis, and ductal proliferation suggestive of syphilitic …

Syphilitic placoid chorioretinitis. A case-report.

CLINICAL CASE: We report the case of a 45-year-old woman, with unremarkable past medical history, who presented with acute visual loss in her left eye due to bilateral posterior uveitis. After the screening, she was diagnosed with acute syphilitic placoid chorioretinitis and was treated with intravenous penicillin. DISCUSSION: Clinical manifestations of ocular syphilis are extremely heterogeneous and may mimic several aetiologies. Anti-treponema treatment usually induces a quick and positive response in affected patients. Prompt and proper diagnosis of these patients is crucial, although anat…

Diffusion-weighted magnetic resonance imaging findings in a patient with cerebral syphilitic gumma.

Cerebral syphilitic gummas are rare entities, consisting of masses of granulation tissue that result from an exacerbated cell-mediated inflammatory response to Treponema pallidum, usually arising from the meninges of the convexity. Conventional magnetic resonance imaging and computed tomography findings of cerebral gummas have been reported, but diffusion-weighted imaging findings have not been previously described. In our patient, magnetic resonance imaging revealed a juxtacortical lesion with nodular enhancement, moderately restricted diffusion, a dural tail, and surrounding vasogenic edema.…
