Acute and long-term efficacy and safety of catheter cryoablation of the cavotricuspid isthmus for treatment of type 1 atrial flutter.

BACKGROUND: Atrial flutter (AFL) is commonly treated by radiofrequency catheter ablation. Catheter-based cryoablation may be an effective alternative with potential advantages. OBJECTIVE: The purpose of this study was to study the acute and long-term safety and efficacy of catheter-based cryoablation for treatment of cavotricuspid isthmus-dependent (typical and reverse typical) AFL. METHODS: Catheter-based cryoablation was performed with a 10Fr catheter in 160 patients with cavotricuspid isthmus-dependent AFL (122 men and 38 women; mean age 63.1 +/- 9.3 years, mean left ventricular ejection

When Sleep and Rhythm Collide: Amiodarone, Obstructive Sleep Apnoea, and Sinus Rhythm Maintenance After Cardioversion.

BACKGROUND: Recurrence of atrial fibrillation (AF) and atrial flutter (AFL) after direct current cardioversion (DCCV) remains a common problem. Several predictors of recurrence have been identified in observational studies. Current guidelines recommend considering amiodarone as an adjunct in patients at high risk of AF/AFL recurrence. However, data in the available literature on the effectiveness of amiodarone in restoring and maintaining sinus rhythm (SR) are sparse. METHODS: This observational cross-sectional study analysed retrospective data from 193 patients who underwent elective DCCV fo

Association of proportional pulse pressure with cardiac function and complications in patients hospitalized with acute exacerbation of chronic heart failure.

BACKGROUND: Proportional pulse pressure (PPP), calculated as pulse pressure divided by systolic blood pressure, is a simple, non-invasive hemodynamic index. Previous studies suggest its potential role in assessing cardiac function and prognosis in heart failure (HF) patients, particularly in identifying low cardiac output. This study aimed to investigate the relationship between PPP and key cardiac functional indices (Cardiac Index [CI] and Cardiac Output [CO]) in patients with hospitalized with acute exacerbation of chronic heart failure (CHF). METHODS: This cross-sectional study included 80

Outcomes Among Drug Refractory Patients With Atrial Fibrillation (AF) and Concomitant Heart Failure With Preserved Ejection Fraction (HFpEF) Treated With Catheter Ablation Versus Antiarrhythmic Drugs.

INTRODUCTION: Heart failure with preserved ejection fraction (HFpEF) accounts for about half of the cases of heart failure. HFpEF and atrial fibrillation (AF) often coexist and share similar risk factors. The objective of this study is to examine the differences in risk of clinical outcomes among drug refractory AF patients with concomitant HFpEF treated with catheter ablation (CA) versus antiarrhythmic drugs (AADs). METHODS: Using the Optum PanTher Electronic Health Record (EHR) database, adult HF patients with left ventricular ejection fraction (LVEF) ≥ 50% and an AF diagnosis with a histor

Shortest pulmonary vein atrial fibrillation cycle length identifies pulmonary vein isolation responders beyond clinical atrial fibrillation pattern: the FARS-AF II study.

AIMS: Atrial fibrillation cycle length (AF-CL) measured in the pulmonary veins (PVs) with a novel simple method [the average of the 10 consecutive Fastest Atrial Repetitive Similar signal interval (FARS10)] accurately identified pulmonary vein isolation (PVI) responders in a preliminary study. This study aims to evaluate differences in PV-FARS10 between paroxysmal and persistent AF and to define the optimal cut-off to predict PVI-only approach success in a large population. METHODS AND RESULTS: We prospectively enrolled consecutive patients with persistent or paroxysmal AF undergoing first PV

Osilodrostat Safety Profile: Findings from Real-World Data in the FAERS Database.

Background/Objectives: Cushing's syndrome (CS), including Cushing's disease (CD)-the most common type-has a substantial negative impact on morbidity, mortality, and patients' quality of life. Medical management of CS is essential for controlling hypercortisolism as part of preoperative preparation for definitive surgical treatment and for managing residual or relapsed hypercortisolism post-surgery. Osilodrostat, a dual inhibitor of glucocorticoid and mineralocorticoid biosynthetic pathways, has been approved for the medical treatment of CS since early 2020. However, real-world data on its adve

Long-term risk of adverse cardiovascular outcomes associated with cutaneous lupus erythematosus: a nationwide cohort study.

BACKGROUND: Autoimmune diseases, including systemic lupus erythematosus, have been associated with a substantial risk of cardiovascular morbidity and mortality. However, data on the long-term risk of incident heart failure and other adverse cardiovascular outcomes among patients diagnosed with cutaneous lupus erythematosus (CLE) are limited. METHODS: In this cohort study, all patients ≥ 18 years with newly diagnosed CLE between 1996 and 2018 were identified through Danish nationwide registries and matched 1:4 by age, sex, and comorbidity with individuals without CLE. Incident adverse cardiova

Thyrotoxic Periodic Paralysis and Cardiomyopathy in a Patient with Graves' Disease.

Thyrotoxic periodic paralysis (TPP) and cardiomyopathy are two established complications of thyrotoxicosis. Emergent management is essential as TPP and cardiac events secondary to thyrotoxic cardiomyopathy can be fatal. We report a unique case of a patient with Graves' disease presenting with symptoms secondary to both these complications. A 34-year-old Hispanic male, diagnosed with Graves' disease, non-compliant with his medications, presented to the emergency room (ER) with complaints of generalized weakness, palpitations, chest pain and multiple episodes of nausea and vomiting for one day.

Outcomes among athletes with arrhythmias and electrocardiographic abnormalities: implications for ECG interpretation.

Electrocardiographic (ECG) aberrations and arrhythmias occur frequently among athletes due to normal variants, subclinical cardiac disease or structural and electrical remodeling in response to training. It is unclear whether these changes are associated with adverse clinical outcomes over time among otherwise asymptomatic, healthy athletes. Consensus guidelines have been developed to guide the clinician regarding further management of these arrhythmias. The purpose of this review is to summarize prospective data regarding cardiovascular outcomes related to ECG changes among athletes and compa

Low-energy cardioversion of spontaneous atrial fibrillation. Immediate and long-term results.

BACKGROUND: Recent studies have suggested that induced atrial fibrillation (AF) could be successfully terminated by using a two-catheter electrode system and low energy (< 400 V). This study evaluated the efficacy and safety of low-energy cardioversion in spontaneous chronic and paroxysmal AF. METHODS AND RESULTS: Forty-two consecutive patients with spontaneous AF underwent low-energy electrical cardioversion. AF was chronic (> or = 1 month) with a mean duration of 9 +/- 19 months in 28 patients (group I) or paroxysmal with a history of recurrent attacks and a mean duration of the present epi
