Management of 210 patients with benign paroxysmal positional vertigo: AMC protocol and outcomes.

CONCLUSION: Benign paroxysmal positional vertigo (BPPV) could be treated with specific maneuvers for the involved canals. Management on a daily basis gave earlier relief than weekly treatment, especially in apogeotropic BPPV. Apogeotropic and multi-canal BPPVs were related to the increased numbers of follow-ups and trauma was related to the increased recurrence. OBJECTIVE: We aimed to report the short-term outcomes of our protocol for BPPV and evaluate the factors influencing the results and recurrence. METHODS: In 210 BPPV patients, posterior semicircular canal (PSCC), geotropic and apogeot…

[Epilepsy, vertigo, dizziness, headache, emesis as neurological manifestation in a Giteleman's Sindrome case].

We report the case of a woman presenting the recent onset of multiple seizure and epilepsy episodes combined with other neurological symptoms (e.g. vertigo, dizziness, vomiting, headache). She was resistant to antiepileptic and symptomatic therapy, having been first admitted to the neurology ward and subsequently to the general medicine ward. In this case, several patient assessments and imaging exams were not conclusive evidence of specific etiopathogenesis, or definitive neurological illness; however, the patient showed laboratory indexes compatible with Gitelmans Syndrome. The correction of…

Small strokes causing severe vertigo: frequency of false-negative MRIs and nonlacunar mechanisms.

OBJECTIVE: Describe characteristics of small strokes causing acute vestibular syndrome (AVS). METHODS: Ambispective cross-sectional study of patients with AVS (acute vertigo or dizziness, nystagmus, nausea/vomiting, head-motion intolerance, unsteady gait) with at least one stroke risk factor from 1999 to 2011 at a single stroke referral center. Patients underwent nonquantitative HINTS "plus" examination (head impulse, nystagmus, test-of-skew plus hearing), neuroimaging to confirm diagnoses (97% by MRI), and repeat MRI in those with initially normal imaging but clinical signs of a central lesi…

Lateral medullary infarction presenting as isolated vertigo and unilateral loss of visual suppression.

Isolated vertigo is rare in lateral medullary infarction. We described early diagnostic challenges in such cases by a neuro-otological approach. We report a 56-year-old man who developed a lateral medullary infarction that presented as isolated vertigo. Before the day 4 from disease onset when diffusion-weighted magnetic resonance imaging (MRI) became positive, this patient showed unilateral loss of visual suppression, a central type of vestibular dysfunction. Since MRI abnormalities may not appear in the early few days from disease onset, unilateral loss of visual suppression might become an …

Prolonged vertigo and ataxia after mandibular nerve block for treatment of trigeminal neuralgia.

Common complications of neurolytic mandibular nerve block are hypoesthesia, dysesthesia, and chemical neuritis. We report a rare complication, prolonged severe vertigo and ataxia, after neurolytic mandibular blockade in a patient suffering from trigeminal neuralgia. Coronoid approach was used for right sided mandibular block. After successful test injection with local anesthetic, absolute alcohol was given for neurolytic block. Immediately after alcohol injection, patient developed nausea and vomiting along with severe vertigo, ataxia and hypertension. Neurological evaluation was normal except…

[Benign paroxysmal positional vertigo: analysis of our population and role of caloric tests].

INTRODUCTION AND OBJECTIVES: Benign paroxysmal positional vertigo (BPPV) is the most common cause of vertigo in those patients who seek medical care. Although videonystagmography seems the most indicated diagnostic test, we tried to decrease the requested caloric tests to optimise resources and reduce associated costs. METHODS: We developed a diagnostic-therapeutic algorithm for patients with suspected BPPV whose provocation tests are positive to evaluate the need for caloric testing. We analysed the results of its application on 98 patients who were referred to our Neuro-otology Unit over 15…

Protracted benign paroxysmal positional vertigo following osteotome sinus floor elevation: a case report.

Benign paroxysmal positional vertigo (BPPV) is a relatively rare condition characterized by onset of rotation dizziness triggered by head movements or change in posture. BPPV etiology includes head injury, infection, vascular disorders, surgical trauma, and idiopathic events. This report presents a case of protracted BPPV following osteotome sinus floor elevation and simultaneous implant placement. A 49-year-old female suffered intense vertigo and nausea immediately after implant placement using an osteotome sinus floor elevation procedure, especially when changing head position while sitting …

How often is dizziness from primary cardiovascular disease true vertigo? A systematic review.

