The association of an increased urinary calcium-to-creatinine ratio, and asymptomatic gross and microscopic hematuria in children.

PURPOSE: The role of hypercalciuria for the evaluation of unexplained asymptomatic gross and microscopic hematuria in children remains uncle … management, a markedly increased urinary calcium-to-creatinine ratio and positive family history of urolithiasis. CONCLUSIONS: Although we cannot establish a direct cause and effect relationship, hypercalciuria is detected in a substantial number of cases of unexplained gross and microscopic hematuria. However, based on our study we do not recommend treatment …

In Vivo Radiographic Characteristics Associated With the Mineral Composition of Calcium Oxalate, Struvite, and Cystine Lower Urinary Tract Uroliths in Dogs.

BACKGROUND: Identifying urolith mineral composition (UMC) is crucial for treatment and prevention; however, current data rely on macroscopic or ex vivo studies. OBJECTIVES: To describe in vivo radiographic characteristics of canine lower urinary tract (LUT) uroliths and identify discriminating radiographic features among UMC types. ANIMALS: Two hundred and two dogs with LUT uroliths composed of ≥ 70% calcium oxalate (n = 109), struvite (n = 58), or cystine (n = 35). METHODS: Retrospective cross-sectional study. Radiographs were evaluated for urolith size, shape, surface, borders, internal a

Adenine phosphoribosyl transferase deficiency leads to renal allograft dysfunction in kidney transplant recipients: a systematic review.

BACKGROUND: Adenine phosphoribosyl transferase (APRT) deficiency has great implications on graft survival in kidney transplant patients. This systematic review investigated the diagnostic pattern, treatment approach, and kidney transplant outcomes among kidney transplant patients with adenine phosphoribosyl transferase deficiency. MATERIAL AND METHODS: Articles reporting the APRT enzyme deficiency and kidney allograft dysfunction were retrieved from PubMed/Medline, ScienceDirect, Cochrane library and Google scholar databases. Descriptive analysis was used to draw inferences. RESULTS: The res

Current Status and Role of Patient-reported Outcome Measures (PROMs) in Endourology.

Kidney stone disease can have a number of adverse effects on patients including quality of life. Studies assessing outcome measures in this area have largely been focused on objective assessments such as stone free rate. However, as part of the evolution to shift healthcare to a more patient centered position, patient-reported outcome measures have emerged as an improved tool to address this deficit. Key patient-reported outcome measures in Urology now include Ureteral Stent Symptom Questionnaire, Wisconsin Stone QoL Questionnaire, Cambridge Ureteric Stone PROM, Cambridge Renal Stone PROM, and

[Pathogenic factors in calcium oxalate stones: Epidemiological investigation].

INTRODUCTION: Calcium oxalate stones are the most common urolithiasis. Changes in dietary habits, socio-economic and health status of populations explain its progression. The aim of our study was to determine metabolic factors leading to lithiasis and clarify its causes. PATIENTS AND METHODS: This is a retrospective study of 100 patients with calcium oxalate stones identified by morpho-constitutional study, collected in our department over a period of 5 years (2008-2013). We analyzed clinical, radiological and metabolic data. RESULTS: They were 73 men and 27 women (gender ratio: 2.7), aged m…

Implications of vitamin D deficiency in lithiasic patient and in general population.

CONTEXT AND OBJECTIVE: Vitamin D deficiency causes problems in mineral metabolism but also overall health. In first place a review of the topic was carried out. Then, in order to contextualize it in lithiasic patient, a study on Vitamin D deficiency and its possible relationship with impaired PTH levels is performed. EVIDENCES ACQUISITION: A review of topics such as metabolism, epidemiology and the relationship of vitamin D deficiency with several pathologies was performed. Besides a multivariate analysis and a correlation study between vitamin D and PTH levels was conducted in 100 lithiasic

[Retroperitoneoscopic ureterolithotomy].

Ureterolithotomy has an alternative which is much less invasive --retroperitoneoscopic ureterolithotomy (RU). Nineteen cases of RU are reported (15 males, 4 females). Thirteen patients have undergone RU as monotherapy, 5 patients--after ineffective sessions of extracorporeal shock wave lithotripsy, 1 patient--after contact ureterolithotripsy. Mean size of the stones was 12.8 (9.5-30) mm. All the stones were eliminated for one procedure. Duration of surgery in the first 5 operations was 1.5-2 hours, in subsequent ones--65-70 min. The number of bed-days varied from 2 to 10 days (4.2 days). One m

Thlaspi Bursa Pastoris in Treatment of Urinary Stones: A Case Series.

