Laser-assisted uvulopalatoplasty: an assessment of a technique.

The Kamami technique of laser-assisted uvulopalatoplasty has recently been introduced into British practice as an office-based multistage procedure for the treatment of snoring and, in certain cases, for obstructive sleep apnoea. We have treated 50 patients with simple snoring, with a minimum follow-up of six months. Six-month telephone interview follow-up has assessed partner and patient satisfaction, as well as willingness to undergo the procedure again. We have found the procedure easy to perform with no serious intra-operative complications. Thirty-four (68 per cent) of patients' partners

Efficacy of cold-steel uvulopalatoplasty.

Seven patients underwent cold-steel uvulopalatoplasty with tissue removal similar to that being performed with laser-assisted uvulopalatoplasty. The severity and length of postoperative pain, days out of work, postoperative complications, and efficacy in reducing snoring were assessed. The minimum follow-up period was 6 months. The average length of postoperative pain (inability to eat a completely normal diet) was 6 days. All but one patient underwent only one session of cold-steel uvulopalatoplasty with satisfactory relief of snoring. The remaining patient required two sessions. One patient

[Polysomnographic results before and after uvulopalatopharyngoplasty (UPPP)].

Uvulo-palato-pharyngo-plastic is one possibility for treatment of obstructive sleep-apnea. We examined in a prospective study design 36 patients, suffering from polysomnographically ensured obstructive sleep-apnea-syndrome, who were undergone uvulo-palato-pharyngo-plastic. We analysed pre- und postoperative polysomnographic data. Most of patients demonstrated a decrease of amount of apneas, duration of apneas and an increase of minimal oxygen-desaturation. Patients who had high amounts of apneas (AI > 40/h) or who had a Broca-index > 140 p.c. gained not as much from surgical treatment as other

[Effects of uvulopalatopharyngoplasty in adult patients with obstructive sleep apnea syndrome].

We performed uvulopalatopharyngoplasty (UPPP) in 51 adult patients with obstructive sleep apnea syndrome (OSAS). After UPPP, there were statistically significant improvements in apnea-hypopnea index (AHI), the lowest value of oxygen saturation during sleep and total time of apnic episodes. 28 patients (54.9%) were good responders who represented more than 50% improvements in their AHI. Poor responders tended to be more severe and older than good responders. After cephalometric analysis, poor responders were revealed to show significantly poor mandibular prognatism and also lower positioned hyo

Nasopharyngeal complications following uvulopalatopharyngoplasty.

This report presents our experience with nasopharyngeal complications of UPPP in 85 patients undergoing the procedure from May, 1982 to January, 1985. Three patients developed nasopharyngeal stenosis and one patient developed permanent velopharyngeal insufficiency. Surgical management in two patients with nasopharyngeal stenosis resulted in adequate nasopharyngeal airway, while one patient still has a moderate stenosis following two surgical procedures. The patient with velopharyngeal insufficiency underwent Teflon paste injection in the posterior pharyngeal wall. This resulted in complete all

Obstructive sleep apnea treated with uvulopalatopharyngoplasty: a systematic follow-up study.

We evaluated ten consecutive cases of obstructive sleep apnea treated with uvulopalatopharyngoplasty (UPPP). The preoperative all-night polysomnograms were compared to those done one to three months after surgery. Excessive daytime sleepiness, the main complaint of every patient, partially improved in three and was completely abolished in seven. The lowest oxygen saturation levels were below 80% in nine cases before UPPP and in only three afterward. The number of apneic episodes per hour of sleep, or apnea index, decreased in every patient and reached normal levels in five. Improvement of othe

Palatal Procedures for Obstructive Sleep Apnea.

Uvulopalatopharyngoplasty was the first surgical procedure described, other than tracheostomy, for the treatment of obstructive sleep apnea (OSA) in 1981. It was recognized then that there were responders who were cured of OSA with the procedure and others that were not. It took many years for a staging system to be described that categorized patients based on Mallampati score, tonsil size, and body mass index to better predict success rates. It was recognized that individuals with retro-palatal obstruction as the cause of the airway obstruction responded well but that the morbidity associated

Managing snoring: when to consider surgery.

Snoring can range in significance from merely annoying the patient's bed partner to being a symptom of obstructive sleep apnea, a risk factor for heart disease and stroke. If conservative measures do not help, primary care physicians can refer patients for consideration of a variety of surgical procedures to keep the airway open during sleep.

The characteristics of five patients with obstructive sleep apnea whose apnea-hypopnea index deteriorated after uvulopalatopharyngoplasty.

The objective of this retrospective, consecutive, case series design study was to determine the number of unselected patients with obstructive sleep apnea (OSA) who deteriorated after uvulopalatopharyngoplasty (UPPP). Sixteen of 27 patients at the Sleep Clinic at Veterans Affairs Medical Center who underwent UPPP for OSA and who completed both a pre- and postpolysomnogram were studied. After comparing the apnea-hypopnea index (AHI) before and after UPPP, three groups of patients were identified: deteriorators, unchanged, and improvers. In five patients (31%), the AHI increased by more than 10%