OBJECTIVES: To assess how frequently cardiovascular dizziness is vertigo. Recent studies suggest providers do not consider cardiovascular causes when a patient reports true vertigo (spinning/motion) as opposed to presyncope (impending faint). It is known that cardiovascular disease causes dizziness, but unknown how often such dizziness is vertiginous, as opposed to presyncopal. DATA SOURCES: Systematic review of observational studies was made: Search--electronic (MEDLINE, EMBASE) and manual (references of eligible articles) search for English-language studies (1972-2007). Efficacy of a new treatment maneuver for posterior canal benign paroxysmal positional vertigo.

Existing treatment maneuvers for posterior canal benign paroxysmal positional vertigo (PC-BPPV) include the Semont liberatory maneuver (SLM) and canalith repositioning maneuver (CRM). Independent investigations reveal that these maneuvers provide an excellent outcome for most patients. However, certain aspects of these maneuvers, such as hyperextension of the neck for CRM and brisk lateral motion for the SLM, are contraindicated for patients with vertebrobasilar insufficiency, cervical spondylosis, back problems, and so forth. A hybrid approach, the Gans repositioning maneuver (GRM) was develo…

Positional vertigo and macroscopic downbeat positioning nystagmus in spinocerebellar ataxia type 6 (SCA6).

To investigate the frequency of positioning nystagmus in degenerative ataxic disorders, we examined downbeat positioning nystagmus (DPN) in 25 patients with spinocerebellar ataxia type 6 (SCA6) and 58 patients with other types of degenerative ataxia. DPN was observed in 21 of the 25 patients with SCA6 (84 %) versus only 3 of the 58 patients (5.2 %) with other types of degenerative ataxia, including multiple system atrophy, SCA1, SCA2, SCA3/Machado-Joseph disease, and non-SCA6 late-onset pure cerebellar ataxia. Our findings indicated that DPN is a distinct part of the clinical presentation of S

Mechanism of posterior semicircular canal stimulation in patients with benign paroxysmal positional vertigo.

A quantitative study of the stimuli and vestibulo-ocular response associated with benign paroxysmal positional vertigo (BPPV) was made to test and further develop the canalithiasis theory of BPPV. The angular velocities of the head in the planes of the semicircular canals during the Dix-Hallpike test were measured in four healthy subjects using electromagnetic sensors to record the position of the head in a six degrees of freedom paradigm. Next, the nystagmus reactions in seven patients diagnosed with idiopathic BPPV were recorded with video-oculography. The characteristics of the vestibulo-oc…

Electrocochleography in Retrosigmoid Vestibular Nerve Section for Intractable Vertigo Caused by Meniere's Disease.

Interest in electrocochleography has increased in recent years because of the discovery of an elevated summating potential to action potential amplitude ratio (SP/AP ratio) in patients with endolymphatic hydrops caused by Meniere's disease or perilymph fistula. It was the purpose of this investigation to determine whether the intraoperative SP/AP ratio will decrease after vestibular nerve section in patients with intractable Meniere's disease. Fourteen patients with medically intractable classic Meniere's disease underwent retrosigmoid vestibular nerve section. Intraoperative transtympanic ele

[Sudden vertigo--differential diagnosis and therapy in peripheral vestibular (inner ear-induced) forms].

The most important forms of inner ear disorders manifesting as sudden vertigo may be distinguished by the duration of the vertiginous spells: sudden vertigo, diminishing during days = acute peripheral vestibulopathy, recurrent spells lasting for hours = Ménière's disease, recurrent vertigo lasting for minutes = "vascular', recurrent positioning-induced vertigo lasting for seconds = benign paroxysmal positioning vertigo (BPPV). In the last years, major progress was made in the treatment of BPPV. Based on new ideas on the underlying pathomechanism, this most common form of vestibular vertigo now…

[Differential diagnosis of vertigo in functional cervical vertebrae joint syndromes and vertebrobasilar insufficiency].

The disequilibrium caused by a functional disturbance of the upper cervical spine is compared with the disequilibrium caused by the vertebrobasilar insufficiency. Anamnesis, otoneurological and manual examination enable the differentiation of the cervical vertigo, what is not only of theoretical interest but above all has therapeutic consequences.
