Knowledge and Attitudes of Medical Students in Palliative Medicine.

BACKGROUND: Primary palliative care (PPC) is a specialized field that focuses mainly on the quality of life through symptom management, goal setting/advanced care planning, and psychosocial support in patients with serious illnesses. However, it is underutilized due to misconceptions about its scope and how to provide it. There remains great variability in the preparation of residents and providers in palliative medicine. OBJECTIVE: This study assessed medical students' level of knowledge and attitudes about PPC concepts. METHODS: A survey was adapted from previously validated questionnaires

Ethical challenges in sensitive research: a reflective narrative on managing the clinician-researcher dual role.

BACKGROUND: Conducting sensitive research in healthcare settings often presents complex ethical challenges, particularly when clinicians assume a dual role as researchers. This dual role, while providing valuable insight, can lead to ethical dilemmas when professional responsibilities intersect with research obligations, especially in resource-limited and culturally sensitive environments. This reflective narrative explores these ethical tensions through the lens of a qualitative study involving men living with advanced prostate cancer and their family caregivers. METHOD: The study employed a

Acute Generalized Exanthematous Pustulosis Induced by Hyaluronic Acid Knee Injection: A Case Report.

Acute generalized exanthematous pustulosis is a severe cutaneous adverse reaction characterized by the rapid onset of nonfollicular, sterile pustules on an erythematous base, typically accompanied by fever (≥38 °C), neutrophilia (7.0 × 10⁹/L), and characteristic histopathological features. This case report presents the first documented instance of acute generalized exanthematous pustulosis after hyaluronic acid viscosupplementation. A 61-year-old female developed a pruritic, erythematous rash that rapidly evolved into generalized erythroderma with systemic manifestations after receiving intra-

Building medication for opioid use disorder prescriber capacity during the opioid epidemic: Prescriber recruitment trends and methods.

INTRODUCTION: Physicians are a critical clinical resource for patient care. Yet physician recruitment has been considerably understudied, particularly in substance use disorder (SUD) settings. This study proposes a conceptual model called the "Physician Recruitment Descriptive Factors Framework" to investigate the role of environmental, organizational, and individual factors in the use of physician recruitment strategies. METHODS: The study setting was 75 sites that provided outpatient SUD treatment services in Florida, Ohio, and Wisconsin from 2016 to 2019. Central to the analysis is the use

Perceived Barriers to Peritoneal Dialysis Among Kenyan Nephrologists: A Cross-Sectional Descriptive Study.

BACKGROUND: Peritoneal dialysis (PD) is a well-recognized technique of renal replacement therapy (RRT), with similar efficacy as well as survival outcomes as hemodialysis (HD). Despite its advantages including prolonged preservation of residual renal function, potentially lower cost and advances with automated techniques, and commercialization of more biocompatible solutions, the overall prevalence of patients treated with PD is still very low in developed countries and even more so in Africa and low-middle income countries like Kenya. According to our knowledge, no local studies have been don

A Survey of Provider-Reported Use and Perceived Effectiveness of Medications for Symptom Management in Telemedicine and Outpatient Visits for Mild COVID-19.

INTRODUCTION: Many patients with mild coronavirus disease 2019 (COVID-19) have symptoms requiring acute and follow-up care. The aims of this study were to assess (1) provider-reported use of medications and their perceived effectiveness and (2) degree of difficulty managing specific symptoms at episodic COVID-19 care sites and in a longitudinal monitoring program. METHODS: We sent an online survey to physicians, advanced practice providers, and registered nurses redeployed to COVID-19 care sites at an academic medical center from March to May 2020. We asked about the use of medications and pe

An international survey to assess referral thresholds for destination therapy in non-inotrope-dependent patients: results of the CONSENSUS-DT study.

BACKGROUND: Criteria for the identification of patients who may progress to advanced heart failure (HF) have been previously suggested. However, identification of appropriate non-inotrope-dependent (NID) patients for destination therapy (DT) remains a challenge, and referral rates are low. We surveyed expert provider opinion about patient selection for early DT. METHODS: An internet-based survey was sent to international HF providers to investigate opinions about clinical indicators for referral of NID patients for DT. Subjects were identified from membership lists of HF professional organiza

From the set-up of a screening program of breast cancer patients to the identification of the first BRCA mutation in the DR Congo.

BACKGROUND: Breast cancer incidence in African population is low compared to western countries but the mortality rate is higher and the disease presents at a younger age and at a more advanced stage. The World Health Organisation and the Breast Health Global Initiative concluded that in low and middle income countries early breast cancer detection can be achieved by informing women on symptoms of breast cancer, on the practice of breast self-examination and clinical breast examination by trained health care workers. Based on these recommendations, we set up a breast cancer awareness campaign i

The discharge process for a child with complex healthcare needs dependent on respiratory technology: A scoping review protocol.

BACKGROUND: Children with complex healthcare needs spend a long time in hospital with frequent delays in discharge. This may be due to fluctuating health status, or contextual factors such as training carers, social circumstances, suitable housing, and community resources such as homecare nurses. Understanding the process for an effective discharge for children with complex healthcare needs who are respiratory technology dependent is essential to guide clinical practice, and shape policy that supports timely, coordinated, and family-centered transitions from hospital to home and inform evidenc

Governance frameworks and decision-making models for intensive care units in low- and middle-income countries: a scoping review protocol.

INTRODUCTION: Intensive care units (ICUs) operate at the intersection of advanced technology, complex ethical decision-making and resource-intensive care. While many high-income countries have developed institutional governance mechanisms-such as Clinical Ethics Committees, structured triage policies and formal palliative care consultation pathways-to support ethical deliberation and accountability in ICUs, the applicability, structure and reported outcomes of such frameworks in low- and middle-income countries (LMICs) remain unclear and dispersed across disciplines. OBJECTIVE: This scoping r

Holistic management of epilepsy in adults with intellectual development disorders.

This seminar addresses the complexity of the management of epilepsy in adults with intellectual development disorders (IDD), advocating holistic and multidisciplinary care aligned with the learning objectives of the International League Against Epilepsy. Epilepsy is significantly more prevalent in people with IDD, presenting unique diagnostic, therapeutic, and psychosocial challenges. Accurate diagnosis is hampered by limitations in communication, necessitating reliance on caregivers' observations. In adults with IDD and epilepsy, utilization of diagnostic tools, e.g., video electroencephalogr

The Current Landscape of Pancreatic Cancer Management in Sub-Saharan Africa - A Perspective Review.

BACKGROUND AND AIMS: Pancreatic cancer remains a significant public health challenge in Sub-Saharan Africa (SSA), with a high mortality-to-incidence ratio driven by late-stage diagnoses, limited therapeutic access, and systemic healthcare disparities. This perspective aims to synthesize current evidence on the epidemiology, diagnostic capacity, treatment options, and structural barriers contributing to poor outcomes, while highlighting opportunities for targeted interventions and policy reforms. METHODS: We conducted a narrative review of peer-reviewed literature, regional reports, and epidem

Post-Intensive Care Syndrome Awareness and Communication: Surveys of ICU Providers and Patients.

BACKGROUND: Survivors of critical illness often experience new or worsening impairments in various domains of health after discharge, collectively referred to as post-intensive care syndrome (PICS). Although this condition is common, it remains unclear whether providers are communicating routinely about survivorship and PICS to patients and families and whether patients are remembering these conversations. RESEARCH QUESTION: How often do ICU providers discuss the concept of PICS with at-risk patients or families, and how often do patients remember being told about the concept of PICS? STUDY