BACKGROUND: Urinary stones are the most common cause of acute ureteral obstruction. The homeopathic medicine Thlaspi bursa pastoris (TBP) has many symptoms similar to the typical symptoms of urolithiasis, yet its clinical usefulness has not been well documented. PATIENTS: In this study, the clinical symptoms of each of five patients, along with ultrasonographic evidence, were taken for the assessment of the outcome. TBP was prescribed in potency 30c and/or 200c to each patient on the basis of the main symptoms, typically dysuria, retention of urine and haematuria. Each case was followed for u…

Calcitriol mediated hypercalcaemia with silicone granulomas due to cosmetic injection.

We present a case of a 41-year-old woman with medical history significant for urolithiasis presenting to our hospital for psychiatric evaluation due to worsening depression and suicidal ideations for the past 2 weeks. Initial laboratory results show hypercalcaemia of 13.5 mg/mL that led to consulting internal medicine. On further questioning, the patient admitted to cosmetic silicone injections in her buttocks which were causing calcium deposition under her skin, leading to disfigurement of the sacrum and lumbar regions. She underwent further evaluation with CT and laboratory testing, which ef…

Guarded long-term prognosis in male cats with urethral obstruction.

The aim of this study was to evaluate the course of urethral obstruction in cats. Forty-five male cats with urethral obstruction or lower urinary tract signs referable to urethral obstruction were included in the study. Follow-up information was gained by telephone interview in most cases and was available in 39 cats. Of the 22 cats with idiopathic urethral obstruction, eight (36%) re-obstructed after 3-728 days (median 17 days). Of 10 cats with urolithiasis, three (30%) re-obstructed after 10, 13 and 472 days, respectively. Of the seven cats with urethral plugs, three (43%) re-obstructed afte…

Clinical evaluation of multimodal environmental modification (MEMO) in the management of cats with idiopathic cystitis.

This prospective observational study evaluated client-reported recurrence of lower urinary tract signs (LUTS) and other signs of abnormalities in cats with idiopathic cystitis after institution of multimodal environmental modification (MEMO). Forty-six client-owned indoor-housed cats with idiopathic cystitis, diagnosed based on a history of recurrent LUTS and evidence of absence of urolithiasis or bacterial urinary tract infection were studied. In addition to their usual care, clients were offered recommendations for MEMO based on a detailed environmental history. Cases were followed for 10 mo…

Extracorporeal shock wave lithotripsy treatment in paraplegic patients with bladder stones.

BACKGROUND: Spinal cord injury patients are at increased risk of developing urolithiasis and many will require treatment, most commonly with extracorporeal shock wave lithotripsy (ESWL). METHODS: We examined 20 consecutive spinal cord injury patients (all male) undergoing ESWL for the treatment of bladder stones between April 1992 and May 2000. The mean number of shock waves administered in these 20 patients was 3600 and the shock power was 19 kW/s. All patients were treated without anesthesia. After ESWL, the bladder was irrigated with povidone iodine solution and stone fragments were evacua…

[Studies of calcium metabolism in patients with hypercalciuria].

The different subgroups of hypercalciuria cannot be separated clearly by the Pak calcium-load test. To improve the diagnosis and therapy we examined all relevant parameters of calcium metabolism in 32 patients with calcium urolithiasis and hypercalciuria (> 6.25 mmol/day). We also conducted bone mineral density measurements as well as the Pak calcium-load test. In most cases the pathophysiological constellations which Pak takes as the basis for his classification of hypercalciuria could not be shown. To date, diagnostics only insufficiently explains the genesis of hypercalciuria (except pHPT).…

The natural history of renal stone disease after parathyroidectomy for primary hyperparathyroidism.

The evolution of renal stone disease has been followed, before and after parathyroidectomy, in 197 patients with primary hyperparathyroidism. Before operation, 120 patients had had a previous history of renal colics or stones, or both, demonstrated on roentgenograms of the urinary tract. In 36 patients with stones that had been passed or removed before exploration of the neck, no recurrence of lithiasis has been observed. In 84 patients who still had stones at the time of the operation, the stones dissolved and disappeared within ten years in 88 per cent of those with urolithiasis and in 77 pe…
